Resolution of binary mixtures of rifamycin SV and rifampicin by UV/VIS spectroscopy and partial least-squares method (PLS).

A procedure is proposed for the joint determination of rifamycin SV and rifampicin by UV/VIS spectroscopy. A partial least-squares regression (PLS) was used for the resolution of the overlapping spectrophotometric signals from mixtures of the two drugs. The application of a genetic algorithm to select some of the predictor variables allows one to considerably reduce the number of experimental variables, as well as to improve the prediction capacity of the PLS model constructed with these selected potentials. Finally, the methods were applied to the determination of these drugs in biological sa

Genomic and epigenomic landscape of ADPKD: towards precision diagnostics and tailored treatments.

Autosomal dominant polycystic kidney disease (ADPKD) represents one of the most prevalent hereditary renal disorders, affecting an estimated 12.5 million individuals globally and characterized by progressive cyst formation in both kidneys. While mutations in PKD1 and PKD2 genes account for most cases, recent research has identified rare causative genes as contributing factors in genetically unresolved cases. The review examines the molecular mechanisms of cystogenesis, highlighting how genetic predisposition interacts with epigenetic modifications, including DNA methylation patterns, histone a

Molecular characterization of Strongylida infecting goats and sheep in northeastern Brazil using cytochrome-c oxidase subunit I partial sequencing.

Strongylida parasitizes goats and sheep; increasing resistance to anthelmintics demands species characterization. This study aims to identify Strongylida of goats and sheep in northeastern Brazil, assessing genetic diversity. Seventeen specimens of Ovis aries and 25 of Capra hircus were studied; DNA was obtained from eggs shed in ruminants' feces. A ∼ 700 bp fragment of cytochrome c oxidase subunit 1 (cox1) was PCR-amplified and sequenced; a computational species delimitation analysis was performed; evolutionary significant units (ESUs) were defined using Poisson Tree Process (PTP) and multi-r

Multiple measures for self-identification improve matching donors with patients in unrelated hematopoietic stem cell transplant.

BACKGROUND: Questions persist around whether and how to use race or geographic ancestry in biomedical research and medicine, but these forms of self-identification serve as a critical tool to inform matching algorithms for human leukocyte antigen (HLA) of varying levels of resolution for unrelated hematopoietic stem cell transplant in large donor registries. METHODS: Here, we examined multiple self-reported measures of race and ancestry from a survey of a cohort of over 100,000 U.S. volunteer bone marrow donors alongside their high-resolution HLA genotype data. RESULTS: We find that these se

[Genetic testing in neurological diseases: a practical guide].

During the last decades, genomic medicine has made it possible to bring the knowledge of molecular genetics to the field of medical consultation. There are several studies that contribute to the diagnosis, the definition of prognoses, as well as the possibility of providing genetic counseling based on accurate scientific data. Advances in genomic sequencing have promoted the reclassification of entities according to an etiological criterion. Such is the case of epileptic encephalopathies, ataxias, dystonias, among many other neurological conditions. Its implementation requires strategies aimed

Determinants of approved acetylcholinesterase inhibitor response outcomes in Alzheimer's disease: relevance for precision medicine in neurodegenerative diseases.

Acetylcholinesterase inhibitors (ChEI) are the global standard of care for the symptomatic treatment of Alzheimer's disease (AD) and show significant positive effects in neurodegenerative diseases with cognitive and behavioral symptoms. Although experimental and large-scale clinical evidence indicates the potential long-term efficacy of ChEI, primary outcomes are generally heterogeneous across outpatient clinics and regional healthcare systems. Sub-optimal dosing or slow tapering, heterogeneous guidelines about the timing for therapy initiation (prodromal versus dementia stages), healthcare pr

How registers could enhance knowledge and characterization of genetic dyslipidaemias: The experience of the LIPIGEN in Italy and of other networks for familial hypercholesterolemia.

Familial hypercholesterolemia (FH) is a common genetic disorder of lipid metabolism, still underdiagnosed and undertreated in the general population. Pathology registers could play a crucial role in the creation of a comprehensive and integrated global approach to cover all aspects of this disease. Systematic data collection of patients affected by FH has increased dramatically worldwide in the past few years. Moreover, results from registers already established for the longest time showed their potentialities in the implementation of the knowledge of FH, comparing country-specific approaches

The Genetic Diagnosis of Neurodegenerative Diseases and Therapeutic Perspectives.

Genetics has led to a new focus regarding approaches to the most prevalent diseases today. Ascertaining the molecular secrets of neurodegenerative diseases will lead to developing drugs that will change natural history, thereby affecting the quality of life and mortality of patients. The sequencing of candidate genes in patients suffering neurodegenerative pathologies is faster, more accurate, and has a lower cost, thereby enabling algorithms to be proposed regarding the risk of neurodegeneration onset in healthy persons including the year of onset and neurodegeneration severity. BACKGROUND: Familial renal glucosuria (FRG) is an inherited renal tubular disorder characterized by persistent isolated glucosuria with normal blood glucose. SLC5A2 gene mutation was the causative of FRG. METHODS: Molecular genetic analysis of SLC5A2 gene by Sanger sequencing was conducted in two unrelated non-consanguineous Chinese families with isolated glucosuria. Extensive laboratory test and physical examination were performed. In silico algorithms were used to explore the potential effect of novel mutation on SGLT2 function. We also summarized the reported SLC5A2 mutations in the Chines

Pseudohypoparathyroidism and Gsα-cAMP-linked disorders: current view and open issues.

