2025 Practice-changing articles in trauma, emergency general surgery, pediatric trauma, and surgical critical care: an EAST narrative review.

Trauma, acute care surgery, and surgical critical care clinicians routinely manage patients in whom small differences in timing, technique, and patient selection can meaningfully influence outcomes. Each year, emerging literature refines clinical decision-making across trauma surgery, emergency general surgery, pediatric trauma, and surgical critical care. A narrative review of the literature was conducted with primary emphasis on studies published in 2025. Selected high-impact articles from 2024 were included when they directly informed contemporary practice. Articles were identified through …

Large Language Models as Examinees and Graders in Simulated General Surgery Oral Board-Style Cases: A Psychometric Pilot Study.

INTRODUCTION: Surgical oral board examinations are vulnerable to examiner variability, potentially compromising scoring reliability. This study evaluated six public frontier large language models (LLMs) as both examinees and graders on general surgery oral board-style cases. METHODS: Six commercially available LLMs were assessed across three standardized cases using a fully crossed psychometric design. Responses were graded on a 0-3 ordinal scale by six LLMs and three blinded senior surgeons using a structured rubric with anchored descriptors for each score level. Analyses included performanc…

A Society of Thoracic Surgeons General Thoracic Surgery Database Analysis of the Association Between Lymph Node Dissection and Chylothorax.

BACKGROUND: Guideline-concordant lymph node (LN) dissection may increase chylothorax risk after lung resection. We analyzed predictors of chylothorax, with a focus on the extent and pattern of LN dissection, using The Society of Thoracic Surgeons General Thoracic Surgery Database. METHODS: We studied patients who underwent elective lung resection for stage I to III non-small cell lung cancer from January 2015 to December 2024 (N = 152,823). The primary end point was 30-day postoperative chylothorax. Independent predictors of chylothorax were identified using multivariable logistic regression.

Recovery quality with regional anesthesia and dexmedetomidine sedation versus general anesthesia for ambulatory breast cancer surgery: A randomized trial.

BACKGROUND: Advances in regional anesthesia techniques and the widespread use of ultrasound guidance have enabled safe and effective anesthesia for ambulatory breast cancer surgery, eliminating the general anesthesia-associated systemic effects. We therefore compared regional anesthesia with dexmedetomidine sedation to general anesthesia on the 15-item postoperative quality of recovery (QoR-15). METHODS: Ninety-six patients scheduled for breast-conserving surgery and sentinel lymph node biopsy were randomly assigned to regional anesthesia with sedation or general anesthesia. In patients assig

Clinical Outcomes in Trans-sphenoidal Surgery for Pituitary Neuroendocrine Tumour in Sarawak General Hospital, Malaysia: Microscopic Versus Endoscopic Approach.

PURPOSE: Trans-sphenoidal surgery is the standard approach to provide direct access to pituitary neuroendocrine tumours (PitNET). Reported outcomes for trans-sphenoidal surgical resection of PitNET were heterogeneous. This work aimed to develop local data for the outcomes of trans-sphenoidal surgery, either via a microscopic or endoscopic approach. METHODS: We performed a retrospective review of patients with PitNET treated by trans-sphenoidal surgery at Sarawak General Hospital from 2014-2021. We included only cases treated by the senior author (AWSH) to avoid inter-surgeon variability. Demo

General anesthesia management for oral surgery in a patient with plastic bronchitis associated with Fontan circulation: a case report.

Plastic bronchitis is a rare condition in which mucus plugs obstruct the bronchi, potentially leading to fatal respiratory failure. It has been reported in some patients with congenital heart disease following Fontan surgery. We report the general anesthesia management of a 22-year-old female patient with Fontan circulation and type II plastic bronchitis that was controlled using regular intravenous heparin injections. In this case, concerns existed regarding airway obstruction by bronchial plugs and hemodynamic instability specific to the Fontan circulation. During endotracheal intubation, th

Intraoperative Hypotension in Patients Having Major Noncardiac Surgery Under General Anesthesia: A Systematic Review of Blood Pressure Optimization Strategies.

