Combined anterior and posterior cruciate and medial collateral ligament injury: nonsurgical and delayed surgical treatment.

A detailed history, thorough examination, and a high index of suspicion for associated injuries together are the cornerstone for diagnosing and treating a knee with multiple ligament injuries. Acute surgery in patients with combined ligament injuries of the knee can lead to stiffness, primarily with medial-side injuries. Knee dislocations with lateral-side injuries require acute repair to achieve a good long-term result. All knee dislocations should not be grouped together because of the difference in healing potential between medial- and lateral-side injuries. Patients likely prefer a knee th…

Reactivation of rheumatoid arthritis in knees following total knee replacement.

Between May 1985 and December 1990, 20 patients with rheumatoid arthritis had 34 total knee replacements performed. The mean follow-up period was 4.1 years and the range was 2.3 to 7.8 years. Five knees with total knee replacements (74.7%) in 4 patients were affected by activation of the rheumatoid process at an average postoperative interval of 28.6 months while the other 29 knees with total knee replacements (85.3%) remained in remission. Factors that appeared to have an influence on post total knee replacement rheumatoid activation included the activity of the disease preoperatively, and wh

Traumatic dislocations of the patella: problems related to treatment.

Traumatic dislocation of the patella is an infrequent occurrence favored by dysplasia of the extensor apparatus of the knee. The problem of diagnosis is at the basis of a choice of treatment, as any pre-existing dysplasia may be worsened by traumatic dislocation. Furthermore, the presence of a patellar or femoral osteochondral fragment, a more frequent finding than is generally believed as demonstrated by arthroscopy, influences surgical treatment. The results of 37 cases are reported, and several details related to surgical method discussed.

A third-generation, posterior-stabilized knee prosthesis: early results after follow-up of 2 to 6 years.

Two hundred seventy-nine primary total knee arthroplasties were performed with a modular, cemented, third-generation, posterior-stabilized prosthesis. At mean follow-up of 48 months (range, 24-72 months), the outcomes of 238 knees (85%) were evaluated with the Knee Society's Knee and Functional Scoring Systems and Roentgenographic Scoring System. The mean preoperative Knee Society Knee Score was 48 points compared with 96 points at latest follow-up. There were no cases of patellar clunk, symptomatic patellar maltracking, or posterior dislocation. There was no radiographic evidence of loosening

The results of arthroscopic lateral release of the extensor mechanism for recurrent dislocation of the patella after 8 years.

We reviewed 41 knees at a mean of 4 years after arthroscopic lateral release of the extensor mechanism. The operations were performed for recurrent complete dislocation, not for subluxation, maltracking, or lateral pressure syndrome. Thirty-three of these knees were reviewed again 8 years after operation. There were 39% excellent results according to the criteria of Crosby and Insall at 4 years and 30% at 8 years. Patellar stability improved between 4 and 8 years (p < 0.01). The results in patients with subluxation of the patella on extension of the knee or generalized ligament laxity were poo

Arthroscopic evaluation of patellofemoral congruence with rotation of the knee joint and electrical stimulation of the quadriceps.

PURPOSE: The aim of this study was to investigate the pathoanatomic features of patellar instability by arthroscopically comparing patellofemoral congruence with rotation of the knee joint and/or electrical stimulation of the quadriceps (ESQ) between knees with and without patellar instability. METHODS: We retrospectively examined 83 knee joints in 83 patients. The joints were classified into 2 groups: group 1 comprised those without a history of patellar dislocation and included 59 patients (25 male and 34 female patients), and group 2 comprised those with a history of patellar dislocation a

Orthopaedic measurements with computed radiography. Methodological development, accuracy, and radiation dose with special reference to the weight-bearing lower extremity and the dislocating patella.

The overall aim of this study was to develop and evaluate a measurement system for computed radiography (CR) and Picture Archiving and Communication Systems (PACS), permitting measurements of long distances and angles in and between related images. The developed measurement system, which was based on the QUESTOR Precision Radiography (QPR) system, was applied to the weight-bearing knee with special reference to the dislocating patella. The QPR system modified for CR fulfilled the criteria for measuring the weight-bearing knee. The special measuring assistance tools that were developed were imp

Rotating platform spinouts with cruciate-retaining mobile-bearing knees.

