The frontal and parietal cortex are intimately involved in the representation of goal-directed movements, but the crucial neuroanatomical sites are not well established in humans. In order to identify these sites more precisely, we studied stroke patients who had the classic syndrome of ideomotor limb apraxia, which disrupts goal-directed movements, such as writing or brushing teeth. Simultaneous recordings of voice and cricothyroid activity during speech gestures by one speaker of Hindi were made. The experimental utterances consisted of monosyllabic and bisyllabic meaningful and nonsense words containing in prestressed initial and medial and poststressed final positions the voiced and unvoiced stops and affricates from both the aspirated and unaspirated categories. We propose Participation-Engagement (PAR-EN) as a philosophically based heuristic for prioritizing interventions in comorbid, complex, and chronic psychiatric conditions. Drawing from 1) the sociologist Talcott Parsons, 2) the continental-philosophical tradition, and 3) our own previous work (Davidson & Shahar, 2009; Shahar, 2004, 2006), we argue that participation in personally meaningful life goals represents a hallmark of mental health. We studied the modification of auditory perception in three different conditions in twenty subjects. Observing other person's discordant articulatory gestures deteriorated identification of acoustic speech stimuli and modified the auditory percept, causing a strong McGurk effect.
