[The temperature hysteresis of water-salt metabolism in the frog Rana temporaria].

Frogs acclimated to 4 degrees C were transported to a medium with temperature 20 degrees C, which caused polyuria; recovery of normal diuresis took about 24 h. During this period, hypernatremia was observed together with the increase in natriuresis, the rate of renal excretion of potassium ions with urea remaining constant. Water content of skeletal muscles decreased. Transportation of frogs acclimated to 20 degrees C into a medium with a temperature 4 degrees C decreased their diuresis. Renal excretion of sodium, calcium and magnesium ions remained unchanged, whereas that of potassium ions si…

Toxic effects of electrolyte and trace mineral administration in the intensive care unit.

Electrolytes and trace minerals are administered routinely to ICU patients to correct deficiencies or as specific therapy for various condit … effects observed with phosphate administration include hypocalcemia, metastatic calcification, and hypernatremia or hyperkalemia. Most of these toxicities are avoidable if appropriate precautions are taken and appropriate monitoring implemented. Finally, when administering any of these agents, the intensivist should be familiar with their toxicologic profiles …

[Changes in electrolyte and acid-base equilibrium in children with acute urinary tract infections].

Between 1980 and 1986 313 patients with urinary tract infection have been investigated in a retrospective study for changes in their electro … was seen: hyponatremia singly or in various combination was observed most frequently (81%) but even hypernatremia was found. In some patients only an acid base disturbance without changes in electrolytes was seen. The pattern of hyponatremia with hyperkalemia--reported in the literature with obstructive uropathy--was observed in 6.8% of our patient population w …

[Rehydration by the oral route and its maintenance in patients from birth to 3 months old dehydrated due to diarrhea].

Studies of oral rehydration have been carried out in patients three months of age or older. With the exception of a previous report by us, n … rs. Several patients presented severe water and electrolyte unbalance, namely metabolic acidosis or hypernatremia. This returned near normality as the patients became rehydrated. We conclude that patients under 3 months of age respond as well as older ones to oral rehydration.

Renal tubular acidosis as the initial presentation of Sjögren syndrome complicated by iatrogenic central pontine myelinolysis: a case report.

Primary Sjögren syndrome is a chronic autoimmune disorder that primarily affects the exocrine glands and may present with extraglandular manifestations, including renal tubular acidosis (RTA). We report the case of a 28-year-old postpartum female who presented with rapidly progressive limb weakness and severe hypokalemia; she was ultimately diagnosed with RTA as the initial manifestation of primary Sjögren syndrome. During treatment with intravenous sodium bicarbonate, the patient developed acute hypernatremia, which was subsequently complicated by central pontine myelinolysis (CPM). This iatr…

Presence of Probst Bundles Indicate White Matter Remodeling in a Dog With Corpus Callosum Hypoplasia and Dysplasia.

Corpus callosum abnormalities (CCA) rarely occur in dogs and are related to hypo/adypsic hypernatremia and seizures. Hypoplasia and dysplasia of the corpus callosum (CC) with concomitant lobar holoprosencephaly is the most common variant. It is currently uncertain using conventional MRI if canine CCA reflects the failure of commissural fibers to develop or the failure of the commissural fibers to cross hemispheres. Diffusion tensor imaging was performed in a 4-year-old Staffordshire mix breed dog with CCA and an age-matched healthy Beagle. In comparison to the control dog, CC tractography of t…

Diabetes Insipidus: An Unusual Presentation of Adenocarcinoma of the Lung in a Patient with no Identifiable Lung Mass.

CONTEXT: Lung cancers are known to metastasize to unusual sites. Despite this knowledge often times the diagnosis of a primary lung cancer gets delayed especially when the patient presents without respiratory symptoms. CASE REPORT: The patient discussed in our review is a 47-year-old female, smoker who had presented to several hospitals with months of headache, nausea and intermittent episodes of vomiting. She was noted to have hypernatremia due to diabetes insipidus and a pituitary lesion on her magnetic resonance images. The pituitary mass on biopsy was found to represent a metastatic focus…

Nephrogenic diabetes insipidus with idiopathic Fanconi's syndrome in a child who presented as vitamin D resistant rickets.

Fanconi's syndrome is a complex of multiple tubular dysfunctions of proximal tubular cells occurring alone or in association with a variety of inherited (primary) or acquired (secondary) disorders. It is characterized by aminoaciduria, normoglycaemic glycosuria, tubular proteinuria without hematuria, metabolic acidosis without anion gap and excessive urinary excretion of phosphorous, calcium, uric acid, bicarbonate, sodium, potassium, and magnesium. Whereas diabetes insipidus is a disease of collecting tubules and child mainly presents with dehydration and hypernatremia. Though all the cases p…

[Disorders of water and sodium balance in intensive care patients].

Intensive care patients often suffer from hypo- or hypernatremia. These dysnatremias reflect an antidiuretic-hormone (ADH)-related water imbalance and are the result of the underlying disease. However, they are often triggered by drug side effects and exacerbated by an intentional or unintentional sodium imbalance. Dysnatremias are also caused by artificial ventilation; however, the mechanisms behind this are beyond the scope of this article. Considerations regarding etiology, water and sodium balance and, in particular, the variable in urine dilution or concentration, take priority over a bri…

Forensic application of ESEM and XRF-EDS techniques to a fatal case of sodium phosphate enema intoxication.

Sodium phosphate enemas and laxatives are widely used for the treatment of constipation, even if a number of cases of significant toxicity due to alterations of the fluid and electrolyte equilibria (hypernatremia, hyperphosphatemia, and hypocalcemia) have been reported. We present the case of an 83-year-old man who died of fecal and chemical peritonitis secondary to an iatrogenic colon perforation (produced performing a Fleet enema through the patient's iliac colostomy) with peritoneal absorption of sodium phosphate. Environmental scanning electron microscopy coupled with an X-ray fluorescence…

Interpretation of the urine osmolality: the role of ethanol and the rate of excretion of osmoles.

One purpose of this report is to illustrate that calculating the rate of excretion of osmoles in the urine can be of value in the differential diagnosis of hypernatremia and polyuria. A second purpose is to illustrate a clinical example where the osmolality of the urine did not reflect the lack of action of ADH. A patient with ethanol intoxication seemed to have central diabetes insipidus on clinical grounds. However, the osmolality of the urine was 287 mosm/kg H2O, a value which made this diagnosis unlikely. Since the concentration of ethanol in plasma was 119 mmol/L, we suspected that the ur…
