Discrimination, Abuse, Harassment, and Burnout in Surgical Residency Training.

BACKGROUND: Physicians, particularly trainees and those in surgical subspecialties, are at risk for burnout. Mistreatment (i.e., discrimination, verbal or physical abuse, and sexual harassment) may contribute to burnout and suicidal thoughts. METHODS: A cross-sectional national survey of general surgery residents administered with the 2018 American Board of Surgery In-Training Examination assessed mistreatment, burnout (evaluated with the use of the modified Maslach Burnout Inventory), and suicidal thoughts during the past year. We used multivariable logistic-regression models to assess the a

Crossing the Communication Chasm: Challenges and Opportunities in Transitions of Care from the Hospital to the Primary Care Clinic.

BACKGROUND: Transitions of care from specialty and acute settings to primary care abound. Compared to the continuity in end-of-shift handoffs, care transitions involve provider communication between practices and facilities with their own cultures and bureaucracies. Using the transition from acute care to outpatient primary care for stroke/transient ischemic attack (TIA) patients as a case study, this qualitative research explored communication practices and institutional arrangements among clinical providers responsible for longitudinal management of hypertension. In this study, researchers i

[Management strategies of eating disorders in adults].

The importance of the acute and chronic impact of eating disorders in adults, from both a somatic and psychological point of view, requires the means used to be commensurate with the potential severity of these disorders. Moreover, the reluctance--or even the opposition--of patients to the treatment increases the difficulty in the management of these already complex disorders. In any event, besides the patient's active collaboration during the treatment, which should be reached gradually, the therapeutic strategy to be adopted should be multimodal and integrated. In addition to the primary car

Mitigating disrespect and abuse during childbirth in Tanzania: an exploratory study of the effects of two facility-based interventions in a large public hospital.

BACKGROUND: There is emerging evidence that disrespect and abuse (D&A) during facility-based childbirth is prevalent in countries throughout the world and a barrier to achieving good maternal health outcomes. However, much work remains in the identification of effective interventions to prevent and eliminate D&A during facility-based childbirth. This paper describes an exploratory study conducted in a large referral hospital in Dar es Salaam, Tanzania that sought to measure D&A, introduce a package of interventions to reduce its incidence, and evaluate their effectiveness. METHODS: After exte

Is cooperation better than competition? Evidence from patient mobility before and after the COVID-19 pandemic in Tuscany, Italy.

INTRODUCTION: According to Italian Law, citizens have the right to choose the preferred place of care; in this context, the competition between various hospitals across the national territory is inevitable. The study aims to analyze the associations between patient mobility between Tuscany (Italy) and other Italian regions and the weight of Diagnosis Related Groups (DRGs) provided for these hospitalizations, as a proxy of complexity of care provided. METHODS: A retrospective study was conducted using 2019 and 2022 Hospital Discharge Cards data of Tuscany provided by the Italian Ministry of He

Addiction Consult Services, Mortality, and Acute Care Utilization in Inpatients With Opioid Use Disorder: A Secondary Analysis of a Cluster Randomized Clinical Trial.

IMPORTANCE: With acute care utilization and mortality rates increasing among people with opioid use disorder, hospital addiction consult services can provide an important touchpoint for care, potentially leading to improved outcomes. OBJECTIVE: To study the effectiveness of interprofessional hospital addiction consultation services on postdischarge acute care utilization and mortality. DESIGN, SETTING, AND PARTICIPANTS: In this pragmatic stepped-wedge cluster randomized implementation and effectiveness (hybrid type 1) clinical trial, 6 New York City public hospitals were randomized to an int

Medications for opioid use disorder prescribed at hospital discharge associated with decreased opioid agonist dispensing in patients with opioid use disorder requiring critical care: A retrospective study.

INTRODUCTION: Buprenorphine is highly effective for the treatment of opioid use disorder (OUD), and, in recent years, the rates of patients maintained on buprenorphine requiring critical care have been steadily increasing. Currently, no unified guidance exists for buprenorphine management during critical illness. Likewise, we do not know if patients maintained on buprenorphine for OUD are prescribed medications for OUD (MOUD) following hospital discharge or if buprenorphine management influences mu opioid agonist dispensing. METHODS: In our cohort of adults over the age of 18 with OUD, receiv

Hospital-initiated Extended-release Injectable Buprenorphine Using a Novel Reallocation Initiative From an Outpatient Addiction Medicine Clinic.

OBJECTIVES: Novel strategies for initiation and continuation of buprenorphine are critical, especially during a pandemic when traditional opioid use disorder treatment pathways may be disrupted. We describe an innovative outpatient to inpatient reallocation initiative for extended-release buprenorphine (XR-BUP) designed to repurpose an expensive medication for use in hospitalized patients facing treatment barriers upon discharge and pilot the feasibility of XR-BUP use in the inpatient setting. METHODS: We collaborated with our institution's inpatient pharmacy and a New Jersey Medicaid managed

A comparison of perceptions of caring behaviours among women in homelessness, Registered Nurses and nursing students.

