Ear deformity in children following high ear-piercing: current practice, consent issues and legislation.

In this presentation we examine the practice of high ear-piercing in children, the issue of informed consent and current legislation. We sampled current practice and consent policy by visiting nine establishments in Sheffield providing this service. There were two high street department stores, two fashion accessory outlets and five body-piercing studios. Enquiries were made as to the technique used, knowledge of complications, customer counselling and consent policy. A photograph of an ear with a cosmetic deformity following high ear-piercing was shown and awareness of this possible outcome w…

In re A (children): in the Royal Courts of Justice (England).

HELD: The proposed operation to separate conjoined twins resulting in the death of one of the twins is lawful. Although Mary's death is foreseeable as an inevitable consequence of the operation, the invasive surgery is intended and necessary to save Jodie's life. Mary's death is not a purpose or intention of the surgery, and she will die only because her body, on its own, was never viable.

[Autopsy of the fetus. Proposed investigatory strategy as a decision aid in the autopsy of fetues with special conditions].

Prenatal examination of fetuses can now yield excellent findings and, by means of Doppler technology, can also reveal functional changes. Despite progress in fetal diagnostics, quality control of prenatal diagnoses remains necessary since prenatal findings can be the basis for deciding whether to perform preterm abortion. This is possible only by autopsy, and therefore increasing demands for precision are being placed on autoptic procedures. This is particularly the case in malformations, which can be associated with a specific syndrome and then become decisive in the context of family counsel

[Alternative for an equitable law on the protection of the person or confinement and mental illness: 2 impossible concepts].

Since the French Revolution legislation has been mainly concerned with the restraint of innocent people by the so-called "letters de cachet" … ment and in particular on the protection of the patient in general. People, who can not give their "informed consent" since they endure extreme physical or mental weakness, should be represented by an authorized person appointed by the justice of the peace, to discuss the treatment in his place. Mentally ill people who are able to hold a discussion but who are dis …

Vocal ligametry. Update.

In 1983, a report was made concerning experience with a technique of vocal ligament tightening in certain cases of dysphonia due to the ligament's stretching or lengthening. Since that time, efforts to measure the relative slackness or loss of tension of the vocal ligament have been partially successful. This report addresses the topics of consent forms; review of the local board of human procedures; and technical problems of instrument design, fabrication, and calibration. A large data base will be needed before true usefulness can be derived from these efforts. The author hopes to stimulate

Is capacity to consent to healthcare a uniformly defined concept? Insights from a document analysis of laws and national guidelines across six European countries.

BACKGROUND: Capacity to consent or decision-making capacity to healthcare is a key prerequisite for a valid informed consent. A clear understanding of this concept is important to protect patients' autonomy. Without it, clinical assessments may vary, jeopardising patients' participation in decisions about their care and increasing the risk of delayed, denied or harmful treatment. Despite substantial international debate and reforms concerning norms on capacity assessment, there remains a paucity of scholarly literature comparing national approaches, which could enable countries to learn from o…

Persona non grata: Identifying patients unsuitable for aesthetic dermatologic procedures.

The rapid growth of aesthetic dermatology has been accompanied by an increasing demand for cosmetic procedures and heightened expectations f … hlight specific patient profiles that require caution. We examine ethical considerations, including informed consent, patient autonomy, and the physician's duty to avoid harm. Additionally, we focus on psychologic aspects, particularly unrealistic expectations, social media-driven aesthetic demands, and psychiatric comorbidities like body dysmorphic disorder. Reco …

"There's more to recovery than just weight gain": Ethical challenges for justice and respect in adjudication of capacity for patients with anorexia nervosa.

Anorexia nervosa (AN) is a complex psychiatric illness with a high mortality rate. When patients with AN refuse medical treatment, it may be ethically justifiable to provide treatment in their best interests if they are found incapable to make the decision themselves. In Ontario, Canada, any person deemed incapable has the right to challenge that decision at the Consent and Capacity Board (CCB), an independent tribunal that adjudicates hearings under six Acts, including the Health Care Consent Act. Recognising both the complexity of capacity assessment in the context of AN and widespread stigm

Reproductive cancer cryopreservation services: Patient information, clinical advice, and support.

Fertility preservation services must offer information to patients, prior to their visit, so that they have time to read and digest the information, and also have the opportunity to write down any questions they wish to ask at the oncofertility consultation appointment. Appointments must be offered immediately, based on a specifically designed referral form. Each fertility service providing oncology cryopreservation should have a robust map of the patient's journey to include referral, counseling session, medical consultation, informed consent, treatment plan, and follow-up. Consent for fertil…

Ethical Issues in Normothermic Regional Perfusion in Controlled Organ Donation After Determination of Death by Circulatory Criteria: A Scoping Review.

Normothermic regional perfusion (NRP) is a surgical technique that can improve the quality and number of organs recovered for donation after the determination of death by circulatory criteria. Despite its promise, adoption of NRP has been hindered because of unresolved ethical issues. To inform stakeholders, this scoping review provides an impartial overview of the major ethical controversies surrounding NRP. We undertook this review according to a modified 5-step methodology proposed by Arksey and O'Malley. Publications were retrieved through MEDLINE and Embase. Gray literature was sourced fr
