Inhaled Gases for Neuroprotection of Neonates: A Review.

Importance: Hypoxic-ischemic encephalopathy (HIE) is a significant cause of morbidity and mortality in neonates. The incidence of HIE is 1-8 per 1,000 live births in developed countries. Whole-body hypothermia reduces the risk of disability or death, but 7 infants needed to be treated to prevent death or major neurodevelopmental disability. Inhalational gases may be promising synergistic agents due to their rapid onset and easy titratability. Objective: To review current data on different inhaled gases with neuroprotective properties that may serve as adjunct therapies to hypothermia. Evidence…

Inhaled anesthesia in neurosurgery: Still a role?

In patients undergoing craniotomy, general anesthesia should be addressed to warrant good hypnosis, immobility, and analgesia, to ensure systemic and cerebral physiological status and provide the best possible surgical field. Regarding craniotomies, it is unclear if there are substantial differences in providing general anesthesia using total intravenous anesthesia (TIVA) or balanced anesthesia (BA) accomplished using the third generation halogenates. New evidence highlighted that the last generation of halogenated agents has possible advantages compared with intravenous drugs: rapid induction

Association Between Inhaled Corticosteroids and Tracheobronchomalacia.

OBJECTIVE: The aim of this study was to assess any association between use of inhaled corticosteroids (ICS) and tracheobronchomalacia (TBM). METHODS: This study was a retrospective analysis of patients with asthma and COPD, with and without TBM. Patients were diagnosed with TBM on the basis of CT imaging, flexible bronchoscopy, or both. Patients were deemed to be on ICS if they had been receiving treatment for at least 3 months. Simple logistic regression models were used to assess the association between TBM status and each proposed factor. A multivariable logistic regression model was used

Assessment and treatment of acute toxic inhalations.

PURPOSE OF REVIEW: Acute toxic inhalation exposures affect thousands of individuals worldwide each year. The acute evaluation of these inhaled exposures is often fraught with difficulty in identifying a specific agent, may involve multiple compounds, and a wide variety of responses are seen depending on the physical properties of the specific toxicant, the length of time of inhalation, and the concentration of the exposure. Recognizing key aspects of the most common acute toxic inhalations is useful in developing a diagnosis and treatment strategy. RECENT FINDINGS: Use of sequential observati…

Drug Interactions With Oral Inhaled Medications.

Objective: To evaluate the potential for drug interactions with oral inhaled medications (OIMs). OIMs include bronchodilators (β-agonists and antimuscarinics), corticosteroids, combination products (2 or more agents combined within a single inhalation device), antibiotics, prostacyclins, anesthetics, acetylcysteine, mucolytics, insulin, antivirals, nitric oxide, and nicotine replacement. Data Sources: A systemic literature search (1980 to May 2018) was performed using PubMed and EBSCO to locate relevant articles. The MESH terms used included each specific medication available as an OIM as well…

Penetration of inhaled aerosols in the bronchial tree.

It has long been recognized that the pattern of particle deposition in the respiratory tree affects how far aerosols penetrate into the deeper zones of the arterial tree, and hence contribute to either their pathogenic potential or therapeutic benefit. In this paper, we introduce an anatomically-inspired model of the human respiratory tree featuring the generations 0-7 in the Weibel model of respiratory tree (i.e., the conducting zone). This model is used to study experimentally the dynamics of inhaled aerosol particles (0.5-20µm aerodynamic diameter), in terms of the penetration fraction of p

Inhalant induced mania: An unexplored entity.

Inhalant abuse is one of the common presentations in de-addiction clinic among children and adolescent age group. Psychiatric morbidity is high among inhalant users but there is very little evidence to consider it a direct result of it. We present a case of 17-year-old boy with symptoms of mania which had time event correlation with inhalant use. He was successfully treated with antipsychotic medications and is maintaining well. The case highlights the need of strong suspicion of inhalant abuse in young patients who present with acute onset of psychiatric symptoms.

Internal dosimetry of inhaled iodine-131.

