Growing evidence suggests an involvement of iron in the pathophysiology of neurodegenerative diseases. Several of the diseases are associated with parkinsonian syndromes, induced by degeneration of basal ganglia regions that contain the highest amount of iron within the brain. The group of neurodegenerative disorders associated with parkinsonian syndromes with increased brain iron content can be devided into two groups: (1) parkinsonian syndromes associated with brain iron accumulation, including Parkinson's disease, diffuse Lewy body disease, parkinsonian type of multiple system atrophy, prog…

In the absence of genetic hemochromatosis and systemic hemosiderosis, patients with cirrhosis can accumulate focal iron within regenerative or dysplastic hepatic nodules, commonly referred to as 'siderotic nodules'. Siderotic dysplastic nodules are premalignant lesions while siderotic regenerative nodules are a marker for severe viral or alcoholic cirrhosis. The relationship of hepatic iron deposition to hepatic cirrhosis and neoplasia has not been fully clarified. This article will review the current literature regarding selective iron accumulation in siderotic nodules in chronic liver diseas…

Synthetic modeling studies of non-heme iron proteins continue to contribute to our understanding of the mechanism of these proteins. Recently, mononuclear Fe(IV)=O complexes have been prepared and characterized to model the same species that are proposed to be the reactive intermediates in reactions involving mononuclear non-heme iron proteins. Generation of such species for the oxidation of organic substrates has also been demonstrated. Other advances include successful modeling of the structural and functional aspects of diiron non-heme proteins with the use of terphenyl-based carboxylate li…

Dietary low iron bioavailability intake is an important causation factor of iron deficiency anemia in Asian countries including Thailand. The aim of this study was to estimate the iron bioavailability in the Thai diet by a calculation method that is based on dependent factors, dietary components and physiological iron store. Based on the the latest national nutrition survey of the Thai diet, 1995, the data of nutrient intake per capita per day by region were used for calculating the iron bioavailability at physiological iron store levels; 0, 250, 500 and 1,000 mg of iron. The results showed th…

The possible causes of abnormal iron metabolism in patients with Friedreich's ataxia are considered. Reduced expression of a frataxin homologue in yeast is associated with mitochondrial iron accumulation at the expense of cytosolic iron, and the same phenomenon can be demonstrated in these patients. A decrease in cytosolic iron causes the expression of a high-affinity iron-uptake protein, and therefore Friedreich's ataxia can be considered to be a disease of abnormal intracellular iron distribution. Friedreich's ataxia is of autosomal recessive inheritance, and the gene associated with it has …

Iron is an oxidant as well as a nutrient for invading microbial and neoplastic cells. Excessive iron in specific tissues and cells (iron loading) promotes development of infection, neoplasia, cardiomyopathy, arthropathy, and various endocrine and possibly neurodegenerative disorders. To contain and detoxify the metal, hosts have evolved an iron withholding defense system, but the system can be compromised by numerous factors. An array of behavioral, medical, and immunologic methods are in place or in development to strengthen iron withholding. Routine screening for iron loading could provide v…

Recent molecular studies have resulted in the identification of genetic alterations underlying hereditary disorders of iron metabolism. One example is the discovery of the HFE gene that is mutated in patients suffering from hereditary hemochromatosis. This autosomal recessive disorder has an estimated carrier frequency that varies between 0.07 and 0.13, thus representing one of the most common genetically determined metabolic disorders. The identification of the hemochromatosis mutations has encouraged efforts to investigate other conditions with iron overload for a putative interaction with t…

Iron deficiency is seen commonly in women during their reproductive years and particularly during pregnancy. The physician who primarily cares for women should be aware of this and initiate iron supplementation when indicated. The association of poor pregnancy outcome is of concern when iron deficiency is present in the first trimester. It is unclear whether this is because of the anemia or other epidemiologic factors. It is also unknown whether correcting iron deficiency before conception improved neo-natal outcome. Until more is known concerning IDA, it is recommended that pregnant women rec…

PURPOSE: Iron overload unexplained by dietary or medicinal iron excess, transfusion, or sideroblastic anemia has been described infrequently in Americans of African descent. The purpose of this study was to characterize iron overload attributable to excessive iron absorption in African Americans.
PATIENTS AND METHODS: In a community hematology and medical oncology practice during the interval 1990 to 1993, we identified and evaluated a series of cases comprised of 6 men and 1 woman, with a mean age of 55 +/- 14 (SD) years (range 33 to 69). Data on clinical features, serum iron parameters, liv…

