A case of diode laser photocoagulation in the treatment of choroidal metastasis of breast carcinoma.

To report a single case of improvement on choroidal metastasis of breast cancer after laser photocoagulation. A 52-year-old female patient who complained of visual disturbance of the right eye with multiple states of metastasis of breast carcinoma. On initial examination, the right best-corrected visual acuity was 0.63. Right fundoscopy revealed an elevated mass-like lesion temporal to the macule with serous retinal detachment. The mass had a 3.5-disc diameter. A right fluorescein angiogram revealed hypofluorescence during the prearterial and arteriovenous phase and hyperfluorescence during th

[Approach for the interventional bronchoscopic treatment methods applied in tracheobronchial obstruction].

Bronchoscopy used commonly in the diagnosis and staging of the lung cancer was applied successfully in the palliative treatment of the patients with lung cancer. The patients with tracheobronchial obstruction may develop many symptoms like dyspnea, cough and haemoptysis, and atelectasis and pneumonia but these symptoms were recovered with applying interventional bronchoscopic methods. These methods are laser therapy, electrocautery, argon plasma coagulation, cryotherapy, brachytherapy, photodynamic therapy and airway stents. The obstructed airway was reopened by these bronchoscopic methods and

[Psoriasis vulgaris. An indication for lasers?].

Treatment of psoriasis is often complicated and remains a challenge. Along with the many new immunomodulatory approaches, various laser systems have been employed in an attempt to treat chronic plaque psoriasis. These include CO(2), Er:YAG, pulsed dye, 1302 nm Nd:YAG and excimer lasers. After reviewing and critically evaluating the literature, we conclude that the 308 nm excimer laser has proven its efficacy, not only in comparison with other laser systems, but also when compared with conventional 311 nm narrow-band UVB treatment. It is an efficient and promising therapeutic option for treatin

Volume-Based Radiofrequency Thermocoagulation for Pediatric Insulo-Opercular Epilepsy: A Feasibility Study.

BACKGROUND: Stereotactic ablation surgeries including radiofrequency thermocoagulation (RFTC) and laser interstitial thermal therapy are recent less invasive treatment methods for insular epilepsy. Volume-based RFTC after stereoelectroencephalography was first proposed by a French group as a more effective method for seizure relief in insular epilepsy patients than stereoelectroencephalography-guided RFTC. OBJECTIVE: To describe the feasibility and technical details about volume-based RFTC in patients with insulo-opercular epilepsy. METHODS: We successfully treated 3- and 6-year-old patients

Management of twin-to-twin transfusion syndrome: update and current challenges.

Since a 2004 randomized trial established fetoscopy as the gold standard for the treatment for twin-to-twin transfusion syndrome (TTTS), advancements in surgical technique and expert training have considerably improved survival rates to 89% for at least one fetus and 65% for both fetuses. However, many challenges remain. This review provides an update on the current principles, issues, and debates. The Solomon technique has helped improve neonatal outcomes by reducing the risks of postoperative twin anemia-polycythemia sequence and TTTS recurrence. Nevertheless, fetoscopic laser photocoagulati

Conjunctival burn using micropulse transscleral cyclophotocoagulation.

We report the case of a 37-year-old patient who experienced a conjunctival burn due to overheating of the probe of a micropulse transscleral cyclophotocoagulation device. Fortunately, the outcome was favorable; however, the persistent conjunctival fibrosis may pose future surgical problems for this young glaucoma patient. It is essential to be aware of this rare complication and to take appropriate precautions to prevent it.

Laser Photocoagulation for Ocular Surface Keratinization: A Minimally Invasive Modality of Treatment.

PURPOSE: To report 2 cases of lid margin keratinization (LMK) associated with chronic ocular surface inflammation, which were successfully managed using laser photocoagulation. METHODS: A case report. RESULTS: The first case was a 20-year-old man with mycoplasma-induced rash and mucositis. He presented with a focal patch of lid margin keratinization extending to a small area of inferior tarsal conjunctiva in the right lower eyelid, which was causing keratopathy. He underwent laser photocoagulation but experienced a recurrence after 2 months, which was successfully managed with a repeat laser

Updates in the treatment of neovascular glaucoma: From conventional approaches to novel therapies.

Neovascular glaucoma (NVG) is a severe and refractory form of secondary glaucoma characterized by the proliferation of abnormal vessels in the anterior segment, leading to a progressive rise in intraocular pressure (IOP) and optic nerve damage. It is most commonly associated with ischemic retinal conditions such as diabetic retinopathy, central retinal vein occlusion, and ocular ischemic syndrome. This review aims to provide an updated overview of the current management strategies for NVG, including recent advancements in medical, laser, and surgical treatment modalities. While anti-vascular e

Laser Treatment of Central Serous Chorioretinopathy - An Update.