INTRODUCTION: Intraoperative hypotension is associated with increased risks of postoperative complications. Consequently, a variety of blood pressure optimization strategies have been tested to prevent or promptly treat intraoperative hypotension. We performed a systematic review to summarize randomized controlled trials that evaluated the efficacy of blood pressure optimization interventions in either mitigating exposure to intraoperative hypotension or reducing risks of postoperative complications. METHODS: Medline, Embase, PubMed, and Cochrane Controlled Register of Trials were searched fr

Value and relevance of routine postoperative blood sample analysis after general surgery-a single centre analysis of 1198 patients.

BACKGROUND: Routine blood sample analysis is performed in most surgical institutions after general surgery on the first postoperative day. Substantial economical impact is thereby generated in addition to patient distress caused by venous puncture. AIM: The presented study was designed to analyse the relevance of postoperative routine blood sampling with special focus on patient safety. METHODS: 1198 patients undergoing minor general surgical procedures (appendectomy, cholecystectomy, groin hernia repair) at our institution were retrospectively analysed. Data was gathered and forwarded to st…

Abbreviated laparotomy (damage control) in emergency general surgery: indications, risks and resource-based applications in the South African context.

"Damage-control" surgery originated in trauma care, emphasising expeditious control of haemorrhage and contamination to avert physiological collapse. This concept has extended into emergency general surgery (EGS) for conditions such as peritonitis, bowel ischaemia, and abdominal catastrophes. Mortality in EGS may reach up to 17% in affluent settings. The precise role of abbreviated laparotomy remains ill-defined, especially in resourcediverse environments such as South Africa. Controversies persist regarding its timing and technique - particularly decisions around deferred versus immediate bow…

Impact of early postoperative oral hydration in paediatric patients undergoing elective surgery after general anaesthesia. A randomised controlled trial.

BACKGROUND AND AIMS: Early feeding or oral hydration post-surgery in paediatric patients has conventionally not been encouraged, with fasting periods ranging from 3 to 6 h. The primary objective of this study was to compare the Face, Legs, Activity, Cry, Consolability (FLACC) score as a surrogate for patient satisfaction in younger children between those who received early versus delayed postoperative feeding/hydration. Secondary objectives were to compare the incidence of complications, perioperative clinical parameters and parental satisfaction. METHODS: A single-blind, randomised trial was

Rigid bronchoscopy and Y-stent for severe central malignant airway obstruction as a bridge to surgery requiring general anaesthesia.

A female patient in her 30s with a history of asthma presented with a chronic cough, worsening dyspnoea, dysphagia and a painless neck lump. Ultrasound revealed enlarged cervical lymph nodes and chest X-ray showed a large anterior mediastinal mass causing central airway obstruction. CT confirmed the mass partially encasing the superior vena cava. Due to concerns of airway collapse during surgery, interventional pulmonologists placed a silicone Y stent to restore airway patency before induction of anaesthesia for surgical lymph node biopsy. The excisional biopsy confirmed classic Hodgkin's lymp

Predicting Postinduction Hypotension in Patients for Elective Surgery under General Anesthesia using a Preanesthesia Ultrasonography of the Subclavian Vein.

BACKGROUND: Estimating fluid status before surgery is not routine, but ultrasound can be used for this purpose. Previous studies have shown a correlation between collapsibility indices of the subclavian vein (SCV) and inferior vena cava (IVC). There is a limited data regarding the suitability of studying SVC for predicting hemodynamic events like hypotension during induction anesthesia. By studying SCV width variations prior to surgery, we hope to get an indirect estimate of the fluid status of the patient, which may predict the occurrence of postinduction hypotension during surgery. METHODOL

Long-term Survival after Volatile or Propofol General Anesthesia for Bladder Cancer Surgery: A Retrospective National Registry Cohort Study.

BACKGROUND: Prospective interventional trials and retrospective observational analyses provide conflicting evidence regarding the relationship between propofol versus inhaled volatile general anesthesia and long-term survival after cancer surgery. Specifically, bladder cancer surgery lacks prospective clinical trial evidence. METHODS: Data on bladder cancer surgery performed under general anesthesia between 2014 and 2021 from the National Quality Registry for Urinary Tract and Bladder Cancer and the Swedish Perioperative Registry were record-linked. Overall survival was compared between patie
