Meconium peritonitis due to fetal appendiceal perforation: two case reports and a brief review of the literature.

BACKGROUND: Meconium peritonitis is an infrequent congenital disease usually caused by perforation of the fetal digestive tract. Meconium peritonitis resulting from intrauterine appendiceal perforation has been rarely reported and is often overlooked during pregnancy. We herein report two cases of fetal appendiceal perforation. CASE PRESENTATION: Two neonates were found to have intestinal distension and gradually increasing ascites antenatally. After birth, diagnostic abdominal punctures revealed meconium peritonitis. Urgent surgery showed both neonates had developed gangrenous appendicitis i

Two Cases of Aortic Intimal Intussusception During Endovascular Repair of an Acute Type B Dissection.

PURPOSE: To raise awareness of a previously undescribed complication of thoracic endovascular aortic repair (TEVAR) for aortic dissection that can lead to acute aortic occlusion and to highlight that early recognition, excision of the intimal flap, and open aortic repair can be lifesaving. CASE REPORT: Two patients underwent TEVAR for acute type B dissections complicated by abdominal malperfusion syndrome. During stent-graft deployment, the intimal flap circumferentially detached from its origin proximally with subsequent intussusception, leading to acute aortic occlusion. Both complications

[Complications of knee arthroscopy].

BACKGROUND: Today, knee arthroscopy is one of the standard interventions performed by an orthopaedist and usually has a low potential for complications. PURPOSE: The surgeon should still be aware of possible problems, and be able to control and explain them to the patient in detail before surgery. MATERIALS AND METHODS: The possible relevant peri-, intra- and postoperative complications of knee arthroscopy are discussed. RESULTS: Evaluation of the patient's medical history and comorbidities is crucial to successful treatment, in addition to a correct diagnosis and indications with the assis

Bilateral pulmonary emboli associated with intraoperative use of thrombin-based hemostatic matrix following lumbar spine interbody fusion.

Here we describe a patient with bilateral pulmonary emboli (PE) associated with thrombin-based hemostatic matrix (TBHM) use in the setting of a possible venous injury during transforaminal lumbar interbody fusion in the treatment of degenerative spondylolisthesis at L4-5. TBHM products are gelatin granules mixed with human or bovine thrombin. They have been used in a wide variety of surgical procedures to facilitate local hemostasis though their use is not without complications. This is the first reported patient, to our knowledge, with a TBHM-related PE following spinal fusion. As TBHM is a w

Transpancreatic hepatomesenteric trunk complicating pancreaticoduodenectomy.

CONTEXT: Standard celiac and hepatic arterial anatomy occur in approximately 60% of the patients; for the remaining, multiple variations have been described. A thorough knowledge of these anomalies is important in order to avoid unnecessary complications. In this report we describe one of the rarest arterial anomalies, a hepatomesenteric trunk supplying the liver. We attempt to elucidate its implications pertaining to the safe performance of a pancreaticoduodenectomy. CASE REPORT: A 45-year-old male with a one-month duration painless progressive jaundice was evaluated and diagnosed as having

Spectral mapping reveals a resemblance of the anesthetic brain state to both sleep and coma.

General anesthesia is often compared to sleep but may more closely resemble a medically induced coma. While all three states involve a loss of awareness, the extent of their neural similarity remains unclear. Electrophysiological markers, such as delta activity (< 4 Hz), are present in slow wave sleep, disorders of consciousness (DoC, including coma), and propofol anesthesia but are absent during rapid eye movement (REM) sleep. Frontal alpha oscillations are a key feature of propofol anesthesia and detectable via intraoperative EEG. However, it remains unclear whether alpha and delta activity

Case Report of an Aggressive Angiomyxoma Presenting as Pelvic Organ Prolapse.