In this paper, the dose assessment for the iodine inhalation exposure in 19 aerosol sizes and three gas/vapor forms at three levels of thyroid uptake, was performed. Two different modes of work (light vs. heavy) and breathing (nose vs. mouth) for aerosol inhalation were investigated. In order to calculate the cumulated activities per unit of inhaled activity, a combined model which included the latest models of both human respiratory and alimentary tract was developed. The S values for 131I were computed based on the ICRP adult male and female reference voxel phantoms by the Monte Carlo method…

Effect of inhaled steroids on laryngeal microflora.

Conclusions As is known, this study is the first study to evaluate the effect of inhaled steroids on laryngeal microflora. The data support that ICS usage causes changes in the larynx microflora. Purpose The aim of this study was to determine the alteration in larynx microbial flora of the patients treated with ICS comparing the culture results of a control group. In addition, laryngeal microflora was compared to the smears obtained from the vallecula and pharynx. Materials and methods The study included 39 patients (mean age = 45.56 ± 12.76 years) who had been using a corticosteroid inhaler a

Olfactory deposition of inhaled nanoparticles in humans.

CONTEXT: Inhaled nanoparticles can migrate to the brain via the olfactory bulb, as demonstrated in experiments in several animal species. Th … r unit surface area is estimated to be higher in humans in the 1--7 nm size range due to the larger inhalation rate in humans. These dose estimates are important for risk assessment and dose-response studies investigating the neurotoxicity of inhaled nanoparticles.

Empyema thoracis from an inhaled peanut.

A 77-year-old man with a history of pulmonary sarcoidosis was referred with persistent cough and reduced air entry on auscultation of the right lung base. He was an ex-smoker with a 40-pack-year history and his general practitioner was concerned about the possibility of bronchogenic carcinoma. A chest radiograph showed a right-sided pleural effusion with right mid-zone airspace opacification. Bronchoscopy revealed a peanut covered in mucus lodged in the right lower lobe bronchus. CT of the thorax demonstrated a multiloculated right pleural effusion with associated compressive atelectasis, cons

Inhalational Steroids and Iatrogenic Cushing's Syndrome.

Bronchial asthma (BA) and Allergic rhinitis (AR) are common clinical problems encountered in day to day practice, where inhalational corticosteroids (ICS) or intranasal steroids (INS) are the mainstay of treatment. Iatrogenic Cushing syndrome (CS) is a well known complication of systemic steroid administration. ICS /INS were earlier thought to be safe, but now more and more number of case reports of Iatrogenic Cushing syndrome have been reported, especially in those who are taking cytochrome P450 (CYP 450) inhibitors. Comparing to the classical clinical features of spontaneous Cushing syndrome…

Inhaled antibiotics to treat lung infection.

The development of inhaled antibiotics to treat lung infection is an active field, with four approved products in the USA and more in the late stages of clinical development. The efficacies of TOBI® tobramycin (Novartis) and Cayston® aztreonam lysate (Gilead), the approved inhaled antibiotics for cystic fibrosis (CF) patients colonized with Pseudomonas aeruginosa, have been well documented. Recent approvals for a second-generation tobramycin solution, Bethkis®, and a tobramycin powder formulation in a dry-powder inhaler (DPI), TOBI Podhaler®, indicate that the inhaled antibiotic marketplace in

[The use of inhaled corticosteroids in preschool children].

Due to the large variation in symptoms presented, diagnosing wheezing or asthma and deciding on treatment for preschool children may be difficult. Inhaled corticosteroids may be effective for controlling asthma symptoms and recurrent wheezing also in this age group, but due to the heterogeneity of the disease and lack of diagnostic tools, both over- and under-treatment may occur. The author discusses current guidelines and recommendations for the use of inhaled corticosteroids in the preschool group. Inhaled corticosteroids are frequently prescribed in Norway for children in the first two year

Adverse consequences of acute inhalant intoxication.

Inhalants are widely misused by adolescents and are among the most toxic of psychoactive substances. This investigation examined the prevalence and correlates of adverse consequences of acute inhalant intoxication. Adolescent inhalant users (n = 279) in residential care completed structured interviews including assessments of the characteristics of their inhalant use. Multivariate logistic and linear regression and path analyses identified correlates of adverse inhalant intoxication-related experiences. Results of this study indicated that high-risk behaviors and adverse outcomes experienced d
