[Hospital hygiene. Norovirus infections].

Outbreaks of gastroenteritis caused by noroviruses have become an increasing problem for institutions in the health-care system over the past years. Staff members are also afflicted by the outbreaks of infection due to the highly contagious nature of noroviruses and this can lead to bottlenecks in health-care management and to economic losses. An acute gastroenteritis due to norovirus usually begins with severe nausea, heavy often projectile vomiting and a pronounced feeling of unwellness. In addition, there can be diarrhoea and abdominal cramps. The incubation time amounts to around one day.

Anaerobic infections in children.

Anaerobic bacteria commonly cause infection in children. Anaerobes are the most predominant components of the normal human skin and mucous membranes bacterial flora and are therefore a common cause of bacterial infections of endogenous origin. Because of their fastidious nature, they are difficult to isolate from infectious sites and are often overlooked. Anaerobic infections can occur in all body sites, including the central nervous system, oral cavity, head and neck, chest, abdomen, pelvis, skin, and soft tissues. They colonize the newborn after delivery and have been recovered from several …

[Hot topics respiratory infections].

A critical review of articles on respiratory infections published in the last three months of 2008 to the end of 2009. There are articles on patients with bronchiectasis that study their quality of life, as well as those that analyse the decrease in the number of exacerbations due to the immunomodulatory effect of using low doses of macrolides in these patients. In the diagnosis of tuberculosis, works on the application of interferon-A are especially relevant, whilst on the treatment of this disease, articles that study therapeutic innovations in the management of TB-MDR and TB-XDR or -TDR are…

Coping with Candida infections.

Although Candida spp. exist as normal flora on much of the human body, life-threatening invasive infections by these organisms have increased in recent decades. Candida spp. are now one of the most common causes of nosocomial blood stream infections worldwide, and patients with critical illness or malignant disease demonstrate particular susceptibility. Risk factors are identifiable for virtually all patients who develop invasive candidiasis, and clinicians must maintain vigilance for Candida infections in the appropriate clinical context, given the attributable mortality. This review addresse…

[Skin infections after transplantation].

Skin infections after transplantation are frequent and of special importance because they may be quite severe. The spectrum of dermatologic infections in transplant recipients includes bacterial, mycotic and viral diseases. Pyoderma, herpes virus 6/7, herpes simplex virus, varicella-zoster virus, cytomegalovirus and candida infections predominate. Rare pathogens must be also considered. Cutaneous infections can be divided into three phases following transplantation. Diagnosis and adequate early therapy together with specific prophylaxis and follow-up of transplant patients should be strived fo…

Rare infections mimicking MS.

The diagnosis of multiple sclerosis (MS), despite well defined clinical criteria is not always simple. On many occasions it is difficult to differentiate MS from various non-MS idiopathic demyelinating disorders, specific and infectious inflammatory diseases or non-inflammatory demyelinating diseases. Clinicians should be aware of various clinical and MRI "red flags" that may point to the other diagnosis and demand further diagnostic evaluation. It is generally accepted that atypical clinical symptoms or atypical neuroimaging signs determine necessity for broad differential diagnostic work up.

Enterovirus infections in neonates.

Enteroviruses, which include echoviruses, coxsackie A and B viruses, polioviruses and the 'numbered' enteroviruses, are among the most common viruses causing disease in humans. A large proportion of enteroviral infections occur in neonates and infants. There is a wide spectrum of clinical manifestations that can be caused by enterovirus infection with varying degrees of severity. In the neonatal age group, enteroviral infections are associated with significant morbidity and mortality, particularly when infection occurs antenatally. This review provides a detailed overview of the epidemiology a…

Severe soft tissue infections.

Severe skin and soft tissue infections (SSTIs) frequently require management in the ICU, in part related to associated septic shock or toxic shock syndrome or associated organ failure. Four fundamental management principles are key to a successful outcome in caring for patients who have severe SSTIs, including (1) early diagnosis and differentiation of necrotizing versus nonnecrotizing SSTI, (2) early initiation of appropriate empiric broad-spectrum antimicrobial therapy with consideration of risk factors for specific pathogens and mandatory coverage for methicillin-resistant Staphylococcus au…

[Nosocomial infections--ecological niches].

Knowledge of ecological niches of nosocomial pathogens is used in treatment and prevention of nosocomial infections. This article offers an analysis of the ecological niches of three important nosocomial pathogens: Clostridium difficile, Staphylococcus aureus and Legionella pneumophila. Relevant terms are explained and examples of deliberate and non-deliberate manipulation of the niches and the following consequences are given. Finally, the consequences of climate changes on the pattern of nosocomial infections are discussed.

[Urinary catheter biofilm infections].

Urinary tract infections, most of which are biofilm infections in catheterized patients, account for more than 40% of hospital infections. Bacterial colonization of the urinary tract and catheters causes not only infection but also other complications such as catheter blockage by bacterial encrustation, urolithiasis and pyelonephritis. About 50% of long-term catheterized patients face urinary flow obstruction due to catheter encrustation, but no measure is currently available to prevent it. Encrustation has been known either to result from metabolic dysfunction or to be of microbial origin, wi…

Oritavancin for skin infections.

Oritavancin (LY-333328) is a semisynthetic lipoglycopeptide for the treatment of serious infections with Gram-positive pathogens, especially methicillin-resistant Staphylococcus aureus and complicated skin and soft tissue infections. A New Drug Application for oritavancin administered intravenously once daily for 3 to 7 days for complicated skin and soft tissue infections was filed with the U.S. Food and Drug Administration in April 2008. Antibiotics and perioperative infections.

Surgical site infections remain a significant contributor to postoperative morbidity and mortality. It is estimated that 500,000 patients suffer from this complication annually. Among other interventions, appropriate administration of prophylactic antibiotics has been shown to decrease the risk of perioperative infections. The goal of prophylactic antibiotic administration is to decrease the risk of contamination of the wound from skin flora in the case of clean procedures, and to add coverage of organisms that are anticipated to contaminate the surgical field, as in open bowel procedures. The…

[Infections in splenectomized patient].

Overwhelming post-splenectomy infection (OPSI) is a long-term risk in asplenic patients, which may be controlled by appropriate preventive measures. Specific guidelines have been developed to reduce its incidence. These guidelines include immunizations, antibioprophylaxis, and education. Immunizations against S. pneumoniae, N. meningitidis, Hamophilus influenzae, and influenza should be administered. Antibioprophylaxis during 2 to 5 years following splenectomy in children, and 2 years in adults is recommended. Furthermore, long-term education is mandatory. Application of preventing measures is

[Biologic therapy and infections].

Biologic therapy is an important therapeutic arsenal in rheumatic diseases. Anti-TNF therapies affect host defenses against infections, since TNF mediates inflammation and modulates cellular immune responses. Cases of tuberculosis have been observed in patients treated with TNF antagonists, mainly due to the presence of latent or "dormant" tuberculosis infection (LTB1). Other microorganisms responsible for the infectious complications associated with biologic therapy are generally intracellular pathogens or pathogens that commonly exist in a chronic latent state: Mycobacterium sp., Listeria mo…
