Mandatory Bicycle Helmet Laws in the United States: Origins, Context, and Controversies.

This article examines the origins and context of mandatory bicycle helmet laws in the United States. Localities began to enact such laws in the early 1990s, having experimented with helmet laws for motorcycles previously. As cycling became increasingly popular in the 1970s and 1980s because of a variety of historical trends, from improved cycle technology to growing environmental consciousness, cycling-related injuries also increased. Bicycle safety advocates and researchers alike were particularly troubled by head injuries. National injury surveillance systems and a growing body of medical li

Craniofacial Injuries Seen Among Electric-Motorized Bicycle Riders.

The popularity of electric-motorized bicycles (E-bikes) has increased dramatically over the past few years. As a result, E-bike--associated injuries are quickly becoming a substantial issue. The aim of the present study was to evaluate the epidemiology and general nature of these injuries, with special attention to craniofacial trauma. This was a retrospective study of 84 E-bike riders who suffered from trauma and treated at our level 1 trauma center between the years 2014 and 2018. The information consisted of demographics, characteristics of injury, Injury Severity Score, and number of hospi

Long-Term Effects of Education and Legislation Enforcement on All-Age Bicycle Helmet Use: A Longitudinal Study.

Bicycle-related injuries are a leading cause of child and youth hospitalizations in Canada. The use of helmets while bicycling reduces the risk of brain injuries. This study investigated the long-term effect of legislation coupled with enforcement to improve helmet use rates. We conducted a longitudinal observational study of helmet use at 9, 11, and 14 years after bicycle helmet legislation was enacted. Data were compared to baseline observations collected after legislation was passed in 1997. A comprehensive enforcement and educational diversion program, Operation Headway-Noggin Knowledge (O

Head injury patterns in helmeted and non-helmeted cyclists admitted to a London Major Trauma Centre with serious head injury.

BACKGROUND: Cycle use across London and the UK has increased considerably over the last 10 years. With this there has been an increased interest in cycle safety and injury prevention. Head injuries are an important cause of mortality and morbidity in cyclists. This study aimed to ascertain the frequency of different head injury types in cyclists and whether wearing a bicycle helmet affords protection against specific types of head injury. METHODS: A retrospective observational study of all cyclists older than 16 years admitted to a London Major Trauma Centre between 1st January 2011 and 31st

Identifying psychological and socio-economic factors affecting motorcycle helmet use.

Sixty percent of motorcyclist fatalities in traffic accidents of Iran are due to head injuries, but helmet use is low, despite it being a legal requirement. This study used face-to-face interviews to investigate the factors associated with helmet use among motorcycle riders in Mashhad city, the second largest city in Iran. Principal component analysis (PCA) and confirmatory factor analysis (CFA) were used for data reduction and identification of consistent features of the data. Ordered and multinomial logit analyses were used to quantify the influences on helmet use and non-use. The data show

Argenta clinical classification of deformational plagiocephaly.

INTRODUCTION: A stratification system is useful in deformational plagiocephaly (DP) to help categorize patients and reproduce a consistent treatment strategy. The Argenta classification is a clinical 5-point scale for unilateral DP and 3-point scale for central DP (CDP). METHODS: A retrospective review was completed for patients with DP and classified using the Argenta clinical classification by plastic surgeons at a tertiary medical center over a 12-year period. RESULTS: In the 4483 patients, type III was the most prevalent DP type (42%) followed by II, IV, I, and V. Within CDP, VIB was the

Treatment of positional plagiocephaly--helmet or no helmet?

INTRODUCTION: Positional plagiocephaly has attained widespread attention. There is a lot of data on helmet therapy available, but the natural course of the deformity has not been investigated in depth. The decision for or against helmet therapy can be controversial. This study examined the outcome of both options. METHODS: 128 infants were enrolled in this prospective, non-randomized, longitudinal study. 62 were treated with and 66 without a helmet. The initial cranial vault asymmetry index (modified CVAI) was determined at 6.3 and 6.2 months of age (SD 1.44/2.14). Follow-up took place at the

Injury patterns and safety practices of rock climbers.

BACKGROUND: Rock climbing is gaining popularity. The injury patterns and safety practices of climbers have not been well described. This study seeks to identify the general injury patterns and safety practices associated with rock climbing. METHODS: An anonymous multiple choice, Likert scale, and short-answer Internet survey was posted on several rock climbing Websites. Data were collected autonomously for a 2-month period in 2004. Demographic data were obtained and subjects self-identified their three most significant injuries. Participants were also surveyed regarding safety practices and t

Association of helmet use with death in motorcycle crashes: a matched-pair cohort study.

