[Corneal complications after PRK for hyperopia].

We report the case of a patient with epithelial hyperplasia, peripheral haze and regression after transepithelial photorefractive keratectomy (PRK) for hyperopia outside the limits set by the committee for refractive surgery (KRC). Retreatment was performed on both eyes 1 year postoperatively and later an ablation on the left eye due to scarring. Regardless of these procedures haze, scarring and regression appeared again in both eyes. The patient will currently retain a permanent reduction of visual acuity as another treatment of hyperopia outside the KRC recommendations should not be performe…

[A case report of SMILE for refractive regression nine years after myopia correction with laser in situ keratomileusis].

A myopic patient with self-reported photorefractive keratectomy 9 years ago requested small incision lenticule extraction (SMILE) as retreatment. The presence of interface bubbles while creating a corneal cap of the right eye revealed that the previous refractive surgery was laser in situ keratomileusis (LASIK). Therefore, the SMILE procedure was resumed in the right eye, but flap-lift LASIK was performed in the left eye. Postoperative follow-up showed that the vision in both eyes reached the preoperative best corrected level. SMILE may be a potential option for refractive regression after LAS…

Compound nevus of the cornea simulating a foreign body.

PURPOSE: To report a corneal nevus, without conjunctival involvement, that clinically simulated a metallic foreign body. METHODS: Case report with clinicopathologic correlation. RESULTS: A 22-year-old man developed a superficial pigmented lesion of his left cornea. It was suspected to be a chronically retained foreign body, but a melanocytic neoplasm could not be excluded. The lesion was removed by lamellar keratectomy and proved histopathologically to be a compound nevus of the cornea, without conjunctival involvement. CONCLUSIONS: Melanocytic nevus can rarely occur in the cornea. Involvem…

Corneal Epithelial-Stromal Fibroblast Constructs to Study Cell-Cell Communication in Vitro.

Cell-cell communication plays a fundamental role in mediating corneal wound healing following injury or infection. Depending on the severity of the wound, regeneration of the cornea and the propensity for scar development are influenced by the acute resolution of the pro-fibrotic response mediated by closure of the wound via cellular and tissue contraction. Damage of the corneal epithelium, basement membrane, and anterior stroma following a superficial keratectomy is known to lead to significant provisional matrix deposition, including secretion of fibronectin and thrombospondin-1, as well as …

Fourth-generation fluoroquinolone-resistant bacterial keratitis after refractive surgery.

We report the first 2 cases of bacterial keratitis resistant to fourth-generation fluoroquinolones after laser in situ keratomileusis (LASIK) and photorefractive keratectomy (PRK). The first patient had Pseudomonas aeruginosa keratitis after PRK despite treatment with moxifloxacin. The second patient was on gatifloxacin post-LASIK when she had methicillin-resistant Staphylococcus aureus (MRSA) keratitis diagnosed. In both cases, culture susceptibilities showed isolates resistant to moxifloxacin and gatifloxacin, and treatment with topical aminoglycosides and surgical intervention was necessary…

Corneal topography in preoperative evaluation for laser keratorefractive surgery - a review.

Corneal topography is a mandatory investigation in the preoperative evaluation of the patient candidate for laser keratorefractive surgery, in order to assess the corneal shape, to determine the radii of curvature and the corneal thickness. Abnormal corneal topography is the most important identifiable risk factor for corneal ectasia. This paper reviews the principles of successive generations of topographers and illustrates several normal and abnormal corneal topographies.

Excimer laser surface ablation: a review of recent literature.

The aim was to review the recently published literature on excimer laser surface ablation procedures, including photorefractive keratectomy (PRK), laser sub-epithelial keratomileusis (LASEK), microkeratome-assisted PRK (epi-LASIK) and trans-epithelial (laser-assisted) PRK, to help elucidate where and how surface ablation may best fit into current refractive surgical practice. The emphasis was on publications within the last three years and included systemic reviews, meta-analyses and randomised controlled trials. Where such evidence did not exist, selective large series cohort studies, case-co…

Big-bubble technique to bare Descemet's membrane in anterior lamellar keratoplasty.

We describe a lamellar keratoplasty technique to bare Descemet's membrane in which air is injected to detach the central Descemet's. After a partial-thickness corneal trephination is performed, a disposable needle is inserted, deeply and bevel down, into the paracentral corneal stroma and air is injected. In most cases, this forms a large air bubble between Descemet's membrane and the corneal stroma. After anterior lamellar keratectomy is performed, a small opening is made in the air bubble and the remaining stromal layers are lifted with an iris spatula, severed with a blade, and excised with…

Ultrastructural Aspects of Corneal Functional Recovery in Rats Following Intrastromal Keratocyte Injection.

PURPOSE: Donor tissue shortfalls and postsurgical complications are driving novel corneal tissue regeneration approaches. Corneal stromal keratocytes (CSKs) have shown promise in promoting corneal repair and restoring transparency. We investigated the impact of intrastromal CSK injection on corneal ultrastructure and proteoglycan (PG) distribution in a rat injury model. METHODS: Rats were divided into four groups: normal (n = 12), injured (irregular phototherapeutic keratectomy centrally; n = 6), CSK (injured + human CSK intrastromal injection; n = 6), and PBS (injured + PBS injection; n = 6)…

[Histological and ultrastructural study of corneal endothelium after excimer laser ablation in rabbit].

To investigate the effect of excimer laser ablation on the corneal endothelium in the early period, an excimer laser ablation was performed on rabbit cornea. Electron microscopy revealed an irregular space in the endothelial membrane facing Descemet's membrane 48 hours after the laser ablation. At 60 hours, electron-dense material was observed between the endothelial cells and Descemet's membrane. At 72 hours, electron-dense material was observed in Descemet's membrane, but no morphologic change was observed in the endothelium. These results suggest that excimer laser ablation may affect rever

Simultaneous Topography-Guided PRK/CXL Versus Topography-Assisted PTK/CXL: 1-Year Prospective Outcomes in Keratoconic Eyes.

PURPOSE: To compare 1-year visual and tomographic outcomes of topography-guided photorefractive keratectomy (TGPRK) and topography-assisted phototherapeutic keratectomy (TPTK) with corneal cross-linking (CXL). METHODS: TGPRK and TPTK were performed in 72 eyes (68 patients) and 74 eyes (71 patients), respectively. Based on the TGPRK ablation plan, the eyes underwent TPTK where the theoretical minimum corneal thickness (MCT) after surgery was less than 400 µm. In the TGPRK group, the theoretical maximum ablation depth was 50 µm after epithelium removal. In TPTK, a decentered single-step PTK was…
