Overlay grafting for lateral nasal wall concavities.

One of the most difficult maneuvers in rhinoplasty is achieving a straight dorsum in the patient with a crooked nose. Often this deformity is a result of trauma involving the bony and cartilaginous vaults, resulting in nasal bone fractures and avulsion of the lateral cartilages into the nasal airway. Reduction of nasal bone fractures is fairly straightforward, but the repair of nasal cartilage trauma is more problematic. The cartilage of the nasal vault possesses fibrous attachments, which when disturbed, are difficult to reestablish. Overlay grafts are known to be effective in filling these d

The "cap" technique: nonmicrosurgical reattachment of fingertip amputations.

With the exception of children, amputations at the level of the lunula survive poorly by direct reattachment. Microsurgical replantation is costly and often fails because of poor venous drainage. In a series of seven adult patients the severed tip was filleted and replaced as a "cap" over the skeletonized distal phalanx of the stump. A 2 mm remnant of germinal matrix was preserved for nail regrowth. The reconstructed digits, although shortened by an average of 6 mm, give the "illusion" of a normal finger. All were successful with small areas of tip necrosis in two, healing by secondary reepith

The borderline patient. A comparative analysis of four sets of diagnostic criteria.

In reviewing the evidence for the validity of the diagnosis borderline, four descriptions in the literature seem to offer comprehensive criteria for the diagnosis. When the four are compared, a total of 104 criteria are enumerated encompassing the mental status, history, interpersonal relationships, defense mechansisms, and other judgments of personality functioning of the borderline patient. Half of these criteria are mentioned in only one of the four diagnostic descriptions. This apparent lack of agreement over diagnostic criteria has three possible interpretations: (1) the borderline concep

Case report of the evidence of a spontaneous Reverse Pulfrich effect in monovision after cataract surgery.

BACKGROUND: Cataracts affect the optics of the eye in terms of absorption, blur, and scattering. When cataracts are unilateral, they cause differences between the eyes that can produce visual discomfort and harm binocular vision. These interocular differences can also induce differences in the processing speed of the eyes that may cause a spontaneous Pulfrich effect, a visual illusion provoking important depth misperceptions. Interocular differences in light level, like those present in unilateral cataracts, can cause the Classic Pulfrich effect, and interocular differences in blur, like those

The Motor Network Reduces Multisensory Illusory Perception.

Observing mouth movements has strikingly effects on the perception of speech. Any mismatch between sound and mouth movements will result in listeners perceiving illusory consonants (McGurk effect), whereas matching mouth movements assist with the correct recognition of speech sounds. Recent neuroimaging studies have yielded evidence that the motor areas are involved in speech processing, yet their contributions to multisensory illusion remain unclear. Using functional magnetic resonance imaging (fMRI) and transcranial magnetic stimulation (TMS) in an event-related design, we aimed to identify

Ability to Process Multisensory Information Is Impaired in Open Angle Glaucoma.

PRÉCIS: Patients with glaucoma demonstrated deficiencies in their ability to process multisensory information when compared with controls, with those deficiencies being related to glaucoma severity. Impaired multisensory integration (MSI) may affect the quality of life in individuals with glaucoma and may contribute to the increased prevalence of falls and driving safety concerns. Therapeutic possibilities to influence cognition in glaucoma should be explored. PURPOSE: Glaucoma is a neurodegenerative disease of the optic nerve that has also been linked to cognitive health decline. This study

Evidence of a Right Ear Advantage in the absence of auditory targets.

The Right Ear Advantage effect (REA) was explored in a white noise speech illusion paradigm: binaural white noise (WN) could be presented i) in isolation (WN condition), ii) overlapped to a voice pronouncing the vowel /a/ presented in the left ear (LE condition), iii) overlapped to a voice pronouncing the vowel /a/ presented in the right ear (RE condition). Participants were asked to report in which ear the voice has been perceived. The voice could be female or male, and it could be presented at 4 different intensities. Participants carried out the task correctly both in LE and in RE condition

[Brodmann Areas 17, 18, and 19 in the Human Brain: An Overview].

Brodmann areas 17, 18, and 19 in the human brain are visual cortices of the occipital lobe. Each area has its own retinotopic representations, particulary area 19, which has many small retinotopic areas representing half or all of the contralateral visual field, several functional areas, and nine cytoarchitectonic areas. Several fasciculi are known as occipital fiber connections, but their precise endpoints are not clear. Lesions in the visual cortices cause several visual disorders including visual field defect, visual hallucinations, metamorphopsia, and different kinds of visual agnosia.

The influence of visual target information on the online control of movements.

The continuously changing properties of our environment require constant monitoring of our actions and updating of our motor commands based on the task goals. Such updating relies upon our predictions about the sensory consequences of our movement commands, as well as sensory feedback received during movement execution. Here we focus on how visual information about target location is used to update and guide ongoing actions so that the task goal is successfully achieved. We review several studies that have manipulated vision of the target in a variety of ways, ranging from complete removal of

Epileptic feeling of multiple presences in the frontal space.

We describe the case of a patient who during a simple focal epileptic seizure due to vascular cerebral sequelae, reported the paroxysmal convincing feeling of the presence of several familiar persons in her peripersonal and extrapersonal space. Because the patient reported sensing multiple presences and recognizing them as family members, her case appears to contradict the hypothesis that the feeling of a non-existent human presence is an autoscopic phenomenon involving the neural "reduplication of the body". As an alternative hypothesis, I propose that the feeling of a non-existent presence m

Seeing the phantom: a functional magnetic resonance imaging study of a supernumerary phantom limb.

