Effects of nocturnal periodic breathing on sympathetic nerve activity and ventilatory control at high altitude: a randomised, crossover study.

During acute exposure to the hypoxia of high altitude, activation of the peripheral chemoreflex increases sympathetic nerve activity (SNA) and pulmonary ventilation. If exposure extends over several days, SNA and ventilation further increase and we investigated whether nocturnal periodic breathing (nPB) - a form of sleep-disordered breathing that is common at high altitude - contributes to these further increases. In a randomised, placebo-controlled, crossover protocol, twelve healthy men completed two 3-day sojourns in hypobaric hypoxia equivalent to 4000 m altitude. nPB was inhibited by incr

Synchronized mechanical ventilation for respiratory support in newborn infants.

BACKGROUND: During synchronized mechanical ventilation, positive airway pressure and spontaneous inspiration coincide. Thus, if synchronous ventilation is provoked, adequate gas exchange should be achieved at lower peak airway pressures, potentially reducing barotrauma and hence airleak and chronic lung disease. Synchronous ventilation can be achieved by manipulation of rate and inspiratory time during conventional ventilation and employment of patient assisted ventilation. OBJECTIVES: To compare (i) the efficacy of synchronized mechanical ventilation, delivered as high frequency positive pre

Effects of hypoxia and hypercapnia on human HRV and respiratory sinus arrhythmia.

PURPOSE: Hypercapnia increases minute ventilation (V’E) with little effect on heart rate (HR), whereas hypoxia may increase HR without affecting V’E. However, the effects of hypercapnia and hypoxia on both heart rate variability(HRV) and the clustering of heart beats during spontaneous breathing (respiratory sinus arrhythmia – RSA), are not clear. METHODS: In this study, 10 human volunteers breathed room air (RA), hypercapnic (5% CO2) or hypoxic (10%O2) gas mixtures, each for 6 min, while resting supine. ECG, mean arterial pressure (MAP), ventilatory flow, inhaled and exhaled fractions of CO2

Neonatal Ventilation Training Using a Low-Cost, High-Fidelity Simulator in a Low-Resource Setting: A Randomized Clinical Trial.

IMPORTANCE: Neonatal mortality rates in sub-Saharan Africa remain high (26 per 1000 live births). Limited access to advanced respiratory support and shortage of trained health personnel make manual ventilation a lifesaving, yet high-risk, essential competence. Simulation-based training may strengthen neonatal ventilation skills, but evidence on acceptability and educational effectiveness of low-cost, high-fidelity simulators in resource-limited settings is limited. OBJECTIVE: To build local capacity in manual neonatal ventilation and to assess acceptability and educational effectiveness of di

Update of the taxonomy of mechanical ventilation modes.

The rapid technological development of mechanical ventilation has resulted in increasingly complex modes, advanced monitoring capabilities and the incorporation of artificial intelligence. However, manufacturers have created a multitude of trade names, which has generated a great deal of confusion in their understanding, handling and application. This problem is exacerbated in Spanish-speaking countries due to inconsistencies in translations and variability in nomenclature between regions. This manuscript aims to provide an updated review of the taxonomic classification of ventilatory modes in

Numerical study of high-frequency oscillatory air flow and convective mixing in a CT-based human airway model.

High-frequency oscillatory ventilation (HFOV) is considered an efficient and safe respiratory technique to ventilate neonates and patients with acute respiratory distress syndrome. HFOV has very different characteristics from normal breathing physiology, with a much smaller tidal volume and a higher breathing frequency. In this study, the high-frequency oscillatory flow is studied using a computational fluid dynamics analysis in three different geometrical models with increasing complexity: a straight tube, a single-bifurcation tube model, and a computed tomography (CT)-based human airway mode

Reduction in ventilator-induced lung injury improves outcome in congenital diaphragmatic hernia?

The purpose of this historical study was to compare the outcome for two treatment strategies, for neonates with congenital diaphragmatic hernia (CDH). The records of 65 infants born between 1991 and 2005 with CDH from a single tertiary care perinatal centre in the United Kingdom were retrospectively reviewed. Conventional mechanical ventilation (CMV) and systemic vasodilators were used from 1991 to 1995 (era 1). High frequency oscillatory ventilation (HFOV) and nitric oxide (NO) were used between 1996 and 2005 (era 2). Main outcome measures were survival and incidence of chronic lung disease.

[Propofol anesthesia for fetal sedation].

We were requested to reduce neonatal respiratory effort at delivery in anesthetic management for Cesarean section. A 26-year-old pregnant woman was suspected through abdominal ultrasound examination and amniotic fluid test, of her baby having immature lungs associated with remarkable pleural effusion. Lungs could be damaged by respiratory effort after delivery, and respiratory management immediately after delivery was planned. Anesthesia was induced with propofol and fentanyl 300 micrograms. Propofol was administered with a target controlled infusion setting with the target blood concentration

Secondary infection presenting as recurrent pulmonary hypertension.

Primary infection in the neonate, especially group B streptococcal infection, has long been recognized as a cause of persistent pulmonary hypertension of the newborn (PPHN), sometimes requiring treatment with inhaled nitric oxide (iNO) and extracorporeal membrane oxygenation (ECMO). However, secondary nosocomial infections in the neonatal period have not been widely reported as a cause of severe recurrent pulmonary hypertension (PHTN). We now present two cases of secondary infection in the neonate leading to significant PHTN. In both cases, the infants presented with PPHN soon after birth, req

Inhaled nitric oxide therapy for pulmonary disease in pediatrics.

Inhaled nitric oxide (iNO) therapy improves gas exchange and lowers pulmonary vascular resistance in neonates and children with diverse cardiac and pulmonary disorders. Recent multicenter studies have shown that iNO reduces the need for rescue therapy with extracorporeal membrane oxygenation in term neonates with severe hypoxemic respiratory failure. In neonates with severe lung disease, responsiveness to iNO therapy can be enhanced by therapies which enhance lung recruitment, especially high-frequency oscillatory ventilation. Although early studies suggest that iNO therapy is not associated w

High-frequency oscillation in the rescue of infants with persistent pulmonary hypertension.

High-frequency oscillatory ventilation (HFOV) was used to treat 41 infants with persistent pulmonary hypertension of the newborn (PPHN). Of the 37 patients who showed early improvement on HFOV, three died. The remaining 34 patients demonstrated, within one hour of the switchover to HFOV, a rise in mean arterial/alveolar oxygen tension ratio (PaO2/PaO2) from 0.093 +/- 0.041 (SD) to 0.132 +/- 0.051 (p less than .001), and a fall in mean PaCO2 from 42 +/- 10 to 34 +/- torr 9 (p less than .01). Mean airway pressure (Paw) fell significantly (p less than .01) within 12 h. The mean duration of conven
