Risk Assessment of Tendon Attrition Following Treatment of Distal Radius Fractures With Volar Locking Plates Using Audible Crepitus and Placement of the Plate: A Prospective Clinical Cohort Study.

PURPOSE: To identify risk factors for tendon attrition after volar locking plate fixation of distal radius fractures. METHODS: We prospectively assessed attrition of the flexor pollicis longus tendon at volar plate removal in 127 hands in 126 patients. We also evaluated preoperative lateral wrist radiographs, sonographs, and crepitus with flexor pollicis longus tendon motion and compared the demographic and radiographic characteristics of patients with and without tendon attrition. Multivariate logistic regression analysis was employed to identify the factors independently associated with ten

Minifragment plate fixation of high-energy navicular body fractures.

BACKGROUND: The purpose of this study was to assess the ability of miniplate fixation in navicular fractures to restore medial column stability, maintain reduction, and determine the impact this approach may have on the development of avascular collapse of the navicular. We hypothesized that comminuted fractures of the navicular can be safely reduced and maintained to union with minifragment plate fixation with a low incidence of avascular collapse. MATERIALS AND METHODS: A retrospective chart review was performed on 24 patients with navicular fractures treated with open reduction and interna

[Resorbable rods and screws of polyglycolide in stabilizing malleolar fractures. A clinical study of 600 patients].

Absorbable internal fracture fixation implants have been in large-scale clinical use since 1984. The main advantage of these devices is that … the use of absorbable implants has made it possible for resources corresponding to several hundred hardware removal procedures to be used for other purposes.

Anterior thoracolumbar spine exposure: critical review and analysis.

BACKGROUND: Indications for anterior thoracolumbar spine interbody fusion have expanded because of safe and expeditious surgical exposure that can be provided by the approach surgeon. In our practice, previous anterior interbody instrumentation, multiple disc level exposure, patient age, and body habitus are not surgical deterrents despite the potential for increased complications. The arterial and venous complications of anterior spine exposure have been well documented; however, the purpose of this study is to document the incidence of other complications, such as deep vein thrombosis (DVT),

Cost-effective reconstruction of long-standing lateral orbital rim defect with bone-prelaminated forehead flap using iliac crest: A case report.

RATIONALE: Reconstructing composite lateral orbital rim defects after high-energy trauma remains challenging, particularly after infected alloplastic failure and in settings where microsurgery or patient-specific implants are unavailable. Autologous regional reconstruction may restore soft tissue coverage and bony support while avoiding additional hardware. PATIENT CONCERNS: A 33-year-old man presented with a long-standing post-traumatic right orbital deformity after ballistic injury, including globe loss and composite defects of the lateral orbital rim, temporal bone, and adjacent soft tissu

Preoperative Depression Is Associated With Increased Complications Following Distal Radius Fracture Surgery.

BACKGROUND: Depression is a known risk factor for inferior outcomes after orthopedic procedures, but its specific relationship with distal radius fractures remains unknown. This study investigates the relationship between preoperative diagnosed depression and common postoperative complications occurring within the first year after open reduction internal fixation (ORIF) for distal radius fractures. METHODS: This retrospective study used Truven MarketScan database and the Current Procedural Terminology (CPT) codes to identify distal radius fracture patients who underwent ORIF in the United Sta

An Unoperated Crouzon Family Treated with Monobloc Distraction: Challenges and Lessons.

UNLABELLED: Crouzon syndrome (CS) is a rare form of craniosynostosis characterized by bicoronal craniosynostosis and facial features including severe midface hypoplasia, exophthalmos, and hypertelorism. Most patients are diagnosed and treated in early childhood; however, there are a few reports of Crouzon patients treated as adults with monobloc facial advancement. To our knowledge, this is the first report of a family affected by CS treated sequentially with monobloc facial advancement using combined internal and external distraction osteogenesis (rigid external distraction). METHODS: We pre

A biomechanical study of headless compression screws versus a locking plate in radial head fracture fixation.

HYPOTHESIS: Fixation of a 3-part radial head fracture with cannulated compression screws will show equivalent stiffness to a locking plate under axial load. Debate exists regarding the management of Mason type III fractures, with many believing that open reduction and internal fixation provides advantages over other options. By virtue of their subarticular placement, screw fixation is less likely to cause impingement compared with plate fixation, which can result in loss of rotation and requirement for hardware removal. Insufficient fixation stability can lead to nonunions, necrosis of the rad…

A Retrospective Study of Thoracolumbar Fractures Treated with Fixation and Nonfusion Surgery of Intravertebral Bone Graft Assisted with Balloon Kyphoplasty.