Spinout or dislocation is a complication of mobile-bearing total knee arthroplasty. We have observed this complication in 7 of 1255 cruciate-retaining mobile-bearing total knees (0.56%). Patient factors associated with a spinout included female sex, obesity, and preoperative valgus deformity. Operative treatment was generally successful, but significant complications occurred in 3 of the 7 patients.

Effects of medial patellofemoral ligament reconstruction on patellar tracking.

PURPOSE: Medial patellofemoral ligament (MPFL) reconstruction has been performed to treat recurrent patellar dislocation. However, the effects on patellar tracking have not been well documented, particularly in patients. The purpose of this study is to compare patellar tracking pattern and chondral status at MPFL reconstruction with those at second-look arthroscopy. METHODS: Between 1999 and 2008, 71 patients with recurrent patellar dislocation underwent MPFL reconstruction using a double-looped semitendinosus tendon. Of these, 25 knees in 24 patients underwent second-look arthroscopy (at 6-2

A comparison of fixed-bearing and mobile-bearing total knee arthroplasty at a minimum follow-up of 4.5 years.

BACKGROUND: Durable long-term independent results with the Low Contact Stress rotating-platform (mobile-bearing) and the Insall Burstein-II (fixed-bearing) total knee prostheses have been reported, but no studies describing either the mid-term or long-term results and comparing the two prostheses are available, to our knowledge. METHODS: Thirty-two patients who had bilateral arthritis of the knee with similar deformity and preoperative range of motion on both sides and who agreed to have one knee replaced with a mobile-bearing total knee design and the other with a fixed-bearing design were p

A Modified 4-in-1 Stanisavljevic Procedure for Treating Obligatory or Congenital Patellar Dislocations in Children: A Surgical Technique.

INTRODUCTION: Patellar instability and dislocation pose complex clinical and surgical challenges, especially in children. Congenital (fixed) and obligatory (habitual) dislocations present significant anatomical and etiological complexity, frequently leading to deformities and functional impairments, which can range from walking difficulties to sports limitations. Conservative treatment is often inadequate. TECHNIQUE: We describe a surgical technique for treating congenital or obligatory patellar dislocations in patients with various underlying diagnoses-including Down syndrome, nail-patella s

Medial patellofemoral ligament reconstruction using a digital tensiometer to determine graft tension: Surgical technique and mid-term follow-up.

PURPOSE: The optimal method for achieving proper graft tension during patellofemoral ligament reconstruction is a topic of debate. In the past, a digital tensiometer was used to simulate the knee structure, and a tension of approximately 2N was identified as suitable for restoring the patellofemoral track. However, it is unclear whether this tension level is sufficient during the actual surgery. The objective of this study was to verify the efficacy of graft tension using a digital tensiometer for medial patellofemoral ligament (MPFL) reconstruction and to conduct a mid-term follow-up. METHOD

Combined distal femoral osteotomy (DFO) in genu valgum leads to reliable patellar stabilization and an improvement in knee function.

PURPOSE: Valgus deformities of the lower extremity influence patellofemoral joint kinematics. However, studies examining the clinical outcome after treatment of patellar instability and maltracking due to valgus deformity are rare in recent literature. This study's purpose is to analyze the clinical results after combined distal femoral osteotomy (DFO) for treatment of patellar instability. METHODS: From 2010 to 2016, 406 cases of patellofemoral instability and maltracking were treated. Twenty cases of recurring (≥ 2) patellar dislocations with genu valgum and unsuccessful conservative treatm

Medial patellofemoral ligament (MPFL) reconstruction for the treatment of patellofemoral instability.

Patellofemoral instability can be a disabling problem. Numerous techniques are employed for its treatment. Medial patellofemoral ligament (MPFL) reconstruction in combination with other procedures has been explored by many orthopedic surgeons. The purpose of the study was to determine the effectiveness of isolated MPFL reconstruction in treating the symptoms associated with patellofemoral instability, preventing recurrence of patella dislocation, and returning patients to preinjury level of activity. This is a case series study. We conducted a clinical follow-up study on 56 knees (49 patients)