AIM: To determine the prevalence of iron deficiency anaemia in infants presenting to their general practitioner in a socially deprived, ethnically mixed suburb.
METHODS: A total of 43 infants between 6 and 36 months of age were tested for iron deficiency anaemia. The blood tests were collected opportunistically over a 10 month period. Infants with other than trivial illness were excluded. Parents of infants with evidence of iron deficiency were offered dietary counselling, and infants with iron deficiency anaemia were treated with oral iron therapy. Follow up blood tests for iron deficient in…

Seven patients with varying psychiatric disorders were found to have iron overload as manifested by abnormal serum ferritin, transferrin saturation index (TSI), or excessive urinary iron. All possible sources of secondary iron overload were ruled out. The patients were treated with the specific iron chelator, deferoxamine, given IM for seven to 22 weeks which resulted in significant clinical improvements. These cases indicate a need to be aware that disordered iron metabolism is a somatic cause of psychiatric illness and that there is clinical improvement upon lowering elevated iron levels in …

Iron deficiency has far reaching consequences on systemic functions apart from the well known feature of anemia. Because iron deficiency anemia is easy to recognize and treat, the non-hematological manifestations of this trace element deficiency have been overlooked. There is a large body of evidence accumulated from animal and human studies to indicate that iron deficiency states are associated with alterations in cellular function, growth, motor development, behaviour and cognitive function. There are consequences for the host's level of immunocompetence against infection, for physical work …

An oxidative stress has been reported to occur at the hepatic level after ethanol administration. We recently reported that such a stress is also apparent at the cerebellar level during acute ethanol intoxication in rats. Since low molecular weight iron chelates (LMW-Fe) are involved in the biosynthesis of aggressive prooxidant species we presently studied the influence of acute and chronic ethanol administration on hepatic and cerebellar total non-heme iron and LMW-Fe. The results show that an acute ethanol load (50 mmoles/kg b. wt.) administered (i.p.) to male Sprague-Dawley rats elicits alt…

Advances in the treatment of iron overload hold the promise of making long-term transfusion therapy a safer approach to the management of thalassemia major, sickle cell disease, and numerous other hematologic disorders. The responsibility now placed on the treating physician is to monitor iron stores carefully, to begin chelation therapy at an appropriate time, and to encourage regular use of deferoxamine. For the clinical investigator, the success of chelation therapy with deferoxamine should prompt an intensive search for new chelators. These new chelators should be not only effective but al…

Iron deficiency anaemia was detected in 23% of cases with homozygous sickle cell disease. The aetiology of iron deficiency was similar to the other population in the community. High serum ferritin level was detected in 15.4% of the cases and was well correlated to the number of transfusions. Tissue haemosiderosis was not detected in any case. Patients with heterozygous sickle cell had either normal or low serum ferritin levels.

In brief: Using several hematologic indexes, researchers examined the iron status of 50 ultraendurance athletes participating in the 1988 Ironman Triathlon in Hawaii. Fifteen (30%) of the 50 triathletes had at least one suboptimal iron index: Two exhibited stage 2 iron deficiency in the presence of normal hemoglobin levels; eight had normal ferritin levels but subnormal levels of transferrin saturation and iron; three had abnormal levels of total iron-binding capacity; and two had low hemoglobin levels with all other iron indices normal, which indicated dilutional anemia. These data suggest th…

Cells of plants, most microorganisms, and animals require well-defined amounts of iron for survival, replication, and differentiation. The metal is an important component of such processes as synthesis of DNA, RNA, and chlorophyll; electron transport; oxygen metabolism; and nitrogen fixation. Because of the insolubility of iron in aerobic environments at neutral and alkaline pH values, cells have had to devise specific strategies to assimilate the metal. These include (1) development of systems for reducing ferric ions to the more soluble ferrous ions at the cell surface, (2) employment of sma…

Even when simple iron deficiency is strongly suspected, the child with severe anemia deserves a careful and thorough work-up before treatment is begun. Many other possible causes of anemia need to be excluded. No one test specifically identifies iron deficiency, but the combined use of several tests of iron status can establish iron deficiency with certainty. Hemoglobin level, mean corpuscular volume, serum iron, total iron binding capacity, free erythrocyte porphyrin and serum ferritin are the important studies.
