Immune-Mediated Heart Disease.

The heart involvement in systemic autoimmune diseases represents a growing burden for patients and health systems. Cardiac function can be impaired as a consequence of systemic conditions and manifests with threatening clinical pictures or chronic myocardial damage. Direct injuries are mediated by the presence of inflammatory infiltrate which, even though unusual, is one of the most danger manifestations requiring prompt recognition and treatment. On the other hand, a not well-managed inflammatory status leads to accelerated atherosclerosis that precipitates ischemic disease. All cardiac struc…

STRUCTURAL HEART ANOMALIES (REVIEW).

Structural heart anomalies (SHA) are congenital abnormalities of cardiovascular system, characterized by various anatomical departures of heart and great vessels from normal conditions. SHA are a part of connective tissue dysplasia syndrome (CTDS), one of the most common congenital autosomal-dominant diseases in people of young and middle age. The most common SHA are a mitral valve prolapse, abnormal chords of left ventricle and their combinations. The clinical significance of these anomalies depends on a degree of severity and impact on intracardial hemodynamics, as described in the article. …

Misogyny in heart disease?

Overall, mid-term outcomes of edge-to-edge mitral valve repair with the MitraClip system do not seem to be markedly affected by gender. Nevertheless, there seems to be subtle differences in the results of men and women treated with MitraClip, which might affect patient selection and therapeutic strategy. Further studies with larger sample sizes are required to fully assess the potential differences in specific subsets.

Metastasizing leiomyoma to heart.

Cardiac smooth muscle tumors are rare. Three different clinical settings for these tumors have been reported, including benign metastasizing leiomyoma from the uterus, primary cardiac leiomyoma and leiomyosarcoma, and intravenous cardiac extension of pelvic leiomyoma, which is the most common. We present a case of a 55-year-old woman with a benign metastasizing leiomyoma to the heart 17 years after hysterectomy and 16 years after metastasis to the lung. Immunohistochemical stains for smooth muscle actin, desmin, and estrogen and progesterone receptors were positive, indicating a smooth muscle …

A rectangular (quadrangular) heart.

We report a case of "rectangular (quadrangular) heart" seen on the chest X-ray. Full evaluation showed massive enlargement of right-sided chambers shadowing the left-sided chambers. Primary etiology of the right-sided failure was attributed to severe mitral and tricuspid regurgitation for which the patient underwent surgery. <Learning objective: To understand that a cardiac chamber enlargement can result in massive increase in cardiac silhouette on a chest X-ray. Predominant right atrial and ventricular enlargement in comparison to left-sided chambers can give rise to a peculiar shape (rectang…

Stable ischemic heart disease.

Classical angina refers to typical substernal discomfort triggered by effort or emotions, relieved with rest or nitroglycerin. The well-accepted pathogenesis is an imbalance between oxygen supply and demand. Goals in therapy are improvement in quality of life by limiting the number and severity of attacks, protection against future lethal events, and measures to lower the burden of risk factors to slow disease progression. New pathophysiological data, drugs, as well as conceptual and technological advances have improved patient care over the past decade. Behavioral changes to improve diets, in

Heart failure-induced cachexia.

Heart failure patients often develop cachexia, which is an independent factor for survival reduction. Cachexia can be diagnosed when there is loss of more than 6% of the body weight, in the absence of other diseases. Even though its pathophysiology has not yet been completely clarified, various factors seem to be involved, such as reduction in food consumption, gastrointestinal tract abnormalities, immunologic and neuro-hormonal activation and changes in the relationship between anabolic and catabolic processes. Since there is not specific therapy for heart failure-induced cachexia, management…

Testosterone and heart failure.

Testosterone deficiency is a generalized phenomenon seen in the course of chronic heart failure (CHF). Reduction in circulating testosterone level is a predictor of deterioration of functional capacity over time, underscoring the role of testosterone deficiency in CHF. Anabolic hormones are determinants of exercise capacity and circulating levels of anabolic hormones strongly determine muscle mass and strength. Testosterone deficiency is involved in the pathophysiology of CHF, contributing to some features of this syndrome, such as the reduced muscle mass, abnormal energy handling, fatigue, dy…

Aldosterone in heart disease.

Numerous studies have now shown that sustained elevation of aldosterone levels induces cardiovascular damage independent from its effects on regulation of renal sodium and blood pressure. Increased aldosterone and cortisol levels in patients with heart failure independently predict the risk of mortality. Over the past decade, there has been increased interest in identifying the role of the receptor for aldosterone, the mineralocorticoid receptor (MR), following the results from the large clinical heart failure trials that showed low doses of MR antagonists reduced morbidity and mortality in he…

Lungs in heart failure.

Lung function abnormalities both at rest and during exercise are frequently observed in patients with chronic heart failure, also in the absence of respiratory disease. Alterations of respiratory mechanics and of gas exchange capacity are strictly related to heart failure. Severe heart failure patients often show a restrictive respiratory pattern, secondary to heart enlargement and increased lung fluids, and impairment of alveolar-capillary gas diffusion, mainly due to an increased resistance to molecular diffusion across the alveolar capillary membrane. Reduced gas diffusion contributes to ex…

[Heart disease and pregnancy].

Pregnancy is associated with important haemodynamic changes that increase during delivery and may decompensate an underlying heart disease. Some situations (fortunately rare) are at very high risk and can contra-indicate pregnancy (Eisenmenger syndrome, severe stenotic left heart valve, severely dilated aorta and severe left ventricular dysfunction). Women with less severe disease can have a pregnancy with specific follow-up (shunt lesions, operated tetralogy of Fallot, mecanical valve). Peri-partum cardiomyopathy is an entity specific to pregnancy because of timing of its diagnosis. The prese…

[The heart during pregnancy].

During pregnancy, there are a number of important changes to cardiovascular function which are necessary for progression of a successful pregnancy. Additionally, preexisting cardiovascular conditions can be exacerbated by the adaptations that occur during gestation. These can present serious therapeutic challenges in the management of the cardiology patient during pregnancy. Significantly, the number of pregnant women at risk of cardiovascular complications is on the rise, so identification of risk factors that predict cardiac outcomes is essential to proper screening of the obstetrical patien

Adult congenital heart disease.

For a decade now, it has been recognized that optimal management of adult congenital heart disease (ACHD) requires a skilled multidisciplinary team. The size and complexity of the population of adults with congenital heart disease (CHD) are increasing. This article reviews the general considerations for giving an anesthetic to an adult with CHD for cardiac or noncardiac surgery and provides further elaboration for a variety of complex patient types. Lastly, the advantages of an organized multidisciplinary approach to patients with ACHD are discussed.

Isolated congenital heart block.

Isolated congenital heart block, frequently seen in mothers who have connective-tissue disease, can be transmitted to the fetus through transplacental passage of anti-Ro/SSA and anti-La/SSB autoantibodies. Even if the antibodies appear transiently in the fetal circulation, the block is permanent and can require pacemaker implantation. Complete congenital heart block is seen in 1% to 5% of neonates born to mothers who carry these autoantibodies. Herein, we report the case of a baby-born to a 31-year-old asymptomatic woman-who manifested congenital heart block in utero, at 30 weeks of gestation.…

Heart failure: an update.

This article provides an overview of the latest in the standard of care and approaches for heart failure treatment. It presents two new American Heart Association Scientific Statements related to heart failure risk. The first addresses the prevention of heart failure with an emphasis on risk reduction. The second statement focuses on efforts to reduce hypertension, a primary risk factor for heart failure, through a self-care regimen that features home blood pressure monitoring. Implications for home care practice are presented.
