Factors associated with crisis pregnancies in Ireland: findings from three nationally representative sexual health surveys.

BACKGROUND: Findings on the demographic and sexual health characteristics associated with the experience of a crisis pregnancy are important to inform the public health policy of a country, including Ireland. Studies from other jurisdictions have suggested that certain demographic groups are at risk for unintended pregnancies and the disparity between the groups has been growing in recent years. Ireland is a country which experienced much economic and societal change in the first decade of the 21(st) century; changes which are likely to have affected demographic variables pertaining to sexual …

PFGE analysis of Listeria monocytogenes isolates of clinical, animal, food and environmental origin from Ireland.

Listeria monocytogenes is an important foodborne human pathogen. Human infection is associated with high mortality rates. Epidemiological investigation and molecular subtyping can be useful in linking human illness with specific sources of infection. This retrospective study describes the use of PFGE to examine relationships of 222 isolates from human and non-human sources in Ireland. Human clinical isolates from other countries were also examined. Eight small clusters of human and non-human isolates (mostly serotype 4b) that were indistinguishable from one another were detected, suggesting po…

Geographical variation in infant mortality, stillbirth and low birth weight in Northern Ireland, 1992-2002.

BACKGROUND: Improving the health of expectant mothers and reductions in health inequalities, are repeatedly prioritised in policy reports in England and Northern Ireland. Measurement of underlying rates, and geographical variation in rates, of adverse birth outcomes are tools in monitoring these priorities. METHODS: Northern Ireland data on stillbirths, infant mortality and low birth weight (1992-2002) were linked to board (n=4), district council (n=26) and 1991 census wards (n=568). Underlying variations in rates were estimated at each geographical level, unadjusted and controlling for year,…

Radiation doses for barium enema and barium meal examinations in Ireland: potential diagnostic reference levels.

Wide variations in patient dose for the same examinations have been demonstrated by several studies throughout Europe. By investigating patient dose, variations can be acknowledged, causal agents sought and the necessary adjustments made. Diagnostic reference levels (DRLs) provide a framework with which dose levels from individual hospitals are compared, and when exceeded, corrective actions can be taken where appropriate. This study aimed to establish DRLs for barium enema and barium meal examinations in Ireland. Measurements were recorded using a dose-area product meter in 12 hospitals repre…

Neonatal Diabetes in Ireland over the Past 19 Years: Clinical Presentation, Management, Genetics, and Outcomes.

INTRODUCTION: Neonatal diabetes mellitus (NDM) is a rare monogenic form of diabetes presenting before 6 months of age. It may be permanent (PNDM) or transient (TNDM). Individuals with KATP channel variants may respond to oral sulphonylurea instead of insulin. The study aimed to review the presentation, genotype, phenotype, management, and outcomes of patients diagnosed with NDM in Ireland over 19 years. METHODS: Data on Irish NDM cases from 2006 to 2024 were collated through paediatric endocrinologists nationally and electronic databases. Analyses were performed using SPSS, with ethical appro…

Molecular identification of Kudoa thyrsites parasite isolated from Atlantic mackerel (Scomber scombrus) landed in Ireland.

Post-mortem myoliquefaction, commonly referred to as 'jelly flesh', is associated with infection by myxozoan parasites of the genus Kudoa, particularly Kudoa thyrsites; the effects of infections can render fish fillets unusable in food preparation. Although the parasite has been documented in Atlantic mackerel (Scomber scombrus) across parts of the Northeast Atlantic, its presence in mackerel landed in Ireland has not been previously confirmed. In this study, mackerel obtained from commercial landings were screened for signs of post-mortem myoliquefaction. Three individuals displaying characte…

Improving oral health in Ireland: a dental and public health nurse collaborative quality improvement initiative.

INTRODUCTION: The Health Service Executive in Ireland has piloted dentist-led initiatives targeting children aged 4-5. These initiatives involved an intra-oral check by dentists in the school setting, and a subsequent dental clinic appointment if dental decay was identified. The aim of this project was to explore the best method of delivering school-based, intra-oral examinations, by piloting a programme to identify children with dental decay with allied health professionals. METHODS: A quality improvement framework (Plan Do Study Act) was used for this oral health improvement project. The in…

Retrospective cross-sectional analysis of concurrent VTE diagnosis in hospitalised socially excluded individuals in Ireland.

