Infections in Patients with Mechanical Circulatory Support.

Patients on mechanical circulatory support are at heightened risk for infection given the invasive nature of the devices with internal and external components, the surgical implantation of the devices, and the presence of foreign material susceptible to biofilm formation. This review discusses the new International Society for Heart and Lung Transplantation mechanical circulatory support device infection definitions, inclusive of durable and acute mechanical circulatory support infections, and describes their epidemiology, diagnosis, and management. A multidisciplinary approach is essential fo

Non-patient factors associated with infections in LVAD recipients: A scoping review.

BACKGROUND: Infections are the most common complication in recipients of durable left ventricular assist devices (LVAD) and are associated with increased morbidity, mortality, and expenditures. The existing literature examining factors associated with infection in LVAD recipients is limited and principally comprises single-center studies. This scoping review synthesizes all available evidence related to identifying modifiable, non-patient factors associated with infections among LVAD recipients. METHODS: Published studies were identified through searching 5 bibliographic databases: PubMed, Sc

Importance of realistic LVAD profiles for assisted aortic simulations: evaluation of optimal outflow anastomosis locations.

Left ventricular assist devices (LVADs) are carefully designed, but the significance of the implantation configuration and interaction with the vasculature is complex and not fully determined. The present study employs computational fluid dynamics to investigate the importance of applying a realistic LVAD profile when evaluating assisted aortic flow fields and subsequently compares a number of potential anastomosis locations in a patient-specific aortic geometry. The outflow profile of the Berlin Heart INCOR® device was provided by Berlin Heart GmbH (Berlin, Germany) and the cannula was attach

Economics of devices.

BACKGROUND: The economics of devices used for mechanical circulatory support not only involve the patient, the provider, and society as a whole, but also, importantly, the developers and manufacturers of these new technologies. The combined effects of years of development and testing with significant regulatory, reimbursement, and acceptance barriers make this a capital-intensive and high-risk endeavor. In addition, long-term circulatory support is, today, essentially limited to bridge to transplantation, a "market" of only $100 million. Competition is increasing, with new devices under develo

New investigations of a pulsatile impeller blood pump.

For circulatory assist devices and total artificial heart systems, impeller blood pumps with small total volumes would be fully implantable. One of the main obstacles, however, is generation of a pulsatile flow. The simplest way to overcome this problem is by changing the pump's revolutions per minute (rpm) periodically, but this often results in severe hemolysis. After theoretic analysis, two in vitro models of impeller blood pumps have been devised, producing pulsatile flow with constant rpm. In the first model, the impeller oscillates in an axial direction during constant rotation. The pump

Immunocompetent cells in durable ventricular assist device-implanted non-ischaemic dilated cardiomyopathy.

OBJECTIVE: Because the presence of immunocompetent cells in the myocardium is associated with the pathological stage and/or myocardial viability, we explored relationships between functional recovery after left ventricular assist device implantation and the distribution of immunocompetent cells in non-ischaemic dilated cardiomyopathy patients. METHODS: We reviewed 50 consecutive dilated cardiomyopathy patients implanted with HeartMate II at our institute between April 2013 and December 2018 who were treated with optimal medical therapy during left ventricular assist device support. Patients w

Successful Pediatric Mechanical Thrombectomy for HeartMate 3-Related Intracranial Thromboembolism.

Thromboembolic events post left ventricular assist devices (LVAD) implantation remain a major cause of morbidity and mortality. Mechanical thrombectomy for the treatment of pediatric intracranial thromboembolic events have been reported in LVADs, but never following HeartMate 3 (HM3) implantation. We present the case of an 8-year-old, 26.5 kg male with dilated cardiomyopathy and decompensated heart failure who presented with extensive intracranial thromboembolism in the early postoperative period following HM3 implantation and underwent successful mechanical thrombectomy with a favorable neuro

Perioperative management in pediatric heart transplantation.

Peri- and early postoperative mortality significantly influences the probability of survival following heart transplantation in children. Main causes of death early after transplantation are rejection, non specific graft failure and RV failure due to pulmonary hypertension. Optimal therapy of pulmonary hypertension and aggressive use of assist devices as a bridge to recovery will substantially improve survival in the early period after transplantation. Furthermore, the use of marginal donor organs will be more acceptable because transient myocardial insufficiency may recover during extracorpor

Changing pulsatility by delaying the rotational speed phasing of a rotary left ventricular assist device.

Continuous-flow left ventricular assist devices (LVADs) have improved the prognosis of end-stage heart failure. However, continuous-flow LVADs diminish pulsatility, which possibly result in bleeding, aortic insufficiency, and other adverse effects. We previously developed a novel control system for a continuous-flow LVAD (EVAHEART®; Sun Medical), and demonstrated that we could create sufficient pulsatility by increasing its rotational speed (RS) in the systolic phase (Pulsatile Mode) in the normal heart model. Here, we aimed to evaluate differences between systolic assist with advanced and del

A blood assist index control by intraaorta pump: a control strategy for ventricular recovery.

