Heart rate variability in frontal lobe epilepsy: Association with SUDEP risk.

OBJECTIVES: Frontal lobe epilepsy (FLE) may impair autonomic heart rate modulation. Decreased heart rate variability (HRV) may enhance risk of sudden death. Our objective was to describe whole day and wakefulness/sleep HRV parameters from FLE patients in comparison with those of healthy controls and correlate HRV parameters to SUDEP-7 scores. METHODS: Ten patients with FLE and 15 healthy controls underwent a 24-hour electrocardiogram holter. The SUDEP-7 score was calculated for patients. Subgroups were identified according to active epilepsy, number of generalized seizures, cognitive deficit,…

Hemodynamic effects of aortic valve and heart rate on coronary perfusion.

BACKGROUND: Reduced coronary flow reserve in aortic stenosis and after transcatheter aortic valve implantation is usually attributed to physiological factors taking place during systole, such as an increase in coronary resistance, and backward waves intensity. In this paper, we suggest an additional factor related to the diastolic hemodynamics in the aortic root. METHODS: We measured left ventricle, aortic and coronary pressure and coronary perfusion in in-vitro models of healthy, aortic stenosis and an artificial valve at different heart rates and cardiac output conditions, to isolate the ef…

Heart rate variability as a biomarker for anorexia nervosa: A review.

OBJECTIVE: Anorexia nervosa (AN) typically begins in early adolescence and other than weight status has few reliable biomarkers. Early diagnosis is a critical prognostic factor, but this can be clinically challenging. Heart rate variability (HRV), the beat-by-beat variance in heart rate (HR), may provide a unique assessment for the presence of AN because it has clinical utility as a biomarker of cardiac autonomic control in various populations (e.g., athletes, the aged, those with cardiovascular diseases, etc.). We present a review of the literature examining HRV in those with AN. METHOD: Rel…

Heart Rate Variability-Measured Stress and Academic Achievement in Medical Students.

OBJECTIVES: Stress can affect learning and memory in students. Previous studies on stress in students were conducted mainly through surveys. We investigated how heart rate variability (HRV)-measured stress is related to academic achievement in medical students during clerkship. SUBJECTS AND METHODS: A cross-sectional study was conducted. HRV measurements were performed in 97 third-year medical students during their family medicine clerkship course. Data on written and skilled examination scores of the end of the school year were also collected. We assessed the association between HRV-measured…

Heart Rate Variability, Cortisol and Attention Focus During Shamatha Quiescence Meditation.

Cardiac vagal tone can be seen as a stable marker for the ability to sustain attention and regulate emotion, two factors associated with the quality of meditation. In this study, heart rate variability (HRV) has been monitored in different forms of Shamatha quiescence meditation: three breath meditations, Settling the Mind in its natural state, Awareness of Awareness, Loving-kindness and Tonglen. Establish and test a practical means of monitoring neurobiological effects of meditation. Over 6 weeks, two experimental groups practiced Shamatha meditation on a daily basis (n = 20). HRV patterns an…

Heart Rate Variability in Individual Psychotherapy: Associations With Alliance and Outcome.

Heart rate variability (HRV) can be conceptualized as a marker of an individual's capability to adaptively respond to its environment and has been linked with mental health. Although conceptually and empirically linked to social behavior and thus relevant in the therapeutic setting, HRV is seldom investigated directly within therapy sessions. In the present examination, we aimed at addressing this research gap by assessing patients' and therapists' HRVs both ambulatory within therapy sessions and under resting conditions. Drawing on polyvagal theory, we hypothesized that higher in-session HRV …

Estimation of Heart Rate Recovery after StairClimbing Using aWrist-Worn Device.

Heart rate recovery (HRR) after physical exercise is a convenient method to assesscardiovascular autonomic function. Since stair climbing is a common daily activity, usually followedby a slow walking or rest, this type of activity can be considered as an alternative HRR test. The present study explores the feasibility to estimate HRR parameters after stair climbing usinga wrist-worn device with embedded photoplethysmography and barometric pressure sensors.A custom-made wrist-worn device, capable of acquiring heart rate and altitude, was used to estimatethe time-constant of exponential decay t, …

Reduction in Heart Rate Variability in Autosomal Dominant Polycystic Kidney Disease.

INTRODUCTION: Cardiovascular disease is one of the main causes of morbidity and mortality in patients with autosomal dominant polycystic kidney disease (ADPKD). Autonomic dysfunction is associated with an increased risk for all cardiovascular events in the general population and can be evaluated with heart rate variability (HRV). OBJECTIVE: To evaluate HRV in ADPKD patients with mild hypertension versus hypertensive patients with organ damage and healthy controls (HC). MATERIALS AND METHODS: We have enrolled 65 patients: 21 ADPKD patients (10 males), 20 patients with hypertension (14 males),…

Fetal heart rate variability with hypoxemia in an instrumented sheep model.

