The purpose of this study was to evaluate prospectively the arthroscopic findings in the lateral compartment of knees with posterolateral knee complex injuries, to help identify individual injured anatomic structures, and to assist in the clinical identification of these injuries. Thirty of 33 consecutive knees noted to have grade 3 posterolateral knee complex injuries on preoperative evaluations and examinations under anesthesia underwent arthroscopic evaluation concurrently with open reconstruction. The arthroscopic evaluation revealed a significant number of pathologic changes in the latera…

1976 through 1992, 30,003 primary knee arthroplasties and their revisions have been recorded in a nation-wide Swedish study. We report on the structure of the register, demographic data and survivorship. We found that operations for osteoarthrosis (OA) counted for the increase in number of arthroplasties in contrast to rheumatoid arthritis (RA), where the number had slightly declined. For primary operations, the total knee prostheses have practically eliminated other types in RA and are steadily gaining popularity in OA at the expense of the unicompartmental prostheses. Total knee replacements…

One hundred eight ICLH knee arthroplasties with cementless tibial component fixation were performed from 1978 through 1982 in Gothenburg, Sweden. The patients were followed for 2-8 years (mean, 4 years) with clinical and radiographic examinations at regular intervals, and the data were computer-analyzed. Patellar subluxation-dislocation, often accompanied by pain, occurred in 50% of the knees. Eleven arthroplasties failed due to aseptic loosening. Survival rate analysis performed using three different definitions of failure resulted in cumulative success rates after 8 years of 86%, 78%, and 72…

Total condylar tricompartmental total knee replacement was performed in ninety-one patients (117 knees) with seropositive rheumatoid arthritis, all of whom were followed for at least two years postoperatively. Angular deformities of as much as 35 degrees of varus and 20 degrees of valgus angulation were seen preoperatively; postoperatively 85 per cent of the knees had alignment of between zero and 5 degrees of valgus angulation. One hundred and ten knees had less than 5 degrees of varus-valgus laxity on postoperative stress roentgenograms. The range of motion was not statistically increased ex…

This is a clinical research study of 415 cases of high tibial osteotomy in arthrosis of the knee, supported by radiographic and aetiopathogenetic data. It is the largest series with complete documentation so far reported in the literature. It is a critical study based on the clinical findings which marks the beginning of a work intended to evaluate the quality of the results during a period of 5-15 years following surgery (187 knees). The results so far confirm the value of the procedure and emphasize the importance of the fixed flexion factor in the probable pathogenesis of pain. It also show…

Understanding knee mechanics and joint loading is vital for optimizing training and rehabilitation. This study analysed knee axis behaviour and lower limb muscle activity during a closed-chain exercise (Bilateral Squat, BLS), compared with an open-chain exercise (Seated Leg Extension, SLE) using time-series analysis of electromyographic (EMG) data and motion capture to assess knee axis inclination and translations through 90 degrees of flexion-extension. EMG data were filtered and normalized to maximum activity, and kinematic curves were analysed across the full movement cycle (0-100%) employi…

INTRODUCTION: Natural synovial knee joints exhibit very low friction, enabling efficient long-term function. In contrast, total knee arthroplasty (TKA) introduces artificial articulating surfaces with altered kinematic and tribological conditions. However, how in vivo friction evolves across different dynamic activities after TKA remains unclear. The aim of this study was to quantify in vivo knee joint friction after TKA and to investigate its relationship with activity-dependent loading conditions and knee joint kinematics.
METHODS: In vivo joint friction was assessed in three subjects with …

This study presents a novel approach to revising failed medial unicompartmental knee arthroplasty into kinematically aligned total knee arthroplasty using an augmented reality (AR) system (NextAR by Medacta International). Traditional methods involve mechanically aligned total knee arthroplasty (TKA), which can be complex and often requires additional components like stems and augments. The proposed technique builds on the caliper-based manual method by integrating AR-based navigation to enhance precision during surgery, particularly in maintaining the patient's native joint line and soft tiss…

Post-paralytic knee arthropathy (PPKA) is a rare and underrecognized cause of progressive joint degeneration, driven by chronic muscle imbalance that leads to abnormal joint mechanics and loading patterns. We report the first known case of bilateral PPKA following spinal ependymoma resection treated with staged hinged total knee arthroplasty (H-TKA). The patient developed severe, progressive deformities over 18 years with preserved sensation, distinguishing this condition from Charcot arthropathy, which is characterized by rapid joint destruction and profound sensory loss. Staged H-TKA restore…

The authors analyze effectiveness of functional alignment in robot-assisted knee arthroplasty in patients with valgus deformity. The study demonstrates a significant improvement in clinical indicators: KSS score increase from 69.5 to 95 in the knee and from 65 to 94 in functional components, median OKS score 45 and high FJS score (93). Radiological assessment confirmed effective correction of valgus deformity: the HKA angle was corrected from 187° to 181.1°, valgus deformity decreased from 3° to 1°. Functional alignment demonstrated significant advantages compared to mechanical and modified ki…

