Repolarization heterogeneity and rate dependency in a canine rapid pacing model of heart failure.

BACKGROUND: Repolarization heterogeneity and rate dependency have long been established as factors contributing to arrhythmogenic risk. However, there are conflicting observations regarding the nature and extent of ventricular repolarization heterogeneity that complicate understanding of arrhythmogenic mechanisms. To explore these disparate findings, we studied ventricular repolarization heterogeneity and rate dependency in a canine, rapid pacing model of heart failure. METHODS AND RESULTS: We studied ventricular repolarization heterogeneity and rate dependency in 10 canine hearts (5 normal a

The heart in Wegener's granulomatosis.

Wegener's granulomatosis (WG) is a necrotizing vasculitis that mainly affects the respiratory tract and kidneys. Of note, involvement of the heart is being increasingly recognized in these patients. Cardiac manifestations can arise from the coronary arteries, pericardium, myocardium, endocardium, valves, conduction system and great vessels, but in most cases cardiac involvement is clinically silent. Consequently, a regular cardiovascular evaluation, including echocardiography, should be performed in every patient with WG, while patients with WG flare should be closely monitored for cardiac com

Isorhythmic interaction between a dual-chamber pacemaker and an intrinsic rhythm: pacemaker malfunctioning or not?

A patient underwent cardiac surgery (including a biatrial maze procedure) for congenital heart disease and paroxysmal atrial tachycardia. Previously, a dual-chamber pacemaker was inserted for bradycardia. Postoperative ECG showed a narrow complex rhythm interspersed with pacing stimuli that appeared premature and unable to capture. The phenomenon was due to isorhythmic interaction between the pacemaker and the patient's own intrinsic rhythm, which is compatible with normal pacemaker functioning. The different patterns possible for the phenomenon and the necessary conditions for their occurrenc

Permanent myocardial dysfunction caused by high-voltage electrical injury--a case report.

Electrical injury, particularly alternating current, may lead to disease of conducting tissue or to myocardial damage, or even may cause sudden death. In the present case, the authors report a 22-year-old man who sustained extensive upper extremity trauma from a high-voltage electrical injury and manifested clinical signs of left ventricular heart failure.

Effects of acute atrial dilation on heterogeneity in conduction in the isolated rabbit heart.

INTRODUCTION: Atrial dilation plays an important role in the development and persistence of atrial fibrillation (AF). The mechanisms by which atrial dilation increases the vulnerability to AF are not fully understood. METHODS AND RESULTS: In 11 isolated rabbit hearts, the right atrium was acutely dilated by increasing the intra-atrial pressure from 2 to 9 and 14 cm H2O. A rectangular mapping array of 240 electrodes (spatial resolution 0.5 mm) was positioned on the free wall of the right atrium. The atrium was paced from four different sites at intervals of 240 and 125 msec. At normal atrial p

Electrocardiographic evidence for cocaine cardiotoxicity in cat.

Recent case studies suggest that cocaine overdose may produce life-threatening cardiac arrhythmias. We therefore investigated its effects on the electrocardiogram (leads II and V1) and arterial blood pressure in cats anesthetized with pentobarbital. Cocaine was administered by intravenous infusion over a 2-min interval at 1 mg/kg in 10 cats. In 5 out of 10 cats an additional infusion of 3 mg/kg cocaine was also administered after hemodynamic and electrocardiographic parameters had returned to control values (i.e., within 10 min). During and following infusion of 1 mg/kg cocaine, no significant

Transcatheter electrical ablation of accessory pathways in children.

Supraventricular tachycardia (SVT), the most common sustained symptomatic arrhythmia of childhood, is often supported by a manifest or concealed accessory pathway. Permanent interruption of the accessory pathway usually requires surgical division. Recent experience with electrical ablation of posterior septal pathways in adults prompted us to apply the technique to children. Six children, ages 8 to 15 years, underwent a complete electrophysiological study followed by transcatheter electrical ablation. Five of the 6 children, 3 with a right posterior septal and 2 with a left posterior septal pa

The spectrum of cardiac rate and rhythm in normal newborns.

The distribution and variation of cardiac rate and rhythm in normal neonates has previously received little attention. This has made clinical assessment of dysrhythmia in newborns difficult. We therefore performed continuous 24-hour electrocardiography in 29 normal newborn subjects (age range, 1 to 6 days; mean, 3.5 days). The ECG tapes were then analysed in detail to define the normal range of cardiac rate, conduction intervals, and rhythm during waking and sleeping periods. Maximum sinus rate (awake) ranged from 150 to 222 beats per minute (mean, 192 +/- 16 [SD]), and minimum rate (awake) fr

Continuous on-line beat-to-beat analysis of AV conduction time.

A simple analog circuit is described which is capable of measuring on a beat-to-beat basis P-R, R-P, P-P, and R-R intervals during sinus rhythm and paroxysmal supraventricular tachycardia. In addition the circuit will emit a pulse when the consecutively alternating P and R wave sequence is interrupted thereby signalling a trigger problem or a change in rhythm. The operation of the device requires proper P and R wave sensing and provides outputs which are linear over a range of rates which are applicable to the human heart.

Facilitation of A-V nodal reciprocation by procainamide.

Procainamide is known to depress conduction through the A-V node, and this property may facilitate the development of ventricular reciprocal beats or echoes. The occurrence of ventricular reciprocal beats was studied in 20 open-chest dogs before and after the administration of procainamide. While the ventricle was paced by basic stimuli, early ventricular premature beats were introduced at various coupling intervals to induce ventricular echoes. When ventricular echoes could be induced in a given heart, there was a continuous range of coupling intervals (or echo zone) within which ventricular

Mechanism of supraventricular tachycardia and pharmacological effects on it.

Electrophysiologic study in 30 patients with documented SVT, 16 of whom had preexcitation, and in 24 control subjects were reported. The mechanism of SVT were found to be various and to include reentry confined to the AV node, SA node or the atrium, reentry through accessory pathway and ectopic atrial automaticity. It was noted that some of SVT might use concealed accessory pathway in VA direction while the some of SVT in WPW syndrome might not utilize accessory pathways. Electrophysiological properties in cases with SVT did not differ from those observed in controls. There were no differences

Sites of entrance block and impulse formation in intermittent atrioventricular junctional parasystole.

The Wenckebach phenomenon of entrance block in intermittent atrioventricular junctional parasystole is reported for the first time in a 40-year-old woman. In this case the presence of dual pathways in the atrioventricular junction is demonstrated. One of them is a pathway without conduction disturbance. The other is a pathway containing both the ectopic focus and the site of second-degree entrance block. This site is located a considerably long distance above the focus. Entrance block occurs also at some site below the ectopic focus, which is a part of the pathway containing the focus. It is s

Electrocardiographic observations in bradycardia and tachycardia-dependent atrioventricular block. Relationship to supernormal phase of intraventricular conduction.

This report describes the clinical course of a patient with bradycardia and tachycardia-dependent atrioventricular block. Bradycardia dependent A-V block (phase 4 block) was transient and precipitated by spontaneous slowing of the sinus rate, atrial and ventricular extrasystoles; The degree of slowing (critical RP interval) required to induce A-V block increased progressively over a three-day period. Bradycardia-dependent A-V block was terminated mostly by critically times spontaneous or paced ventricular escape beats, but normally conducted atrial impulses also appeared to restore A-V conduct
