Bordetella pertussis in a neonatal intensive care unit: identification of the mother as the likely source.

Bordetella pertussis, the cause of whooping cough, is highly contagious. A female, twin 1, born at 34 weeks of gestation and present on a neonatal intensive care unit for 19 days, became apnoeic and bradycardic. A pernasal swab, sent when pertussis was clinically suspected, grew B. pertussis. Twin 2 had similar symptoms. The mother admitted having a prolonged cough. Polymerase chain reaction of pernasal swabs was positive for both twins, and the mother had positive pertussis serology. An incident management committee was convened. Fifty neonates and 117 healthcare workers were identified as co

Early inflammation in the absence of overt infection in preterm neonates exposed to intensive care.

BACKGROUND: Systemic inflammation, typically attributed to sepsis, has been repeatedly linked to adverse long-term outcomes in infants born prematurely. However, it is unclear whether other factors can contribute to potentially harmful systemic inflammatory responses. OBJECTIVE: To determine the timing and extent of systemic inflammation occurring in absence of infection in preterm infants exposed to intensive care. METHODS: First, we screened for inflammation biomarkers most strongly linked to infection in a large prospective cohort of 425 newborns (gestational age 24-42 weeks). Second, we

[Determinants of doxapram utilization: a survey of practice in the French Neonatal and Intensive Care Units].

UNLABELLED: Methylxanthines and doxapram have been used to stimulate breathing and to prevent apnea in preterm infants. The use of doxapram is controversial because the therapeutic index seems to be narrow and short-term adverse effects have been described. OBJECTIVE: To determine the use of doxapram in the French neonatal and intensive care units. METHODS: A structured postal questionnaire was sent to all the 236 neonatology and neonatal intensive care units of level IIa, IIb and III in France. The questionnaires were analysed after four months. RESULTS: Answers were obtained from 159 chie

[Barotrauma in newborns. Study of 81 cases at a neonatal intensive care unit of a province].

We present 81 patients with diagnosis of air leaks in a Neonatal Intensive Care Unit (NICU). During 6 and a half year period. Air leaks were present in a 7.3% of overall admissions and in 18% in those whom received mechanical ventilation. We observed pneumothorax in all the patients, accompanied by pneumomediastinum in 12% and with interstitial emphysema in 7%. Other forms of air leak were present only rarely. Eighty percent of our patients were managed with intrapleural drainage. Almost 40% of events happened after resuscitation of tracheal aspiration management. The concomitant disease most

Facilitators and Barriers to Skin-to-Skin Care for Preterm Infants in a Neonatal Intensive Care Unit: An Integrative Review.

BACKGROUND: Skin-to-skin care (SSC) improves health outcomes in preterm infants. Given its benefits, it is crucial to understand the facilitators and barriers that influence its implementation within the socio-ecological model (SEM), which considers the intrapersonal, interpersonal, institutional, community, and policy-level factors. PURPOSE: This integrative review employed the SEM for synthesis and evaluation of the evidence surrounding facilitators and barriers for providing SSC with preterm infants and parents in the NICU. METHODS/SEARCH STRATEGY: PubMed, PsycINFO, CINAHL, and EMBASE wer

Norepinephrine Versus Dopamine as Initial Vasoactive Therapy in Neonatal Septic Shock: A Randomized Controlled Trial in a Tertiary Care Neonatal Intensive Care Unit in Eastern India.

Background Septic shock remains a major cause of neonatal morbidity and mortality, particularly in preterm and low birth weight neonates. Although dopamine was traditionally used as the first-line vasoactive agent in neonatal shock, norepinephrine has emerged as a potential alternative agent of late because of its potent vasoconstrictive effects and favourable hemodynamic profile. However, evidence comparing the efficacy of these agents in septic shock in neonates remains limited. Objective To compare the efficacy of norepinephrine and dopamine as the initial vasopressor in neonatal septic sho

Determinants of neonatal sepsis among neonates admitted to neonatal intensive care units in ethiopian hospitals: A systematic review and meta-analysis.

OBJECTIVE: Several studies have identified risk factors for neonatal sepsis, but they are limited to specific geographical areas with results that may not be generalizable to other populations. Hence, the objective of this study was to determine the contributing factors, representative at a national level, that influence the occurrence of neonatal sepsis in neonates receiving hospital care in Ethiopia. METHODS AND MATERIALS: A thorough search was conducted across PubMed/Medline, Hinari, Cochrane Library, and Google Scholar to identify relevant studies. The pooled odds ratio was estimated usin

The stability and incompatibility of parenteral nutrition in the neonatal intensive care unit: A narrative review.