OBJECTIVE: Social exclusion (such as that experienced by people who are homeless, incarcerated or use drugs) increases morbidity across a range of diseases but is poorly captured in routine data sets. The aim of this study was to use a novel composite variable in a national-level hospital usage dataset to identify social exclusion and to determine whether social exclusion is associated with concurrent venous thromboembolism (VTE) in hospitalised patients in Ireland. Identifying and characterising this association in people who are socially excluded will inform VTE prevention and treatment stra…

Geocoding cryptosporidiosis cases in Ireland (2008-2017)-development of a reliable, reproducible, multiphase geocoding methodology.

BACKGROUND: Geocoding (the process of converting a text address into spatial data) quality may affect geospatial epidemiological study findings. No national standards for best geocoding practice exist in Ireland. Irish postcodes (Eircodes) are not routinely recorded for infectious disease notifications and > 35% of dwellings have non-unique addresses. This may result in incomplete geocoding and introduce systematic errors into studies. AIMS: This study aimed to develop a reliable and reproducible methodology to geocode cryptosporidiosis notifications to fine-resolution spatial units (Census 2…

Economic Evaluation of Selective Neonatal Bacillus Calmette-Guérin Vaccination of High-risk Infants in Ireland.

BACKGROUND: In Western Europe, currently only Ireland and Portugal continue to provide universal neonatal bacillus Calmette-Guérin (BCG) vaccination programs, despite not being considered as high tuberculosis (TB) incidence countries. Other European countries only vaccinate infants considered at high risk of contracting TB. We evaluated the cost-effectiveness of selective BCG vaccination compared with strategies of universal and no vaccination. METHODS: An economic model was used to simulate a cohort from birth to life expectancy, taking the perspective of the publicly funded healthcare syste…

Laboratory trend in vitamin D status in Ireland: Dual concerns about low and high 25OHD.

Serum 25-hydroxyvitamin D (25OHD) is the pre-eminent estimate of vitamin D status that we have been measuring in a hospital laboratory setting since the 1970s. We previously evaluated the trend in 25OHD results in our laboratory from 1993 to 2013. Using a time series analysis of monthly average 25OHD results the trend was modelled, and this was used to forecast monthly average 25OHD from 2014 to 2016. In this study, all 25OHD results from 2014 to 2016 were retrieved (n = 67,922) and trimmed to 40,307 results after duplicates were excluded. The average monthly actual 25OHD was almost identical

The Molecular Epidemiology of Resistance in Cefotaximase-Producing Escherichia coli Clinical Isolates from Dublin, Ireland.

In view of continued high clinical prevalence of infections involving extended-spectrum β-lactamase (ESBL)-producing Escherichia coli, this … dentified. CTX-M plasmids and integrons resembled those identified recently in animal isolates from Ireland and Western Europe. The molecular epidemiology of CTX-M-producing E. coli in Dublin suggests that horizontal spread of mobile genetic elements contributes to antimicrobial resistant human infections. Further investigations into whether animals or an …

Ergot alkaloid intoxication in perennial ryegrass (Lolium perenne): an emerging animal health concern in Ireland?

Four primary mycotoxicosis have been reported in livestock caused by fungal infections of grasses or cereals by members of the Clavicipitaceae family. Ergotism (generally associated with grasses, rye, triticale and other grains) and fescue toxicosis (associated with tall fescue grass, Festuca arundinacea) are both caused by ergot alkaloids, and referred to as 'ergot alkaloid intoxication'. Ryegrass staggers (associated with perennial ryegrass Lolium perenne) is due to intoxication with an indole-diperpene, Lolitrem B, and metabolites. Fescue-associated oedema, recently described in Australia,

Enhanced surveillance of initial cases of pandemic H1N1 2009 influenza in Ireland, April-July 2009.

From 28 April to 18 July 2009 there were 156 cases of pandemic H1N1 2009 influenza confirmed in Ireland. During this time, Ireland was in containment phase, and detailed case-based epidemiological information was gathered on all cases presenting in the community and acute health care setting. Active case finding was performed among contacts of cases. Eighty percent of cases were in people less than 35 years of age and 86% were imported. The most frequent symptoms were fever, sore throat, myalgia and dry cough. Nine people were hospitalized, no fatalities occurred.