Pseudohypoparathyroidism exemplifies an unusual form of hormone resistance as the underlying molecular defect is a partial deficiency of the α subunit of the stimulatory G protein (Gsα), a key regulator of the cAMP signalling pathway, rather than of the parathyroid hormone (PTH) receptor itself. Despite the first description of this disorder dating back to 1942, later findings have unveiled complex epigenetic alterations in addition to classic mutations in GNAS underpining the molecular basis of the main subtypes of pseudohypoparathyroidism. Moreover, mutations in PRKAR1A and PDE4D, which enco

Precision Dosing of Tacrolimus in Liver Transplantation: Integrating Donor-Recipient CYP3A5 Pharmacogenomics and Drug Interactions.

Tacrolimus dosing in liver transplantation is complicated by a narrow therapeutic index and high CYP3A5 genetic variability. While saturable Michaelis-Menten kinetics can explain nonlinearities, identifying saturable parameters from routine clinical data remains challenging. This study aimed to determine the optimal structural model and develop a precision dosing algorithm. A population pharmacokinetic analysis was conducted in 114 patients, yielding 1989 observations. CYP3A5 genotypes were determined for both recipients and donors. Using Phoenix NLME, we rigorously compared linear versus Mich

Incidence, Impact, and Management of Incidentalomas on Preoperative Computed Tomographic Angiograms for Breast Cancer Patients with and without Genetic Mutations.

BACKGROUND: Preoperative abdominal computed tomographic angiograms for free flap breast reconstruction improve operative safety and efficiency, but incidental findings are common and potentially affect management. In addition, the authors hypothesized that patients with genetic mutations might have a higher rate of significant findings. The authors present the largest series of computed tomographic angiogram "incidentalomas" in these two populations and an evidence-based algorithm for managing common findings. METHODS: All patients undergoing free flap breast reconstruction at Northwell Healt

A clinical criterion to exclude the hyperimmunoglobulin D syndrome (mild mevalonate kinase deficiency) in patients with recurrent fever.

OBJECTIVE: The hyperimmunoglobulin D syndrome (HIDS) is an autosomal recessive autoinflammatory disease caused by mutations in the mevalonate kinase gene. Our objective was to define a clinical criterion able to exclude HIDS without the need of genetic testing. METHODS: A recursive partitioning algorithm was applied to derive the clinical criterion in 149 patients with genetic testing in a French laboratory, among whom 35 had HIDS. The criterion was validated in 93 patients with genetic testing in a Dutch laboratory, among whom 28 had HIDS. RESULTS: The most discriminatory composite clinical

[Gallstone disease - interdisciplinary treatment].

Gallstones develop in the gallbladder or the bile ducts. According to their chemical composition, gallstones can be divided into cholesterol stones, which are common, and the rare bile pigment stones. Altogether, up to 20 % of all adults develop gallstones and more than 20 % of them symptoms or complications. Female sex, age, pregnancy, physical inactivity, obesity, overnutrition and genetic factors such as ABCB4 deficiency of the hepatic lecithin transporter are kown risk factors for gallstone formation. In about one half of all patients biliary symptoms precede the three common and potential

Recommendations for the classification of diseases as CFTR-related disorders.

Several diseases have been clinically or genetically related to cystic fibrosis (CF), but a consensus definition is lacking. Here, we present a proposal for consensus guidelines on cystic fibrosis transmembrane conductance regulator (CFTR)-related disorders (CFTR-RDs), reached after expert discussion and two dedicated workshops. A CFTR-RD may be defined as "a clinical entity associated with CFTR dysfunction that does not fulfil diagnostic criteria for CF". The utility of sweat testing, mutation analysis, nasal potential difference, and/or intestinal current measurement for the differential dia

Accurate interpretation of genetic variants in sudden unexpected death in infancy by trio-targeted gene-sequencing panel analysis.

In sudden unexpected death in infancy cases, postmortem genetic analysis with next-generation sequencing potentially can extract candidate genes associated with sudden death. However, it is difficult to accurately interpret the clinically significant genetic variants. The study aim was to conduct trio analysis of cases of sudden unexpected death in infancy and their parents to more accurately interpret the clinically significant disease-associated gene variants associated with cause of death. From the TruSight One panel targeting 4813 genes we extracted candidate genetic variants of 66 arrhyth

Concordant genetic breaks, identified by combining clustering and tessellation methods, in two co-distributed alpine plant species.

Natural genetic breaks may indicate limitations to gene flow or the presence of contact zones of previously isolated populations. Molecular evidence suggests that genetic breaks have aggregated in distinct geographical areas. We propose a new application of well-established statistical methods for analysing multilocus genetic data to identify intraspecific genetic breaks. The methodological approach combines Bayesian clustering with a spatially explicit maximum-difference algorithm to visualize and quantify breaks between clusters. We used amplified fragment length polymorphism data of two co-
