OBJECTIVE: To investigate a new operation of breast reduction for gynecomastia.
METHODS: In the past 5 years we treated 42 cases (80 sites) of gynecomastia with the double-ring incision, a supra-lateral derma-mammary pedicle of the nipple and areola and the tumescent technique.
RESULTS: The tumescent anesthesia was effective. The operation was easily performed with slight injury and less bleeding. Leydig cell tumor is a testicular tumor with a low incidence characterized by a high estrogens secretion from the tumoral cells. Its more frequent clinical presentation is a testicular nodule with or without other endocrine manifestations due to estrogenic hypersecretion. Gynecomastia is a condition commonly associated with puberty. An endoscope-assisted transaxillary procedure was used to treat a patient with this condition. An endoscope system was inserted into a 4-cm skin incision in the axilla, and the glands and adjacent fat were removed en bloc. The resulting scar in the axilla was small and inconspicuous, and the postoperative breast contour satisfied the patient. The endoscopic system provided a useful transaxillary approach. These results show that this method appears to be the most suitable one for surgical management of gynecomastia.

The treatment of larger types of gynecomastia is significantly different than that of less severe gynecomastias. Special concerns of the former include areola enlargement, nipple-areola ptosis, and redundant skin. We report a 14 year-old male with severe, long-lasting gynecomastia. Baseline serum androstenedione levels were elevated compared to testosterone levels (330 ng/dl vs 28 ng/dl). Thirty-three-year-old male patient, without recognized risk factors, presenting with recurrent thrombophlebitis and bilateral gynecomastia as the only sign of feminization. Twenty-seven months after onset hepatocarcinoma of the right lobe was diagnosed. The rest of the liver was normal. Gynecomastia resolved after surgical removal of the tumor. Thromboembolism did not recur.

A six and a half-year-old boy was investigated for prepubertal gynecomastia and hyperpigmentation of the skin. The biological, clinical and radiological work-up did not reveal any endocrinological or tumoral cause. It was incidentally noticed that the patient's sister presented with similar clinical signs which could be related to an estrogen containing ointment. Long-term follow-up of the boy showed complete but delayed regression of the gynecomastia and persistence of hyperpigmentation. His sister developed later on a normal puberty.

Serum levels of testosterone, estradiol, progesterone, 17alpha-hydroxyprogesterone, follicle-stimulating hormone, and luteinizing hormone were measured in 16 boys with pubertal gynecomastia. Six patients had elevated serum estradiol concentrations, and four of these six also had elevated progesterone levels.
