Palliative Management of Advanced Pancreatic Cancer: The Role of Gastroentero-hepatologist.

Pancreatic cancer commonly diagnosed at late stage due to subtle clinical manifestation and associated with low 5-year survival rate. Only 10-20% of patients were found in resectable or localized stage. Several complications may arise due to advanced pancreatic cancer such as obstructive jaundice, gastric outlet obstruction, pancreatic cancer cachexia, pruritus of cholestasis, and cancer pain. Palliative management should be optimized in order to improve patient's quality of life. A gastroentero-hepatologist should collaborate with other specialties to give comprehensive palliative care for ad

The use of human albumin in patients with cirrhosis: a European survey.

BACKGROUND: The administration of human albumin (HA) in patients with decompensated cirrhosis is still debated. The European Foundation for the Study of Chronic Liver Failure (EF-CLIF) promoted an online survey to assess its use across Europe. METHODS: Hepatologists were invited to participate to an electronic questionnaire based on multiple-choice questions divided in 6 different areas. A descriptive statistical analysis was performed to analyze the responses. RESULTS: One hundred-one hepatologists (36% non-EF-CLIF member), belonging to 86 centers (25% non-academic hospitals) completed the

Surgical decision-making in the management of children with intractable functional constipation: What are we doing and are we doing it right?

BACKGROUND: Children with intractable functional constipation (FC) may eventually require surgery, often guided by motility testing. However, there are no evidence-based guidelines for the surgical management of intractable FC in children. AIM: To assess the diagnostic and surgical approach of pediatric surgeons and pediatric gastroenterologists towards children with intractable FC. METHODS: A survey was administered to physicians attending an international conference held simultaneously in Columbus (Ohio, USA) and Nijmegen (the Netherlands). The survey included 4 questions based on cases wi…

Pioneering Endoscopic Submucosal Dissection in the West: Technique Introduction With a Comprehensive Case Report on the First Application for Oropharyngeal Squamous Cell Carcinoma.

Endoscopic submucosal dissection (ESD) for pharyngeal squamous cell carcinoma (SCC) has predominantly been performed in Asia but interest in this procedure is growing in the West. This report details the first known Western application of ESD for oropharyngeal SCC. A Caucasian female with superficial oropharyngeal SCC detected during endoscopic surveillance post-ESD for esophageal SCC underwent successful en-bloc resection without complications. Pathology confirmed SCC in situ with tumor-free margins. This case underscores the importance of mindset to include the pharynx in upper gastrointesti

Iatrogenic rectal perforation after coccygeal radiofrequency ablation.

Coccygodynia (pain of the coccygeal bone) can be treated locally with anti-inflammatory drugs, local steroid injections, surgical removal of the coccyx and, more recently, with radiofrequency thermal ablation. Complications, such as perforation of the colon, can occur as a consequence of the close relationship between the rectum and the sacrococcyx and with the heat from the thermal ablation expanding to the surrounding tissue causing delayed damage with severe consequences. The treatment of this complication requires the combined effort of the gastrointestinal surgeon as well as a gastroenter

Iliac Artery-Inferior Vena Cava-Duodenal Fistula: An Unusual Cause of Massive Gastrointestinal Bleeding.

An arteriovenous-enteric fistula is a 3-way connection between the vascular and enteric system and associated with high mortality. We describe a case of iliac artery-inferior vena cava-duodenal fistula in a young female with a retroperitoneal mass presenting with sepsis and hemorrhagic shock with a catastrophic clinical course. These fistulas can be missed on endoscopy/colonoscopy and are usually diagnosed on computed tomography angiogram of the abdomen. Complex vasculoenteric fistula should be among differentials in patients presenting with gastrointestinal bleeding, especially with a history

[Anal complaints; article for education and training purposes].

Anal complaints are common, often disabling and may have a big impact on quality of life. Treatment is aimed at improving underlying conditions and starts with recognizing symptoms. Diagnosis can be made by taking a structured clinical history and meticulous perianal examination with rectal exam. Anorectal disorders can be effectively treated in general practice with primarily conservative measures like improving life style, toilet habits and pelvic physiotherapy. Symptoms can further be relieved by correct anal hygiene and topical ointments. Follow up is essential to evaluate response and adj

Team Approach: Complex Regional Pain Syndrome in Children and Adolescents.

Children and adolescents with chronic pain are best managed by an interdisciplinary team. In cases of complex regional pain syndrome (CRPS), the interdisciplinary team consists of a pain management specialist, such as an anesthesiologist or physiatrist, a physical therapist, an occupational therapist, a pain psychologist, and an orthopaedist. It may also include other professions, such as a gastroenterologist, psychiatrist, nurse practitioner, nutritionist, endocrinologist, acupuncturist, or social worker. Key factors include rapid recognition of CRPS and the initiation of appropriate treatmen

Recommendations by the Spanish Working Group on Crohn's Disease and Ulcerative Colitis (GETECCU) on the treatment of patients with inflammatory bowel disease associated with spondyloarthritis.

Extraintestinal manifestations, in general, and in particular arthropathies, are a common problem in patients with inflammatory bowel disease. In fact, the relationship between those 2entities is close and there are increasingly more data which suggest that the bowel plays a significant role in the aetiopathogenesis of spondyloarthritis. The association of inflammatory bowel disease with any kind of spondyloarthritis represents a challenging clinical scenario. It is therefore necessary that both gastroenterologists and rheumatologists work together and establish a fluent communication that ena…

Evaluation and reporting of biopsies from the columnar-lined esophagus and gastro-esophageal junction (GEJ).

This review is part of a collaboration between the American Registry of Pathology (the publisher of the Armed Forces Institute of Pathology Fascicles) and Annals of Diagnostic Pathology. It is in a series of expert recommendations on topics encountered in daily practice. The authors, two pathologists and a gastroenterologist, met on 19 January 2019 tasked with developing expert recommendations on reporting biopsies from the columnar lined esophagus and gastroesophageal junction. Our opinions for reporting revolve around noting the presence and absence of goblet cells and clues for confirming w

Identifying Gaps and Disparities in Screening for Cystic Fibrosis Associated Liver Disease: Insights From a CF Center Analysis.

BACKGROUND: New 2023 CF liver disease (CFLD) guidelines advocate for additional screening in people with cystic fibrosis (PwCF), including biennial abdominal ultrasound. As a first step towards effective and equitable guidelines implementation, we examined our current practice of CFLD screening and hepatobiliary involvement (HBI) evaluation. We identified characteristics of PwCF at-risk for incomplete screening and factors affecting evaluation. METHODS: We retrospectively reviewed medical records of PwCF aged 0-21 years, with native liver and ≥ 2 outpatient CF clinic visits 2017-2023. Logisti
