Differential expression of salivary microRNA associated with keratoconus.

Research into biomarkers has transformed the identification and management of many diseases. Keratoconus (KC) is a multifactorial corneal ectasia, and due to limited understanding of disease pathophysiology, no biomarkers have been fully validated. microRNAs (miRNAs) are involved in regulating gene expression. Differentially expressed miRNAs have been reported in KC corneal tissue and as biomarkers for other ocular disorders. Salivary biomarkers mirror systemic processes and saliva contains substantial miRNAs. However, it is unknown if salivary miRNAs are differentially expressed in KC. Saliva…

Management of astigmatism in keratoconus with therapeutic refractive surgery.

Astigmatism is a major determinant of visual impairment in keratoconus (KC), yet in this setting it cannot be understood as a simple cylindrical refractive error. Instead, it reflects a complex optical phenotype generated by anterior and posterior corneal asymmetry, epithelial remodeling, internal optical compensation, and progressive biomechanical instability, often in combination with coma-dominant higher-order aberrations. Beyond reducing monocular image quality, KC-related astigmatism may also compromise binocularity through interocular asymmetry, impaired binocular summation, reduced ster…

Omics in Keratoconus: From Molecular to Clinical Practice.

Keratoconus (KC) is a progressive ocular disorder marked by structural and functional alterations of the cornea, leading to significant visual impairment. Recent studies indicate that these corneal changes are linked to molecular and cellular mechanisms that disrupt and degrade the extracellular matrix. This degradation is influenced by proteinases that contribute to a loss of homeostasis and an imbalance in the antioxidant/oxidative state within the cornea, fostering oxidative stress, inflammation, and apoptosis. Although these biological processes have been identified primarily through molec…

Economics of corneal cross-linking for keratoconus treatment.

AIM: To optimize the real-world applicability of a health economic model estimating the cost-effectiveness of corneal cross-linking for the treatment of keratoconus in the United States. MATERIALS AND METHODS: A previously reported discrete-event simulation (DES) model was adapted to reassess the cost-effectiveness of corneal cross-linking (CXL) from the US payer perspective. The simulation of keratoconus progression, which is a key driver of model outcomes, was remodeled to more accurately reflect the real-world relationship between age and the rate of disease progression. All costs were upd…

[Effect of keratoplasty on the quality of life of keratoconus patients].

Corneal deformations caused by keratoconus produce high levels of optical aberration (OA). Despite appropriate optical correction, these alter the quality of vision and diminish the patient's quality of life, especially since the affected population is predominantly young and of working age. When thinning is too severe or corneal transparency too impaired, a corneal transplant may be considered. In this study, we compare the quality of life of patients with keratoconus in the early (stages 1 and 2) or advanced (stages 3 and 4) stages of the Krumeich classification, as well as patients who have…

Evaluation of parameters for early detection of pediatric keratoconus.

BACKGROUND: Keratoconus in children is often more severe and rapidly progressive compared to adults. While Scheimpflug imaging (Pentacam) improves screening, there are no established pediatric guidelines. Due to challenges in diagnosis, we aimed to correlate clinical factors with abnormal imaging results to aid in the development of screening protocols. METHODS: A single-center retrospective cohort analysis of patients 3-18 years screened for keratoconus with ophthalmic examination and initial Scheimpflug imaging between 2021 to 2023. Scheimpflug indices including final D, ART max, and calcul…
