Modified McLaughlin procedure combined with proximal humeral locking plate for proximal humeral fractures with posterior shoulder dislocation and reverse Hill-Sachs lesion: a retrospective case series.

OBJECTIVE: Proximal humeral fractures with posterior shoulder dislocation are rare, high-energy injuries that pose significant treatment cha … 0°, and 65°, respectively. No cases of redislocation, implant failure, or avascular necrosis of the humeral head occurred. CONCLUSION: For patients with proximal humeral fractures and posterior shoulder dislocation involving a reverse Hill-Sachs defect occupying 25%-40% of the articular surface, treatment with the combined surgical technique of modified McLau …

Translations of the humerus in persons with shoulder impingement symptoms.

STUDY DESIGN: Two-group mixed-model analysis of covariance and correlation analysis. OBJECTIVES: To determine whether differences in humeral translations exist between patients with shoulder impingement symptoms and an asymptomatic comparison group, and if so, to determine if shoulder range-of-motion (ROM) measures are associated with abnormal translations. BACKGROUND: Abnormal translations of the humeral head are believed to reduce the available subacromial space and to contribute to the development or progression of shoulder impingement symptoms. These abnormal translations have also been …

Treatment of Proximal Humeral Fractures: A Critical Analysis Review.

There is substantial variability in the treatment of proximal humeral fractures because of classification systems with poor interobserver re … f bone substitutes, fibular strut allografts, and inferior head-supporting calcar screws to prevent humeral head displacement and screw perforation. A number of recent trials have brought into question the advantage of both ORIF and hemiarthroplasty over nonoperative treatment for patients over the age of sixty years who have three and four-part fractures with …

Mid-term outcomes of a stemless ceramic head anatomic total shoulder replacement.

BACKGROUND: In an anatomic shoulder replacement (aTSR) good results have been reported with the use of a stemless humeral prosthesis. In vitro a ceramic articulation with polyethylene has been shown to produce less polyethylene wear particles than with metal. This study aims to evaluate clinical and radiographic results of a stemless aTSR with a ceramic head articulating with a polyethylene glenoid component, with mid-term follow-up. METHODS: All patients (n = 92) in this prospective study had an aTSR utilizing a stemless humeral component with a ceramic head and a cemented double pegged ceme

Plate osteosynthesis versus hemiarthroplasty in proximal humerus fractures--does routine screening of systemic inflammatory biomarkers makes sense?

BACKGROUND: Increases in C-reactive protein (CRP) and white blood cell (WBC) counts after orthopedic surgical procedures can give evidence of postoperative infection. However, there is a lack of knowledge about the kinetics of these biomarkers in cases with an uneventful clinical course after osteosynthesis of upper limb fractures. This study investigated CRP and WBC serum levels after osteosynthesis or hemiarthroplasty of humeral head fractures. METHODS: A retrospective study on patients with humeral head fractures who had open reduction and internal fixation via plate osteosynthesis (PO) (n…

Glenohumeral motion in patients with rotator cuff tears: a comparison of asymptomatic and symptomatic shoulders.

The purpose of this study was to determine whether there was a relationship between altered scapular plane glenohumeral kinematics end shoul … spectively). The symptomatic and asymptomatic groups showed progressive superior translation of the humeral head on the glenoid with increasing arm elevation. The normal group, in contrast, maintained a constant center of rotation along the geometric center of the glenoid. Symptomatic and asymptomatic rotator cuff tear groups showed superior head migration fro …

Temporary Cement Prosthesis as a Bridge After Control of Axillary Artery Hemorrhage in Proximal Humeral Fracture-Dislocation: A Case Report.

CASE: A 64-year-old woman underwent open reduction and internal fixation for four-part proximal humeral fracture-dislocation with palpable distal pulses. Upon dislocated head removal, life-threatening hemorrhage occurred from posterior circumflex humeral artery tear. Bleeding was controlled through packing, resuscitation, and open vascular repair. Owing to unavailable shoulder prosthesis and prolonged operative time, a temporary cement prosthesis with Steinmann pins was used. At the 3-year follow-up, partial function was restored with no loosening. CONCLUSION: This case highlights axillary br

Osteonecrosis in pilot whales (Globicephala macrorhynchus).

Bone lesions, particularly osteonecrosis, have been rarely described in cetaceans, with limited studies correlating imaging, gross and microscopic findings. Aseptic osteonecrosis is especially relevant as a chronic form of decompression sickness, well-documented in human divers but controversially reported in cetaceans. This study examined the humeral heads of five stranded short-finned pilot whales (Globicephala macrorhynchus) using computed tomography (CT) and plastic embedding histologic techniques. Three specimens exhibited CT-detectable lesions, while two did not. CT imaging revealed dist…

Management of Shoulder Instability in 2020: What, When, and How.

Anterior glenohumeral instability remains a common clinical problem in the athletic and working patient populations, and further, recurrence rates following stabilization surgery continue to be problematic. Over the past 5 years, there have been substantial improvements in the understanding of anterior shoulder instability. To better counsel and treat patients, it is imperative for surgeons to have a comprehensive understanding of the epidemiology of shoulder instability, the anatomy of the glenohumeral joint particularly as it relates to glenoid and humeral head bone loss, surgical indication…

A new method for measurement of subcoracoid outlet and its relationship to rotator cuff pathology at MR arthrography.

OBJECTIVE: Orthopaedic surgical studies have shown that variations in the vertical distance between the tip of the coracoid process and the supra-glenoid tubercle alter the shape of the subcoracoid outlet. Our objective was to measure the vertical distance between the coracoid tip and the supra-glenoid tubercle (CTGT) on MR and to assess whether this showed better correlation with rotator cuff pathology compared with the axial coraco-humeral distance. MATERIALS AND METHODS: A retrospective review was performed of 100 consecutive shoulder MR arthrograms. Vertical distance between the coracoid

[Cortisone-induced humerus head necrosis in acute myeloid leukemia: cartilage-preserving arthroscopic spongioplasty].

Osteonecrosis is a long known side effect in patients receiving cortisone or chemotherapy. A young patient was diagnosed with acute myeloid leukemia (AML) in 2005. After receiving combined cortisone and chemotherapy the patient was in complete remission. In 2007 a total hip replacement was necessary due to femoral head necrosis. Years after the joint replacement the patient reported acute shoulder pain without trauma. In this article an alternative procedure to an endoprothesis in a young patient with a humeral head osteonecrosis is presented.

Outcomes of humeral osteotomies versus soft-tissue procedures in secondary surgical procedures for neonatal brachial plexus palsy: a meta-analysis.

Secondary surgical procedures can be used in brachial plexus birth injury to correct shoulder movement imbalances. This study compares outcomes of the two secondary surgical procedure types: humeral osteotomies and soft tissue procedures. Outcome measures assessed included active and passive internal and external rotation, active and passive abduction and adduction, active and passive flexion and extension, percentage of the humeral head anterior to the middle glenoid fossa, glenoid version, and Mallet Score. Nineteen full-text articles were included in the analysis. Humeral osteotomies result…