The association of helmet use with death in a motorcycle crash can be estimated using matched-pair cohort methods. By estimating effects among naturally matched pairs on the same motorcycle, one can account for potential confounding by motorcycle characteristics, crash characteristics, and other factors that may influence the outcome. The authors used Fatality Analysis Reporting System data, from 1980 through 1998, for motorcycles that crashed with two riders and either the driver or the passenger, or both, died. For their main analysis, the authors estimated the relative risk of death using c

Injuries involving off-road cycling.

BACKGROUND: Data on injuries due to off-road bicycling are scarce, but do indicate that injuries in this sport are frequent. We examined the pattern of injuries to off-road riders as part of a larger study of bicycle injuries and helmet use. METHODS: We undertook a prospective study of bicycle-related injuries identified at seven emergency departments in Seattle, Washington, between March 1992 and August 1994. Hospitalized patients and medical examiners' cases were included. Detailed questionnaires and abstraction of all medical records provided information on crash and rider characteristics

The biomechanics of hip protectors.

The mode of action of one design of hip protector was investigated. A dummy femur surrounded by a flesh substitute was impacted from a range of heights while the forces on the trochanter and the surrounding flesh were measured. The protector doubled the estimated fall distance for fracture of the trochanter. The protection mechanisms are load transfer to the muscles of the thigh and energy absorption in the protector. Hip protectors could absorb more energy safely if they were thicker, or if the shell design was modified.

Football head and neck injuries--an update.

In the last 5 years there has been a dramatic decrease in the deaths directly related to football participation. The incidence of serious spinal cord injuries, however, appears to be increasing. The number of quadriplegic athletes varies from an estimated 1 per 7,000 to 1 per 58,000 participants per year in different areas of the country. The majority of catastrophic head and neck injuries occurs while tackling or blocking, and defensive players are much more liable to sustain these injuries than offensive players. In addition to permanent and irreversible spinal cord damage, football players

Ice hockey shoulder pad design and the effect on head response during shoulder-to-head impacts.

Ice hockey body checks involving direct shoulder-to-head contact frequently result in head injury. In the current study, we examined the effect of shoulder pad style on the likelihood of head injury from a shoulder-to-head check. Shoulder-to-head body checks were simulated by swinging a modified Hybrid-III anthropomorphic test device (ATD) with and without shoulder pads into a stationary Hybrid-III ATD at 21 km/h. Tests were conducted with three different styles of shoulder pads (traditional, integrated and tethered) and without shoulder pads for the purpose of control. Head response kinematic

Mice lacking GFAP are hypersensitive to traumatic cerebrospinal injury.

Glial fibrillary acidic protein (GFAP) is an intermediate filament protein expressed primarily in astrocytes. We have tested whether GFAP protects against mechanical stress by inducing percussive head injury in GFAP-null mice with a weight drop device. When mice were positioned on a foam bed which allowed head movement at impact, all 14 wild-type mice tested survived, but 12 of 15 GFAP-null mice died within a few minutes. The cause of death appeared to be upper cervical spinal cord injury resulting in respiratory arrest. When the foam bed was replaced by a firm support, both GFAP-null and wild

The state of head injury biomechanics: past, present, and future: part 1.

This article is the first of two parts of a comprehensive survey of the biomechanics of head injury since its inception in 1939 in the United States, the separation being made for temporal and spatial reasons. The second portion of this material will be published at a later time in this journal. The discussion will be almost exclusively limited to nonpenetrating events. The topics presented in the following sections include an introduction that discusses the magnitude of the problem, the basic tools of biomechanics, and significant major reference sources covering this subject. This is succeed

Neighborhood-based Social Determinants of Health are Associated With Decreased Helmet Use in Young Children.

PURPOSE: We sought to determine the association between neighborhood-level social determinants of health (SDoH) and protective helmet use for young children involved in bicycle, scooter, and skateboard crashes. We hypothesized that children from disadvantaged communities would be less likely to be wearing helmets at the time of their crash. METHODS: A retrospective cohort study was performed of all children ≤10 years old who presented after bicycle, scooter, and skateboard crashes to a single Level 1 Pediatric Trauma Center (7/1/2017-6/30/2021).