INTRODUCTION: Extensor mechanism disruption following total knee arthroplasty (TKA) is an uncommon but serious complication. Surgical management is often complex, particularly in cases of failed reconstruction or persistent instability, and may ultimately necessitate amputation.
CASE PRESENTATION: We report a 79-year-old female with a history of TKA and prior patellar tendon repair, who developed a fixed flexion deformity and periprosthetic knee dislocation. Given the failure of conventional interventions and compromised soft tissues, she underwent a through-knee amputation. The femoral compo…

INTRODUCTION: Hop distance tests are commonly used to determine when it is safe to return to sports (RTS), but symmetrical test results do not necessarily indicate the absence of biomechanical deficiencies. Three-dimensional motion analysis may quantify ongoing lower extremity instability following anterior cruciate ligament reconstruction (ACLR).
MATERIALS AND METHODS: We estimated the instantaneous knee rotation axis during the landing phase of the triple hop test. The angular deviation in the orientation of the knee rotation axis between two successive instants was computed and used as a v…

PURPOSE: The infrapatellar fat pad (IFP) is a source of pain in patients with knee osteoarthritis (OA). Changes in pressure due to knee joint movement may increase IFP stiffness and potentially cause pain. This study aimed to investigate changes in IFP stiffness during knee flexion in knee OA patients.
METHODS: Sixteen patients with knee OA and 14 healthy elderly controls participated in this study. IFP stiffness was measured with the patient in the supine position at maximum knee extension, 45°, 90°, and maximum flexion using ultrasound elastography. Stiffness was measured at the superficial…

AIMS: Data on revision of unicompartmental knee arthroplasty (UKA) to total knee arthroplasty (TKA) are limited by low patient numbers and short-term follow-up. This study evaluated the mid-term outcomes of revision of UKA to TKA focusing on implant survival, radiological results, and clinical outcomes in a large series.
METHODS: We identified 235 revisions of UKA to TKA performed between 1995 and 2022 utilizing our institutional total joint registry. The mean age was 65 years (SD 10), mean BMI was 32 kg/m2 (SD 6), and 50% were female (n = 118). The most common indications for revision were a…

INTRODUCTION: Although the surgical techniques and functional outcomes of conventional total knee arthroplasty (TKA) are well-established, there is limited data available on robotic arm-assisted TKA (RATKA) in the context of valgus knee arthroplasty. The purpose of this study is to assess the efficacy of RATKA in the correction of moderate to severe valgus knee deformities using minimally constrained implants and to evaluate the short-term functional outcomes associated with this technique.
METHODS: This prospective study was conducted on patients with moderate to severe grade valgus knee def…

BACKGROUND: Runner's dystonia (RD), a rare task-specific lower-limb dystonia affecting high-mileage runners, presents as abnormal lower-extremity muscle contractions during running. Treatment of RD is challenging and often confounded by significant diagnostic delays due to overlapping symptomatology with other conditions. This case series examines the relationship between stiff knee gait RD and musculoskeletal (MSK) knee pathology.
METHODS: Eight RD cases, evaluated at the NIH Movement Disorders Clinic since 2018, were retrospectively reviewed. Patients underwent neurological, biomechanical, …

The anterior cruciate ligament (ACL)-deficient osteoarthritic knee presents a challenging disease entity, which requires careful thought to restore function and enable return to activities. Advancements in technology and surgical techniques have expanded indications for unicompartmental knee arthroplasty (UKA), to inlcude ACL-deficiency in appropriately-selected patients. An improved understanding of the ACL-deficient osteoarthritic knee can aid in clinical and surgeon decision-making to restore knee function. This review will discuss current practice guidelines for the ACL-deficient knee with…

BACKGROUND: Total knee arthroplasty (TKA) alleviates pain and improves daily living activities in individuals with end-stage osteoarthritis of the knee. However, up to 20% of patients have sub-optimal outcomes after TKA.
OBJECTIVE: No studies have clarified the intraoperative factors that affect postoperative range of motion (ROM) after cruciate-retaining (CR) TKA. Thus, this study aims to clarify these factors.
METHODS: Patients with knee osteoarthritis with varus knee deformity who underwent CR-TKA between May 2019 and December 2020 were included in this study. One year after surgery, pati…

There have been no earlier reports of knee osteoarthritis with valgus knee deformity in which the patellar tendon infiltrates the tibial bone marrow instead of attaching to the tibial tubercle. This case report describes a total knee arthroplasty (TKA) performed for the treatment of a primary knee osteoarthritis resulting from a valgus knee joint position attributed to an abnormality of the patellar ligament attachment. During a TKA, the tendon tissue in the tibial medullary canal interfered with the reamer used to prepare for the stem extensions needed to improve the fixation of the component…
