Spontaneous abortion occurs in about one in five pregnancies, and for many women can lead to significant psychological sequelae. This article reviews the literature on the psychological morbidities after pregnancy loss, their risk factors, differences in partner's reactions, potential screening tools and interventions for physicians. The most common reactions women experienced after pregnancy loss were grief, depression, and anxiety, with rates cited of approximately 40%, 12-50%, and 22-41%, respectively. The work of perinatal nurses sometimes includes emergencies involving death, or near death, which can leave health care providers with feelings of stress and grief. After experiencing a particularly stressful period, nurses at our organization identified processes to help themselves recover and to support each other. The result of this work is a written plan to facilitate the support of perinatal nurses after critical events. This article describes the development and implementation of this plan.

Hospice caregivers are often targets of appropriate or displaced anger from the patients and family members that they try to help. Although anger is often an essential part of the grieving process, it may be difficult to endure. Caregivers must therefore understand the causes and signs of anger in themselves, patients, and family members, and find strategies to reduce the anger. A therapeutic response to anger will better facilitate the grief process as well as the effectiveness and well-being of the hospice caregiver.

Difficult patient encounters are common in clinical practice, with many arising from patient hostility owing to a breakdown in communication and the health care alliance. Patient anger may be a manifestation of fear, grief, or discontent with prior experiences in the health care system, but there may also be contributions from specific patient, physician, or situational factors. Physicians may intervene with specific actions based on these individual factors, while focusing on self-reflection to better understand their part in creating a hostile physician-patient dyad.

This is a personal reflection on my and my family's experience with Telomere Biology Disorders (TBDs). This reviews the journey from identifying a familial pattern with idiopathic pulmonary fibrosis to identifying a genetic cause and a specific gene mutation. It also discusses the journey for the author from wanting to not know whether they had the genetic mutation to deciding to test for TBD and the gene. The reflection also discusses how I have dealt with the grief of losing my father and sister to this disease.

Acceptance of chronic conditions often has been studied from the perspective of grief and loss. This paper presents a new conceptualization of acceptance of chronic conditions that is grounded in the perspective of those living with a chronic condition and focuses on adaptation to and acceptance of change in one's life. Data obtained from a sample of 822 persons with multiple sclerosis and a sample of more than 2,000 polio survivors support the reliability and validity of the new Acceptance of Chronic Health Conditions Scale.

Having worked for several years in prevention of heart attack recurrence, I am inclined to agree with his conclusion that the construct is a trail which should now be abandoned.
