Vitamin D, secondary hyperparathyroidism, and preeclampsia.

BACKGROUND: Secondary hyperparathyroidism, which is defined by a high concentration of intact parathyroid hormone when circulating 25-hydroxyvitamin D [25(OH)D] is low, is a functional indicator of vitamin D insufficiency and a sign of impaired calcium metabolism. Two large randomized controlled trials examined effects of calcium supplementation on preeclampsia but did not consider the vitamin D status of mothers. OBJECTIVE: We examined the association of secondary hyperparathyroidism with risk of preeclampsia. DESIGN: Circulating maternal 25-hydroxyvitamin D [25(OH)D] and intact parathyroid…

Vitamin D and primary hyperparathyroidism (PHPT).

Vitamin D deficiency and primary hyperparathyroidism (PHPT) are two common conditions, especially in postmenopausal women. Vitamin D deficiency is said to be even more frequent in PHPT patients than in the general population due to an accelerated conversion of 25-hydroxy vitamin D (25OHD) into calcitriol or 24-hydroxylated compounds. Although several studies have reported worsening of PHPT phenotype (larger tumours, higher parathyroid hormone [PTH] levels, more severe bone disease) when vitamin D deficiency coexists whereas vitamin D supplementation in PHPT patients with a serum calcium level …

Surgical management of normocalcemic primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (pHPT), typically defined as elevated serum calcium levels associated with inappropriately elevated parathyroid hormone (PTH) levels, can occur also in patients with normal serum calcium levels. This study investigated the characteristics, workup, and surgical management of patients with normocalcemic pHPT. METHODS: A retrospective chart review of a prospectively collected, single-institution parathyroid database was performed on patients with sporadic pHPT who underwent parathyroidectomy between 12/99 and 12/08. RESULTS: In all, 93 of 771 (12%) pHPT p…

[Normocalcemic primary hyperparathyroidism: a growing problem].

Normocalcemic primary hyperparathyroidism is at present one of the most common reasons for consultation in bone metabolism units. It is characterized by increased levels of intact parathyroid hormone in the presence of normal serum calcium (total and ionized) in generally asymptomatic individuals. The differential diagnosis should be considered in all situations that occur with secondary hyperparathyroidism. Its natural history is not well known, and it does not always progress to hypercalcemia. As a recently recognized entity, there are still no specific recommendations for its management. In…

Acute pancreatitis associated with primary hyperparathyroidism.

Coexistence of primary hyperparathyroidism and acute pancreatitis has widely been reported in literature, but a causal relationship remains controversial. A case of acute pancreatitis as a first symptom of primary hyperparathyroidism with severe hypercalcemia is reported. In this patient a reduction of serum calcium level was obtained with medical therapy and resulted in the resolution of acute pancreatitis symptoms within 10 days. At the same time a parathyroid adenoma was clinically identified and elective parathyroidectomy was performed with complete normalization of intact parathyroid horm…

Recurrent renal hyperparathyroidism due to parathyromatosis.

Parathyromatosis is the most severe type of recurrent secondary hyperparathyroidism (SHPT) after parathyroidectomy (PTX) in haemodialysis patients. It is difficult to completely remove all foci of parathyroid tissue and neck re-explorations are often required. Here, we report for the first time a case of recurrent SHPT due to parathyromatosis treated by radio-guided PTX. A haemodialysed 48-year-old woman with recurrent SHPT due to parathyromatosis was treated by radio-guided PTX. Preoperatively Ultrasonography, (99)Tc-SestaMIBI scintigraphy and magnetic resonances of the neck and thorax were p…

Vitamin D treatment in primary hyperparathyroidism.

BACKGROUND: Primary hyperparathyroidism (PHPT) and vitamin D insufficiency are two very frequent conditions. In cases where the combination of both vitamin D insufficiency and PHPT is diagnosed, vitamin D repletion is an option. However, only limited evidence exists for this treatment. OBJECTIVE: The aim of this review is to describe different aspects of concomitant vitamin D insufficiency and PHPT and in this setting to evaluate existing evidence on safety and possible outcome of vitamin D treatment. METHODS: Background literature was found based on a search in pubmed.com and scirus.com. R…

[Hyperparathyroidism: molecular, diagnostic and therapeutic aspects].

The sensitivity of parathyroid glands to a low calcium level in plasma results in parathyroid hormone (PTH) release in order to restore the normal Ca²+ concentration. Hyperparathyroidism is a common endocrinopathy, caused by uncontrolled growth of parathyroid cells. In primary hyperparathyroidism, hypercalcemia develops due to extensive autonomous secretion of PTH. Secondary hyperparathyroidism is a well-established complication of chronic renal insufficiency, where marked parathyroid hyperplasia occurs, especially in patients with long dialysis vintage. The elevated PTH level in the circulati…

Vitamin D deficiency is said to be even more fr … stent vitamin D deficiency induces, in the long-term, an autonomous secretion of PTH (i.e. tertiary hyperparathyroidism). The mechanism by which this could occur is unclear however. Finally, as many, otherwise normal, subjects with vitamin D insufficiency may have an increased serum PTH level we believe that those with vitamin D insufficiency should be excluded from …

Managing primary hyperparathyroidism in primary care.

Patients with calcium concentrations above 3 mmol/L typically start to develop symptoms of hypercalcaemia, which can include nausea, vomiting, thirst and polyuria, malaise, confusion, lowered pain threshold and coma. Milder hypercalcaemia (calcium concentrations <3 mmol/L) is often asymptomatic, and the problem is therefore usually discovered as an incidental finding on routine biochemical screening. Primary hyperparathyroidism is a common cause of hypercalcaemia. Here, we consider the recognition and further management of patients presenting with asymptomatic primary hyperparathyroidism in pr…

[Minimally invasive treatment of primary hyperparathyroidism].

BACKGROUND: Parathyroid glands have traditionally been identified by bilateral neck exploration under general anaesthesia in connection with surgical removal of enlarged glands in primary hyperparathyroidism. Unilateral neck exploration can now be done under local anaesthesia without dissecting all glands, due to improved preoperative scintigraphy and measurement of parathyroid hormone (PTH) intra-operatively. This minimally invasive technique was introduced at the University Hospital of Northern Norway in 2004. MATERIAL AND METHODS: Data on patients who had undergone minimally invasive surge…

Asymptomatic hyperparathyroidism--need for multicentre studies.

Long-term follow-up is initially considered appropriate for the majority of patients with primary hyperparathyroidism (PHPT) having small increases in calcium levels (< 2.8 mmol/l) and lacking the 'classical' symptoms of PHPT. The supportive reasoning is that many such patients never progress to more severe biochemical or clinical disease. There are, however, arguments in favour of early surgical treatment of such patients but adequately powered studies have not been carried out in this subgroup of patients to asses the impact of PHPT on their quality of life, cardiovascular risk and bone dens…

[Secondary hyperparathyroidism: incidence, clinical presentations, treatment].

Prevalence of secondary hyperparathyroidism (SHPT) was studied in 235 patients treated by program hemodialysis. SHPT was diagnosed according to recommendations of K/DOQI Clinical Practice Guidelines with the parathyroid hormone (PTH) level higher than 300 pg/ml. In the beginning of replacement renal therapy with hemodialysis the secondary hyperparathyroidism of different degree was found in 145 patients (61.7%): mild form (PTH from 300 to 450 pg/ml) in 37.8%, medium (PTH from 450 to 1000 pg/ml) in 40.5%, severe (PTH more than 1000 pg/ml) in 21.7%. Normal indices of PTH were determined in 70 pa…
