Clostridium difficile colitis associated with treatment of Helicobacter pylori infection.

Helicobacter pylori infection of the stomach is being detected and treated more often now than ever before. This is likely to result in an increase in complications such as antibiotic-associated diarrhea. However, there is no literature on the incidence of such diarrhea, particularly Clostridium difficile colitis, in patients treated for Helicobacter pylori infection. We report the case of a patient who developed Clostridium difficile colitis after treatment for Helicobacter pylori infection with metronidazole, amoxicillin, H2 blockers, and bismuth subsalicylate. This patient presented with se

Transcriptional expression of gastrin mRNA in Helicobacter pylori infected patients.

In order to study the association between gastrin and H. pylori infection the density of antral G cells was evaluated by transcriptional expression of gastrin mRNA using a sensitive cold probe labelled with digoxigenin. the study group included 22 patients with symptomatic H. pylori positive gastritis and/or duodenal ulcer, 12 of whom were re-evaluated after eradication of H. pylori and 6 controls. The number of G cells per high power field in H.pylori positive patients (26.68 +/- 9.51) was significantly higher (P < 0.01) compared to controls (5.83 +/- 3.37). Among the 12 patients re-evaluated

Failed treatment of Helicobacter pylori infection associated with resistance to clarithromycin.

BACKGROUND: Resistance of Helicobacter pylori to clarithromycin is uncommon. Initial studies have suggested that primary resistance has a significant adverse effect on bacteriological cure rates and acquired resistance develops frequently with failure of treatment following regimens containing clarithromycin. MATERIALS AND METHODS: H. pylori isolates were obtained from patients with duodenal ulcer treated with clarithromycin and omeprazole and examined for susceptibility to clarithromycin using the E-test method. A 13C urea breath test was used to confirm infection with H. pylori and successf

Sucrose permeability in children with gastric damage and Helicobacter pylori infection.

BACKGROUND: Increased permeability to sucrose has been recently shown to be a good marker of gastric mucosal damage in adults. METHODS: This test was evaluated in 40 children consulting for recurrent abdominal pain and the results were correlated with endoscopic and histologic findings and with the presence of H. pylori. RESULTS: The gastric mucosa was considered endoscopically normal in 31 children; 3 had duodenitis and 6 had mild gastritis. Abnormal endoscopic findings were associated with increased urinary sucrose excretion (MANOVA F = 7.30; p = 0.002). In the 6 children with mild gastrit

Is duodenal gastric metaplasia a consequence of Helicobacter pylori infection in children?

BACKGROUND: Duodenal gastric metaplasia (DGM) is commonly found in association with Helicobacter pylori (Hp)-associated gastritis in adults. DGM is also considered a risk factor for duodenal ulcer development. The prevalence of DGM in children and its association with gastritis, duodenitis, or the presence of Hp organisms is not clear. We investigated the prevalence of DGM in children and explore its association with several possible risk factors, including age, gender, gastritis, duodenitis, or Hp presence in the gastric antrum. METHODS: A retrospective analysis of 173 upper endoscopy proced

Relation between Helicobacter pylori infection and gastrointestinal symptoms and syndromes.

BACKGROUND: Helicobacter pylori is a human pathogen that colonises the gastric mucosa and causes permanent gastric inflammation. AIMS: To assess the symptoms of H pylori infection in an adult unselected population. SUBJECTS: A random sample of 3589 adult Danes who were examined in 1982 and 1987 (n = 2987). METHODS: Abdominal symptoms within the preceding year were recorded at both attendances. Circulating IgG antibodies against H pylori in serum samples drawn in 1982 were measured by using in-house indirect enzyme linked immunosorbent assays (ELISA). RESULTS: People with increased levels o

Mast cell involvement in gastritis with or without Helicobacter pylori infection.

BACKGROUND & AIMS: Mast cells are initiators and regulators of inflammation, but their role in the human stomach remains unclear. Therefore, the extent and distribution of mast cell involvement in gastritis with or without Helicobacter pylori infection was investigated. METHODS: Mapped biopsy specimens from 17 H. pylori-positive and 20 H. pylori-negative subjects were examined. Sections were assessed for infection and inflammation and stained with anti-human mast cell tryptase to count mucosal and epithelial mast cells. Density of mast cells in different gastric compartments, their response t

A possible role of Helicobacter pylori infection in the etiology of chronic laryngitis.

In order to study a possible role of Helicobacter pylori infection in chronic laryngitis, we performed endoscopic and histological assessments in addition to a urease test for the bacterium in 35 patients with chronic hoarseness. Six of the patients investigated (17.1%) revealed a positive urease test of the laryngeal biopsy (four male and two female patients). These H. pylori-positive patients were treated with omeprazole and an antibiotic regimen using clarithromycin and metronidazole. This led to an eradication of the H. pylori and resolution of clinical signs and symptoms. These findings s

[B-cell lymphoma (MALToma) of the stomach and Helicobacter pylori infection].