BACKGROUND: The population of women in homelessness in Europe is increasing and women in homelessness experience multiple healthcare needs. However, there is insufficient understanding about how perceptions of caring behaviours compare between women in homelessness as patients and nurses in their clinical practice. AIM: This study aimed to investigate perceptions of caring behaviours among women in homelessness, Registered Nurses and nursing students. METHODS: A cross-sectional design was used with convenience sampling to recruit groups of women in homelessness (n = 37), Registered Nurses (n

Diagnosis and Management of Opioid Use Disorder in Hospitalized Patients.

The diagnosis of opioid use disorder (OUD) is often overlooked or inadequately managed during the inpatient admission. When recognized, a common strategy is opioid detoxification, an approach that is often ineffective and can be potentially dangerous because of loss of tolerance and subsequent risk for overdose. Medication for addiction treatment (MAT), including methadone and buprenorphine, is effective and can be dispensed in the hospital for both opioid withdrawal and initiation of maintenance treatment. Hospitalists should be knowledgeable about diagnosing and managing patients with OUD, i

Ethical issues in knowledge, perceptions, and exposure to hospital hazards by patient relatives in a tertiary institution in North Western Nigeria.

INTRODUCTION: Patient relatives are a significant part of the health care team in any hospital setting. This study determines their knowledge, perceptions, and factors responsible for their exposure to health hazards at the Ahmadu Bello University Teaching Hospital (ABUTH), Zaria, Kaduna State. The outcome of this study is intended to help the hospital managers to have a reference to make plans for the patient relatives' welfare and also address the ethical issues regarding patients' relatives safety while in the hospital. MATERIALS AND METHODS: This is a cross-sectional descriptive study inv

Patient-related violence against emergency department nurses.

In a finding that reflects international experiences, nurses in Australia have been identified as the occupation at most risk of patient-related violence in the health-care sector. A search of the literature was undertaken to explore this concept, with a focus on the emergency department and triage nurses. Significant findings included the fact that nurses are subjected to verbal and physical abuse so frequently that, in many instances, it has become an accepted part of the job. This attitude, combined with the chronic under-reporting of violent incidents, perpetuates the normalization of viol

[Team management of orofacial clefts].

In the Netherlands 15 centres provide multidisciplinary care for cleft lip and palate patients. Usually the following disciplines participate in such teams: paediatrics, plastic and reconstructive surgery, orthodontics, genetics, social work or nursing, ENT, speech therapy, maxillofacial surgery, prosthetic dentistry, psychology and oral hygiene. An overview is given of the treatment protocol from birth until 20 years of age for a child with a complete UCLP or BCLP. It is concluded that properly designed prospective clinical trials are rare, resulting in a lack of evidence based care in the fi

[Misuse of inpatient care--background, indicators and evaluation parameters].

17a of the German hospital financing law constitutes the basis for the evaluation of not justified hospital admission. It demands not to admit patients into hospital who do not need inpatient treatment or, treat them in an inpatient-setting longer than medical required. Unjustified inpatient treatment results from faulty decision with regard to admission, to adequate intensity of medical care or duration of stay. The priority of outpatient treatment over the pre-, post-, partial and complete inpatient treatment must be taken into consideration at each decision. The following items can indicate

Impact of substance use and trauma on postpartum readmission rates and hospitalization costs: An observational study.

INTRODUCTION: Substance use disorder (SUD) has been seen to be a contributing factor in the rise in maternal morbidity and mortality in the United States, and more information is needed regarding the impact of SUD on peripartum outcomes. We hypothesized that patients with an SUD diagnosis at the time of delivery would have higher rates of readmission compared with patients without an SUD diagnosis. In addition, we assessed the impact of a trauma-/stress-related diagnosis, both alone and when cooccurring with SUD, at the time of delivery on readmission and healthcare utilization, given the high

Catheter-Associated Urinary Tract Infection Reduction in a Pediatric Safety Engagement Network.

BACKGROUND: Catheter-associated urinary tract infections (CAUTIs) are a leading cause of health care-associated infection. Catheter insertion bundles (IBs) and maintenance bundles (MBs) have been developed to prevent CAUTIs but have not been extensively validated for use in pediatric populations. We report the CAUTI prevention efforts of a large network of children's hospitals. METHODS: Children's hospitals joined the Children's Hospitals' Solutions for Patient Safety engagement network from 2011 to 2017, using an open start time engagement approach, and elected to participate in CAUTI preven