BACKGROUND: Posterior fixation and fusion is the primary treatment for thoracolumbar fractures, although this treatment may sacrifice range of motion (ROM) to achieve stability, rather than treating the fracture itself. Two issues addressed when treating thoracolumbar fractures are 1) replacing the fractured vertebrae, especially the upper end plate of the injured vertebrae and 2) providing strong fixation with biomechanical stability and flexibility. METHODS: This retrospective study included 61 consecutive patients with thoracic or lumbar fractures treated from October 2010 to May 2014. Pat

Are embrasure wires better than arch bars for intermaxillary fixation?

PURPOSE: To compare the outcomes of mandible fractures treated with open reduction and internal fixation versus adjunctive intermaxillary fixation (IMF) using 2 different techniques. MATERIALS AND METHODS: We performed a retrospective medical record review. The medical records of consecutive patients with mandible fractures treated surgically with adjunctive use of IMF (embrasure wires vs arch bars) were reviewed for demographic data, etiology, fracture location, antibiotic use, and interval to repair. Specifically, the complications, including infection, malunion or nonunion, hardware failur

Clinical results of olecranon fractures treated with multiplanar locked intramedullary nailing.

BACKGROUND: Proximal ulnar fractures have traditionally been fixed with either tension band wiring or plate and screw fixation. These traditional techniques often irritate the surrounding soft tissues, potentially leading to subsequent secondary hardware removal surgeries. Intramedullary proximal ulnar fixation provides similar rigid fixation, however, no review yet exists to support the clinical use of currently available implants. OBJECTIVES: To investigate the clinical and radiographic short-term outcomes for multiplanar locked intramedullary nails used to treat proximal ulnar fractures. …

Extra-articular Deformity Correction via Clamshell Osteotomy of the Tibia With Simultaneous Bilateral Total Knee Arthroplasty.

Total knee arthroplasty (TKA) aims to alleviate severe pain and functional impairment in advanced knee arthritis. However, extra-articular deformities must be addressed preoperatively to ensure optimal implant positioning and soft tissue balancing. We present a case of a 58-year-old man with a history of a right tibial malunion and left femoral shaft fracture, who developed progressive bilateral knee osteoarthritis. He underwent simultaneous bilateral TKA with a right tibial clamshell osteotomy to correct a 17° varus deformity and left femoral hardware removal to accommodate a long stem implan…

A Foreign Body in an Unexpected Place-Ingested Hardware Nails Within the Appendix.

Foreign body ingestion is commonly encountered in clinical practice, with over 100,000 cases reported annually in the United States. The majority of objects pass through the gastrointestinal tract spontaneously and without consequence, with fewer than 1% of objects requiring surgical intervention. Foreign bodies have rarely been found lodged within the appendix. We report the therapeutic management of a young patient who ingested over 30 hardware nails. The patient originally underwent an esophagogastroduodenoscopy with attempted removal from the stomach and duodenum, though only 3 nails were

Use of Peripheral Nerve Stimulation to the Recurrent Laryngeal Nerves to Successfully Treat Refractory Globus Sensation: A Case Report.

BACKGROUND: Globus sensation is a nonpainful sensation of a lump or foreign object stuck in the throat that is not associated with dysphagia. Persistent globus sensation is distressing for patients and often the result of no clear etiology. CASE REPORT: We present a case of a 71-year-old woman with persistent globus sensation following cervical spine surgery. Initial evaluations for etiology and treatment included hardware removal, endoscopy, pharmacological treatment, botulinum toxin injections, collagen injection laryngoplasty, speech therapy, anesthetic nerve blocks, and attempts at periph…

[Coraco-clavicular loop and tension band suture in type II and type V distal-third clavicle fractures].

Many surgical techniques have been used to address unstable distal third clavicle fractures. Complications and the need for hardware removal are still a concern. We propose a surgical technical using high-strength sutures to restore vertical and horizontal stability in Neer type II and Neer type V distal-third clavicle fractures. It has been used in three cases; two type V and one type II. In all cases, bone healing was achieved uneventfully and all patients resumed their pre-injury activities including sports at sixmonths postoperatively. The coraco-clavicular loop and tension band suture tec…