The authors, after illustrating physiopathologic aspects of gastric MALTomas, examine the H.P. infections and the possible relations between this kind of bacterium and gastric MALTomas. They hypothesize that H.P. infection represents an important predisposition to gastric lymphoma, due to anatomopathological modifications over gastric mucosa. They conclude that a decreasing of gastric lymphoma could be correlatable an opportune eradication of H.P. despite of at moment it's impossible hypothesize a marked decreasing of lymphoma by an eradication of H.P.

Helicobacter pylori infection in Thai children with recurrent abdominal pain.

Sixty-five children with recurrent abdominal pain underwent gastrointestional endoscopy which showed Hp gastritis without duodenal ulcer in 16.9 per cent of cases. The prevalences of infection in recurrent abdominal pain and asymptomatic children were not different. Either urease test or histological method was appropriate for diagnosis of this infection. Triple therapy including bismuth subcitrate, amoxicillin and metronidazole improved abdominal pain symptom in 72.7 per cent without any side effect.

Parietal cells in the duodenal bulb and their relation to Helicobacter pylori infection.

AIM: To investigate the prevalence, and relation to Helicobacter pylori, of parietal cells in the duodenal bulb using a monoclonal antibody directed against H+,K(+)-ATPase (HK12.18). METHODS: Twenty six patients with duodenal ulcer disease and 16 healthy controls were studied. H pylori status was determined by gastric histology and culture and by the 13C-urea breath test. Four biopsy specimens were taken from the duodenal bulb and stained with HK12.18. The presence/absence and number of parietal cells in the duodenal bulb were assessed blindly by a histopathologist. RESULTS: The overall prev

The economics of eradicating Helicobacter pylori infection in duodenal ulcer disease.

Approximately 5 million people in the United States suffer from peptic ulcer disease, making this an important clinical problem. In addition to the huge costs associated with treatment, peptic ulcer disease also results in losses to industry as a result of loss of productivity. Infection with Helicobacter pylori is now accepted as the cause of duodenal ulcer in the majority of patients. Eradication of this infection leads to healing of the ulcer and prevents disease recurrence. A number of treatment regimens have been described for the eradication of H. pylori, but there is uncertainty regardi

Helicobacter pylori infection. The importance of eradication in patients with gastric disease.

Discovery of Helicobacter pylori infection in patients with peptic ulcer disease has completely transformed the approach to the disease. Implications of this discovery for healthcare in general and for medical economics are only now being appreciated. Noninvasive and invasive methods of establishing the presence of infection are currently available, and clinical introduction of the recently approved urea breath test in the near future will add another method for documenting both active infection and successful eradication. Patients with initial presentation or recurrence of gastric or duodenal

Modified rapid urease test for detection of Helicobacter pylori infection.

BACKGROUND: The rapid urease test is the most widely used standard procedure for the detection of Helicobacter pylori infection. This is because it is a simple, reliable and inexpensive test, which provides results rapidly. AIM: The aim of the present study was the evaluation of a modified rapid urease test (HUT-test) in comparison with the standard CLO-test. In addition, it was questioned whether the performance of the rapid urease test in two biopsy samples from antrum and gastric body would increase the sensitivity of the test. METHODS: One hundred and fifteen consecutive patients undergo

Histological responses to Helicobacter pylori infection: gastritis, atrophy and preneoplasia.

It is interesting that the principal histological features of acute H. pylori gastritis, surface epithelial degeneration and neutrophil polymorph infiltration, remain as the most sensitive indicators of the 'activity' of infection in the chronic phase. It is not surprising therefore that these are the first features to resolve after successful H. pylori eradication therapy. In one of the earliest studies of histological response to eradication, McNulty et al (1986) endoscoped patients immediately after a three-week treatment regime and found a highly significant decline in polymorph scores. Schönlein-Henoch purpura associated with gastric Helicobacter pylori infection.

Schönlein-Henoch purpura is characterized by palpable purpura, colicky abdominal pain, gastrointestinal hemorrhage, arthralgias, and renal involvement. Bacterial and viral infections, as well as drugs and diseases associated with immune complexes, are thought to be responsible. We describe the case of a 21-year-old woman with Schönlein-Henoch purpura and chronic active gastritis with erosions. Helicobacter pylori was found in gastric mucosa using the newly introduced, nontoxic, noninvasive 13C-urea breath test; infection was confirmed by gastric mucosal biopsy. After eradication of H. pylori w

Peptic ulcer and duodenal stenosis: role of Helicobacter pylori infection.

A 17-year-old boy who developed a symptomatic duodenal ulcer at 10 years of age with melena, and was then treated continuously for 6 years with ranitidine therapy that only partially controlled symptoms and peptic lesions, came to us with vomiting due to duodenal bulb stenosis and active ulcer. Four months of omeprazole (40 mg/die o.m.) did not modify the endoscopic picture. The diagnosis of H. pylori infection and its treatment with triple therapy led to the cure of both duodenal ulcer and bulbar stenosis. Afterwards he remained asymptomatic without any lesions or complications for 18 months.