The administration of parenteral nutrition (PN) to neonates presents challenges related to the stability and incompatibility of both 2-in-1 and 3-in-1 admixtures when co-administered with drugs. Incompatibility data derived from adult patients cannot be applied to neonates because of distinct clinical conditions and differences in components of the admixture (e.g., neonatal amino acid formulation). This review presents an overview of the stability and influencing factors, along with the incompatibility of drug-PN mixtures in neonates within the neonatal intensive care unit setting. Various nut

Prevalence of thyroid dysfunction in neonates receiving parenteral nutrition in the intensive care unit.

OBJECTIVE: The use of parenteral nutrition (PN) formulations that do not contain iodine can contribute to the deficiency of this mineral, potentially leading to hypothyroidism and, ultimately, neurocognitive impairments. This study aimed to evaluate TSH alterations in newborns receiving PN. METHODS: Retrospective study of neonatal intensive care unit patients receiving PN for > 15 days. Nutritional, anthropometric, and biochemical variables (TSH, T4, CRP) were analyzed. Hypothyroidism was defined by TSH > 10 mU/L. RESULTS: Two hundred newborns were evaluated [156 (78%) preterm, 31±5 weeks of

Pattern discovery in critical alarms originating from neonates under intensive care.

Patient monitoring generates a large number of alarms, the vast majority of which are false. Excessive non-actionable medical alarms lead to alarm fatigue, a well-recognized patient safety issue. While multiple approaches to reduce alarm fatigue have been explored, patterns in alarming and inter-alarm relationships, as they manifest in the clinical workspace, are largely a black-box and hamper research efforts towards reducing alarms. The aim of this study is to detect opportunities to safely reduce alarm pressure, by developing techniques to identify, capture and visualize patterns in alarms.

The pattern of cerebral injury in a primate model of preterm birth and neonatal intensive care.

Survivors of very premature birth face an increased risk of adverse motor, cognitive, and behavior sequelae. In order to understand the pathogenesis of these adverse outcomes, an animal model of premature birth and neonatal care in a species with a close similarity to the human infant is sought. In this histological and immunohistochemical study we have defined the pattern of cerebral injury in a premature baboon model undergoing similar neonatal intensive care to that of the human premature infant. Sixteen baboons were delivered at 125 days gestation (dg; term approximately184 dg) with 14 day

Noise levels within the ear and post-nasal space in neonates in intensive care.

BACKGROUND: Noise exposure in neonatal units has long been suspected of being a cause of hearing loss associated with such units. The noise intensity to which the neonate is exposed varies with the type of ventilatory support used. Also, the post-nasal space is an enclosed cavity that is close to the inner ear and an area of turbulent and hence potentially noisy airflow. AIM: To determine noise intensities within the ear and post-nasal space in neonates on different modes of ventilatory support using probe microphones, measures previously not undertaken. METHODS: A portable instrument with a

The next challenge for newborn intensive care in Alaska: improving the survival of the larger neonate.

Using information from our database, a review of mortality for the Newborn Intensive Care Unit at Providence Alaska Medical Center was conducted for 1987-1996. There has been a significant decline in mortality over the last decade (p = 0.003). An analysis of mortality by birthweight and gestational age groups demonstrated a decline in mortality (p = 0.005) for infants with birthweight < 2 kg and infants < or = 34 weeks gestation, but no change for infants > or = 2 kg and > or = 35 weeks gestation. As a result, larger and more mature babies now account for an increasing proportion of NICU death

Utility of point-of-care ultrasound of the liver in the neonatal intensive care unit: experience from a case series.

INTRODUCTION: Point-of-care ultrasound (POCUS) is increasingly being utilized for the management of sick and premature neonates in the neonatal intensive care unit (NICU). While neonatologist-performed cranial, lung, and cardiac ultrasound have become the standard of care in many NICUs across the world, the use of POCUS for the evaluation of the liver remains relatively unexplored. MATERIALS AND METHODS: This case series included neonates admitted to a tertiary-care NICU between January 2021 and April 2025, with radiological liver abnormalities detected on point-of-care ultrasound by treating
