Mechanisms and Management of Acquired Aseptic Intracardiac Communications After Transcatheter Aortic Valve Implantation: Single-Center Case Series Insights.

BACKGROUND: Acquired aseptic intracardiac communications (AICCs) are rare but clinically relevant structural complications following transcatheter aortic valve implantation (TAVI), typically resulting from mechanical injury to the aortic annulus, interventricular septum, or adjacent structures. Evidence regarding their incidence, mechanisms, and optimal management remains limited. AIM: To review the mechanisms and management of post-TAVI AICCs and to present a single-center experience focusing on multimodality imaging, procedural characteristics, and outcomes of transcatheter closure. METHOD

Type A Aortic Dissection in the United Kingdom: The Untold Facts.

There is a lack of evidence on multiple levels for appropriate recognition, management, and outcome results in Type A aortic dissection management in the United Kingdom. A huge amount of retrospective data exists in the literature which provides nonmeaningful prospect to a service that meets the current era. Electronic searches were performed on PubMed and Cochrane databases with no limits placed on dates. Search terms were charted to MeSH terms and combined using Boolean operations, and also used as key words. Papers were selected on the basis of title and abstract. The reference lists of sel

Carcinoma of the tongue and tonsil (oropharynx).

In this article, the multidisciplinary approach to the management of carcinomas of the tongue and oropharynx has been emphasized. The decision for the primary mode of therapy will depend on the multiple factors discussed herein. However, a surgeon skilled in the management of head and neck tumors, including major reconstructive techniques, should lead the team. Most patients with head and neck carcinoma present with lesions that are relatively far advanced, and the combination of surgical excision and radiation therapy has proved to be the best method of controlling this disease. Impact of Abortion Bans on Emergency Care for Patients With Vaginal Bleeding: An Ethnographic Analysis.

BACKGROUND: The Dobbs decision enabled widespread state abortion bans, creating legal ambiguity for clinicians managing reproductive emergencies in the ED. While impacts on obstetricians are documented, less is known about how these laws influence emergency medicine practice. We explored how ED clinicians navigate care for patients with vaginal bleeding under Indiana's restrictive abortion ban. METHODS: We conducted a team-based ethnographic study at an urban academic ED and Level 1 Trauma Center in Indiana between August 2023 and October 2024. Data included six weeks of site-based observatio

Early Experience with Tarlatamab (T-Cell Engagers) for Extensive-Stage Small Cell Lung Cancer (ES-SCLC) in Canada: Lessons Learned and Implementation Strategies.

As bispecific T-cell engagers (TCEs) gain traction in the oncology treatment landscape, cancer centres must develop robust clinical pathways to ensure their safe and efficient delivery. Given the limited experience of the Canadian medical oncology community with TCEs, collecting and publishing early clinical experiences with these novel agents will be essential to inform best practices and support their safe and effective adoption across the broader Canadian oncology community. The approval of tarlatamab, the first-in-class delta-like ligand 3 (DLL3)-targeted TCE for extensive-stage small cell

Modernizing antibiotic regimens for obstetric infections through antimicrobial stewardship.

Maternal sepsis remains a major and largely preventable contributor to maternal morbidity and mortality, with intra-amniotic infection and postpartum endometritis remaining common obstetric sources of sepsis. Intra-amniotic infection complicates approximately 2%-5% of term deliveries, and postpartum endometritis occurs in up to 7% of births. Both conditions are typically polymicrobial, arising from ascending infection involving gram-positive, gram-negative, and anaerobic organisms. Despite rising antimicrobial resistance, the recommended antibiotic regimens for obstetric infections have remain

Educational programs and mental health outcomes in individuals with type 1 diabetes: a scoping review.

BACKGROUND: Mental health is a critical yet often underemphasized dimension in the management of individuals with Type 1 Diabetes Mellitus (T1DM), who are at elevated risk for psychological disorders. Educational interventions, including traditional education, psychoeducational, and psychosocial programs, are increasingly recognized as supporting self-care and promoting psychological well-being. AIM: This scoping review aims to systematically map the existing literature on educational programs for individuals with T1DM, with a specific focus on their impact on mental health outcomes. METHODS:

Anaesthetic considerations for delivery in an obstetric patient with a RYR1 gene variant: a case report.

Malignant hyperthermia (MH) is a rare, potentially life-threatening pharmacogenetic disorder most commonly associated with variants in the ryanodine receptor 1 gene (RYR1). Obstetric patients with undetermined risk of MH present distinct challenges, as the potential need for urgent delivery limits opportunities for anaesthetic preparation and necessitates a high level of multidisciplinary readiness. We describe the perioperative management of a 32-year-old woman heterozygous for a RYR1 splicing variant who underwent an elective caesarean delivery at 39 weeks' gestation following an in vitro fe

Valvular Heart Disease Care in Pakistan: Impact of the Multidisciplinary Valve Heart Team.

BACKGROUND: Patients with complex valvular heart disease (VHD) should be evaluated by a multidisciplinary heart team (HT). In low- and middle-income countries, referral practices are more variable, permitting any physician to refer patients directly to a cardiac surgeon without prior formal evaluation by a cardiologist with expertise in VHD. OBJECTIVES: The goal of the study was to examine the demographics of VHD patients seen in a large heart valve center in a low- and middle-income country and to assess the impact of the multidisciplinary HT in patients referred for valve surgery. Portal vein aneurysm, a pathological entity or an innocent bystander? A review of six cases.

Portal venous aneurysms (PVAs) are a rare venous aneurysms. The mean diameter of a healthy portal vein varies considerably, with maximum diameter of 15 mm in healthy subjects and 19 mm in cirrhotic patients. The presentation varies; they could come with abdominal pain or more often as an incident imaging finding. Although risk factors like portal hypertension and liver cirrhosis have been highlighted, the aetiology remains to be clarified. PVA may be associated with various complications: thrombosis, aneurysmal rupture, inferior vena cava obstruction or duodenal compression. A conservative tre