Laser treatment has been a mainstay for management of central serous chorioretinopathy for a few decades. Different types of lasers have been used and non-damaging retinal laser is the most recent option. The aim of this review is to provide an update on this form of treatment, based on the research published during last 5 years, in comparison with earlier studies published. A MEDLINE database search was performed with a combination of the following terms: central serous chorioretinopathy and laser photocoagulation or subthreshold laser or subthreshold micropulse laser or nanosecond laser or m

[National protocol for diagnosis and care of retinopathy of prematurity: Summary for the attending physician].

Retinopathy of prematurity (ROP, ORPHA: 90050) is the main cause of visual impairment in preterm infants and the leading preventable cause of childhood blindness in high- and middle-income countries. However, severe stages of the disease remain rare. While screening recommendations for the disease are well-established in France, management of ROP requiring treatment is less standardized, especially since new therapeutic options have been approval on this indication. The management of preterm infants requiring treatment for ROP is complex and involves a multidisciplinary team, including pediatr

Neurotrophic Keratopathy in Marfan Syndrome Patient After Micropulse Transscleral Cyclophotocoagulation: A Call for Risk Stratification.

BACKGROUND Micropulse transscleral cyclophotocoagulation is a non-invasive, widely employed procedure that uses diode laser to target the ciliary body to lower the intraocular pressure. Despite its acknowledged efficacy, certain complications are recognized, with neurotrophic keratopathy being a rare yet serious consequence. This report seeks to shed light on a potential high-risk subgroup susceptible to neurotrophic keratopathy, exemplified by our patient with Marfan syndrome, a condition characterized by thin sclera. CASE REPORT Our patient, who was confirmed to have Marfan syndrome with pse

Adult retinopathy of prematurity: treatment implications, long term sequelae, and management.

PURPOSE OF REVIEW: Classically, ROP has been considered a neonatal disease only; however, pediatric ophthalmologists and retinal specialists worldwide are recently facing a new paradigm shift. retinopathy of prematurity (ROP) is now considered a lifelong disease that extends well into adulthood. The purpose of this review is to describe the adult ROP anatomy and discuss the late sequelae and management of this disease. RECENT FINDINGS: Neonatal ROP treatments affect both anterior and posterior segment anatomy. Anterior segment changes secondary to inflammation and posterior ciliary nerve abla

Use of micropulse trans-scleral cyclophotocoagulation for acute rise in intraocular pressure due to anterior segment inflammation.

PURPOSE: The aim of this study was to report the use of micropulse trans-scleral cyclophotocoagulation as an adjunct therapy for two cases of medically uncontrolled intraocular pressure spikes due to anterior segment inflammation.Case description: Case 1 had previous cataract surgery and exhibited an intraocular pressure spike due to phacoantigenic uveitis (right eye intraocular pressure = 52 mmHg). Despite medical treatment, the right eye intraocular pressure remained high (43 mmHg), thus micropulse trans-scleral cyclophotocoagulation was carried out as a rescue therapy. After micropulse tran

Macular hard exudates and scar formation after laser photocoagulation in retinopathy of prematurity.

The authors report the formation of hard exudates and macular scarring after laser photocoagulation therapy in patients with retinopathy of prematurity (ROP). Two premature neonates, the first born at 24 weeks and the second at 25 weeks gestational age, were diagnosed as having ROP that necessitated laser photocoagulation treatment at 32 and 36 weeks, respectively. Subretinal fluid and macular hard exudation developed in both patients that eventually caused bilateral macular scarring. Subretinal macular fluid with hard exudation could lead to macular scar formation in neonates with ROP after l

Recurrent twin-twin transfusion syndrome after selective fetoscopic laser photocoagulation: a systematic review of the literature.

OBJECTIVES: Selective fetoscopic laser photocoagulation (SFLP) is now the treatment of choice for twin-twin transfusion syndrome (TTTS). The incidence of recurrent TTTS following SFLP has been inconsistently reported across different studies. We performed a systematic review of TTTS recurrence following SFLP. METHODS: PubMed and MEDLINE online databases were searched for articles published between 2000 and August 2011, using combinations of the terms 'twin-twin transfusion', 'TTTS', 'laser', 'recur' and 'outcome'. Citations identified in the primary search were screened for eligibility. Studi