Aggressive angiomyxoma (AAM) is a rare, slow-growing, locally infiltrative mesenchymal tumor of the pelvis and perineum, most often affecting women. Its nonspecific presentation and tendency to mimic more common vaginal or periurethral masses frequently delay diagnosis. Early recognition is critical to guide appropriate surgical planning, optimize resection, and reduce recurrence risk. In this case, a 61-year-old woman with prior vaginal hysterectomy presented with symptoms of pelvic organ prolapse, urinary incontinence, and occasional bleeding. Examination revealed a 10 × 3-cm protruding ante

Sigmoid gallbladder in a 38-year-old female with gallstone disease: a case report.

BACKGROUND: The sigmoid gallbladder is a rare anatomic variant characterized by an S-shaped curve in its structure, often stemming from abnormal embryonic development during gestation. This anomaly complicates surgical procedures like cholecystectomy, primarily due to its convoluted shape and potential for misdiagnosis. Case studies indicate that preoperative imaging techniques, including ultrasound and MRCP, are essential for accurate diagnosis and preventing bile duct injuries. Although sigmoid gallbladders are typically asymptomatic unless associated with gallstones, thorough imaging and su

Femoral nerve palsy following total hip arthroplasty via posterolateral approach: a report of two cases and literature review.

Femoral nerve palsy is a rare but serious complication following total hip arthroplasty (THA), particularly when performed through the posterolateral approach. Early diagnosis and prompt rehabilitation are crucial for functional recovery. We report two cases of femoral nerve palsy following THA, highlighting the clinical course, management, and outcomes. The first case involved a 53-year-old male with ankylosing spondylitis that underwent right-sided uncemented THA via the posterolateral approach. Postoperatively, he developed femoral nerve palsy, likely due to skin traction applied for residu

Female sexual function evaluation and intraoperative vaginal reconstruction in bladder cancer.

RC significantly negatively impacts sexual function (SF) in both men and women. While significant research resources have been allocated to examine the deleterious effects of post prostatectomy erectile dysfunction, little attention has been directed towards female sexual function and organ preservation post cystectomy. These academic shortcomings often result in poor provider awareness and inadequate preoperative assessment. As such, it is crucial for all providers involved in female RC care to understand the necessary and available tools for preoperative evaluation, in addition to the anatom

Complications in craniosynostosis surgery in patients with rickets: illustrative case and systematic review of literature.

BACKGROUND: Craniosynostosis (CSS) is the premature fusion of calvarial sutures associated with identified genetic mutations or secondary to alterations in intracranial pressure, brain, or bone growth patterns. Of the metabolic etiologies implicated in CSS, X-linked hypophosphatemic rickets (XLHR) is the most common, with dysfunctional bone mineralization leading to progressive hyperostosis and delayed synostosis. There is a paucity of literature discussing the unique surgical considerations for XLHR-related CSS. OBSERVATIONS: A 26-month-old male with XLHR-related sagittal CSS underwent crani

Anatomy and motor function of extra-laryngeal branching patterns of the recurrent laryngeal nerve; an electrophysiological study of 1001 nerves at risk.

INTRODUCTION: Safe thyroid surgery depends on a deep knowledge of human neck anatomy, including the recurrent laryngeal nerve (RLN). Anatomic variations such as extra-laryngeal terminal branching (ETB) are common. PATIENTS AND METHODS: We studied the ETB pattern of 1001 RLNs at risk in 596 patients. We identified and exposed the location of division points on the cervical part of bifid RLN. The function of nerve branches was assessed through intraoperative nerve monitoring (IONM). RESULTS: Bifid RLNs was identified in 39.6% of patients. The nerve-based prevalence of ETB was 28.5%. The preval

Acute inferior wall myocardial infarction induced by aortic dissection in a young adult with Marfan syndrome: A case report.

BACKGROUND: Aortic dissection (AD) is an emergent and life-threatening disorder, and its in-hospital mortality was reported to be as high as 24.4%-27.4%. AD can mimic other more common disorders, especially acute myocardial infarction (AMI), in terms of both symptoms and electrocardiogram changes. Reperfusion for patients with AD may result in catastrophic outcomes. Increased awareness of AD can be helpful for early diagnosis, especially among younger patients. CASE SUMMARY: We report a 28-year-old man with acute left side chest pain without cardiovascular risk factors. He was diagnosed with