OBJECTIVE: Supernumerary phantom limb (SPL) is a rare neurological manifestation where patients with a severe stroke-induced sensorimotor deficit experience the illusory presence of an extra limb that duplicates a real one. The illusion is most often experienced as a somesthetic phantom, but rarer SPLs may be intentionally triggered or seen. Here, we report the case of a left visual, tactile, and intentional SPL caused by right subcortical damage in a nondeluded woman. METHODS: Using functional magnetic resonance imaging, we investigated the multimodal nature of this phantom, which the patien

Use of the analyst as a fetish.

In some cases a gratifying transference fantasy is the subject of progressive analytic work, while in other cases the same type of fantasy eludes investigation, and its enactment causes treatment to become an unproductive endless task. One cause for the latter difficulty can be that the patient uses his or her analyst as a fetish, permitting the distinction between reality and fantasy to remain inconclusive, so that relinquishment of magical expectations does not take place. The particular form of thinking involved in use of the analyst as a fetish is described. The role of illusion, its vario

Induced self-motion in central vision.

Previous research on visually induced self-motion found that stimulation of the central visual field (up to 30 degrees in diameter) results in perceived object motion while self-motion requires peripheral stimulation. In the present study, perceived self-motion was induced with a radially expanding pattern simulating observer motion through a space filled with dots, with visual angles of 7.5 degrees, 10.6 degrees, 15 degrees, and 21.2 degrees. Speed and texture density were also varied. The duration of reported self-motion (a) decreased with increased speed, (b) failed to increase with increas

When in Doubt, Touch Is More Convincing Than Vision.

We sometimes feel compelled to touch objects after having looked at them. Is this because touch gives us more accurate information, or is it simply because we tend to trust what we feel more than what we see? To disentangle these possibilities, participants were subjected to a variant of the 'Vertical-Horizontal' illusion where the length of a vertical bar is consistently overestimated relative to the length of a horizontal bar. Our variant focuses on points of subjective equality where the bars almost look the same, and possesses two key characteristics: it creates ambiguity as stimuli can be

Acute right opercular stroke-associated polyopic heautoscopy and hallucinations caused by disconnection to the inferior parietal lobule through the superior longitudinal fasciculus III: A single case study.

Illusory neuropsychiatric symptoms such as hallucinations or the feeling of a presence (FOP) can occur in diffuse brain lesion or dysfunction, in psychiatric diseases as well as in healthy individuals. Their occurrence due to focal brain lesions is rare, most probably due to underreporting, which limits progress in understanding their underlying mechanisms and anatomical determinants. In this single case study, an 86-year-old patient experienced, in the context of an acute right central opercular ischemic stroke, visual hallucinatory symptoms (including palinopsia), differently lateralized aud

Face pareidolia is enhanced by 40 Hz transcranial alternating current stimulation (tACS) of the face perception network.

Pareidolia refers to the perception of ambiguous sensory patterns as carrying a specific meaning. In its most common form, pareidolia involv … es in the Noise pareidolia test. The current study indicates that 40 Hz tACS can enhance pareidolic illusions in healthy individuals and, thus, that high frequency (i.e., gamma band) oscillations are critical in forming coherent and meaningful visual perception.

Neural mechanisms of perceptual and affective body-image disturbance during own-body and ideal-body estimation.

Body-image disturbance is a core feature of eating disorders and can predict their development in healthy individuals. There are two components of body-image disturbance: perceptual disturbance (associated with overestimation of body size) and affective disturbance (associated with body dissatisfaction). Previous behavioral studies have hypothesized that attention to particular body parts and negative body-related emotions resulting from social pressure are associated with the respective degrees of perceptual and affective disturbance; however, the neural representations that underlie this hyp

Increased insula activity precedes the formation of subjective illusory Gestalt.

The constructive nature of human perception sometimes leads us to perceiving rather complex impressions from simple sensory input: for example, recognizing animal contours in cloud formations or seeing living creatures in shadows of objects. A special type of bistable stimuli gives us a rare opportunity to study the neural mechanisms behind this process. Such stimuli can be visually interpreted either as simple or as more complex illusory content on the basis of the same sensory input. Previous studies demonstrated increased activity in the superior parietal cortex during the perception of an

Reach planning with someone else's hand.

To investigate the relationship between the sense of body ownership and motor control, we capitalized on a rare bizarre disorder wherein another person's hand is misattributed to their own body, i.e., a pathological form of embodiment (E+). Importantly, despite E+ is usually associated with motor deficits, we had the opportunity to test two E+ patients with spared motor function, thus able to perform a reaching task. Crucially, these patients had proprioceptive deafferentation, allowing us to purely isolate the embodiment-dependent effect from proprioception-dependent ones that are usually ass

Alice in Wonderland Syndrome: A real life version of Lewis Carroll's novel.

Alice in Wonderland Syndrome was originally coined by Dr. John Todd in 1955. The syndrome is named after the sensations experienced by the character Alice in Lewis Carroll's novel Alice's Adventures in Wonderland. Alice in Wonderland Syndrome consists of metamorphopsia (seeing something in a distorted fashion), bizarre distortions of their body image, and bizarre perceptual distortions of form, size, movement or color. Additionally, patients with Alice in Wonderland Syndrome can experience auditory hallucinations and changes in their perception of time. Currently, there is no known specific ca