OBJECTIVE: To examine the effect of hypoxemia on fetal heart rate (FHR) variability, using an instrumented pregnant sheep model. METHODS: In this prospective study, 19 pregnant sheep were instrumented under general anesthesia, at a mean gestational age of 127 days. After a 5-day recovery period, hypoxemia was induced by attaching the mother to a rebreathing circuit. Hypoxemia was sustained for 120 min, following which it was reversed until maternal and fetal partial pressure of oxygen (pO2 ) returned to baseline. FHR recordings at baseline, after 30 and 120 min of hypoxemia and at recovery we…

Heart Rate Variability Biofeedback to Treat Fibromyalgia: An Integrative Literature Review.

OBJECTIVES: Fibromyalgia (FM) is associated with debilitating pain and a reduced heart rate variability (HRV), reflecting decreased emotional adaptability and resistance to stress. Common pharmacological treatments are ineffective, and opioids are highly addictive and cause an estimated 15,000 overdose deaths per year. Effective recommendations include patient-centered interventions like physical activity, cognitive behavioral therapy, and biofeedback. Heart rate variability biofeedback (HRVB) may be effective in improving HRV, thus increasing stress resistance and emotional adaptability and r…

Necrotizing enterocolitis attenuates developmental heart rate variability increases in newborn rats.

BACKGROUND: We have shown previously that a decreased high-frequency spectrum of heart rate variability (HF-HRV), indicative of reduced vagal tone, shows promise in predicting neonates likely to develop necrotizing enterocolitis (NEC) before its clinical onset. We hypothesized that NEC induction in rat pups decreases HF-HRV power; subdiaphragmatic vagotomy worsens the severity of the NEC phenotype, increases levels of pro-inflammatory cytokines, and alters the myenteric phenotype. METHODS: Newborn Sprague-Dawley rats, representative of preterm human neonates, were subjected to 7-8 days of bri…

The effects of fasting on heart rate variability in hypertensive patients.

Heart rate variability (HRV) is an independent indicator of increased mortality in patients with myocardial infarction and congestive heart failure. The effects of fasting on the HRV are not known in hypertensive patients. Therefore, studying the effects of Ramadan fasting on hypertensive patients' HRV seems reasonable to address. We conducted a prospective study including 20 hypertensive patients with sinus rhythm. HRV was determined twice by ambulatory 24-hour Holter recordings at fasting during and after Ramadan. Subjects mean age was 55 ± 11.8 years. Sex-ratio was 1.5. When two groups comp…

Heart Rate as a Predictor of Outcome Following Percutaneous Coronary Intervention.

Data from previous studies of patients with heart failure and coronary artery disease suggest that those with higher resting heart rates (HRs) have worse cardiovascular outcomes. We sought to evaluate whether HR immediately before percutaneous coronary intervention (PCI) is an independent predictor for 30-day outcome. We analyzed the outcome of 3,720 patients who had HR recorded before PCI from the Melbourne Interventional Group registry. HR and outcomes were analyzed by quintiles, and secondarily by dichotomizing into <70 or ≥70 beats/min. Patients with cardiogenic shock, intra-aortic balloon…

Evaluation of Heart Rate Variability in Drug Free Panic Disorder Patients.

INTRODUCTION: It is well-known that autonomic function is impaired in panic disorder (PD). Heart rate variability (HRV) is a reliable and noninvasive method to evaluate autonomic functions. The aim of this study is to evaluate 24-hour heart rate variability (HRV) parameters in drug-free panic disorder patients. These results are compared with those obtained from healthy controls. METHODS: Thirty-two (mean age 33.12±11.94 years, 20 female, 12 male) drug-free patients for at least 2 weeks, (for fluoksetin 3 weeks) diagnosed with panic disorder (PD) on the basis of DSM-IV criteria were included …

The correlation of heart rate between natural sleep and dexmedetomidine sedation.

BACKGROUND: Sedation by dexmedetomidine, like natural sleep, often causes bradycardia. We explored the nature of heart rate (HR) changes as they occur during natural sleep versus those occurring during dexmedetomidine sedation. METHODS: The present study included 30 patients who were scheduled to undergo elective surgery with spinal anesthesia. To assess HR and sedation, a pulse oximeter and bispectral index (BIS) monitor were attached to the patient in the ward and the operating room. After measuring HR and BIS at baseline, as the patients slept and once their BIS was below 70, HR and BIS we…