Left ventricular assist devices (LVAD) are increasingly used for long-term support in heart failure patients. To promote ventricular reverse remodeling, a defined and adjustable energy distribution of LVAD and native heart is important. Therefore, a blood assist index (BAI), which is a ratio of power of LVAD and total power of the cardiovascular system, is defined to indicate the energy distribution of LVAD and native heart. Subsequently, an LVAD control algorithm that uses the BAI as control input is designed. The control strategy maintains the measured BAI tracking the desired BAI. A mathema

Beyond Continuous-Flow: The Next Wave of Fully Implantable LVADs and Total Artificial Hearts.

PURPOSE OF REVIEW: Ventricular assist devices (VADs) have evolved from a bridge-to-transplantation strategy into a cornerstone of long-term advanced heart failure management. While continuous-flow technology has dramatically improved survival and device reliability, the paradigm shift toward extended mechanical circulatory support has exposed critical unresolved challenges. This review examines current limitations and four transformative technological directions shaping the next generation of VAD therapy. RECENT FINDINGS: Four key areas are driving innovation in the field. First, growing evid

Primary graft failure after heart transplantation. Successful recovery with pneumatic biventricular assistance.

A 41-year-old male underwent orthotopic heart transplantation complicated by intraoperative acute allograft failure. The patient required immediate placement of a pneumatic biventricular assist device which was kept for 49 days until graft recovery resulted in successful explantation of the device. The patient was discharged from hospital on postoperative day 112. Management of primary cardiac allograft failure with mechanical ventricular assistance is discussed.

Advanced Heart Failure Treatment Modalities and Hospice Care: The Need for High Level Care Coordination.

Advanced heart failure therapies such as ventricular assist devices and home inotrope use are becoming more common. Technology advances as well as increased indications for use of such therapies is leading to a higher percentage of patients with end-stage heart failure receiving these therapies at end of life. We present a case of a young man with dilated cardiomyopathy who undergoes advanced cardiac care in the setting of progressively declining cardiac function. Our case outlines the importance of acute care, palliative care, and hospice services being coordinated prior to and during acute-c

[Heart and lung transplantation in acute organ failure].

Heart and lung transplantation represent a realistic therapeutical option in endstage cardiac and pulmonary diseases. Transplantation for acute organ failure, however, is limited due to the scarcity of suitable donors with respect to the shortage of time. Aside from pharmacological means a number of mechanical assist devices are available for bridging of those patients in acute myocardial or respiratory failure. Indications for acute organ transplantation are primary graft failure following heart or lung transplantation and acute myocardial insufficiency due to extensive myocardial infarction

Preoperative risk factors for mortality after biventricular assist device implantation.

BACKGROUND: A right ventricular assist device is a treatment option for patients with severe right ventricular failure after left ventricular assist device (LVAD) implantation. Recognition of risk factors for mortality after biventricular assist device (BiVAD) implantation is important for patient selection and optimal outcomes. METHODS: We reviewed our experiences between 1991 and 2005 in 44 patients who were supported by both an LVAD and a right ventricular assist device. RESULTS: Thirteen patients (30%) survived until heart transplantation, and 31 patients (70%) died while on support. The

Acute humoral rejection in an infant without risk factors after heart transplantation.

Graft rejection is the most common factor that limits graft survival after transplantation. During infancy, the humoral immune system is partially suppressed and humoral rejection of a cardiac allograft has not been reported in the absence of risk factors such as prior transplantation, blood transfusions, ventricular assist devices, and elevation of panel reactive antibodies. We present a case of an infant with dilated cardiomyopathy who developed multiple episodes of acute humoral rejection after heart transplantation in the absence of risk factors.

Total support of the circulation of a patient with post-cardiotomy stone-heart syndrome by a partial artificial heart (ALVAD) for 5 days followed by heart and kidney transplantation.

A patient with acute bacterial endocarditis in whom ischaemic contracture of the left ventricle (stone-heart syndrome) developed during aortic and mitral valve replacement had an emergency implantation of an intracorporeal partial artificial heart (an abdominal left-ventricular assist device of ALVAD). This device functioned as a total artificial heart for nearly 6 days, while a donor heart for transplantation was sought. The ALVAD was then removed, and the patient received allografts of a heart and a kidney. The transplanted heart functioned well, but the patient died 15 days later from gram-

Quality of life with an LVAD: A misunderstood concept.

The present study aims to synthesize current evidence on the impact of LVAD implantation on quality of life. Current evidence was systematically reviewed to obtain relevant quantitative and qualitative articles published after 2007. Sandelowski's recommended steps for meta-summary were used to analyze the 19 studies that met the inclusion criteria. LVADs can improve HF symptoms and some aspects of QoL. Emotional and physical adaptation involves many changes and learning to manage the device takes time. Functional limitations still exist and patients still lack independence. LVAD-related compli
