The principles governing the use of anti-inflammatory drugs in rheumatology are reviewed. Non-steroidal anti-inflammatory drugs and corticosteroids, either systemic or local, have a transient effect. On the other hand, treatments with basic medications such as gold salts of D-penicillamine induce prolonged beneficial action. They are, however, useful chiefly in rheumatoid arthritis but not in ankylosing spondylitis or variants, nor in gout or osteoarthrosis. The side effects and contraindications are described and discussed.

The Ebola Virus Disease (EVD) in West Africa has stimulated investments in EVD research. While these research efforts are most welcome, we are concerned about the potential to ignore effective community ethics engagement programmes and critical government regulatory agencies in light of the urgency to conduct clinical trials for EVD therapies and vaccines. We discuss the reasons why community engagement with various research stakeholders is essential, how community engagement should be conducted, and the potential consequences of failing to engage both communities and regulatory agencies by dr

The study explored why actions for early treatment of febrile illnesses in children are delayed by caregivers of children less than five years in five Local Government Areas (LGAs) of southwestern Nigeria using four indicators: caregivers' perception of illness, notion of causation and seriousness, belief in efficacy of selected pathway, and the decision making process. Seven types of febrile illnesses (yellow fever, typhoid fever, ordinary fever/malaria, hot body fever, rain fever, cold fever, and headache fever) were identified and yellow fever was perceived as the most severe type (60.8%). …

A serological survey of primary school children from six schools in Chachoengsao Province, Thailand, was performed at the end of the peak of dengue transmission. GIS analysis of sero-positive cases was carried out to determine transmission foci. Vector control implementation was conducted in the foci and also within 100 meters around the foci in the treated areas by community participation in collaboration with the local government. Vector control strategies included source reduction together with the use of screen covers, a combination of Bacillus thuringiensis subsp. israelensis and Mesocycl…

Echinococcus multilocularis is a fatal zoonotic parasite in the Northern Hemisphere. Recently, it has become endemic in many countries in Asia, especially in the northern island of Hokkaido in Japan. The increasing threat of public health due to alveolar echinococcosis has compelled researches for sensitive diagnosis and effective control. This paper reviews on the epidemiology, diagnosis and control of echinococcosis specifically in Japan. International collaborative responses by researchers and government initiatives such as mandatory reporting system for veterinarians who diagnose echinococ

The decades-long contamination of Gruinard Island by anthrax is now a well-known part of the history of biological weapons (BW) development, as well as that of military encroachments in the Scottish Highlands and Islands (and the authorities' rather less persistent efforts at damage limitation). Some accounts have included the related episode, reportedly well-remembered by local people, of anthrax contamination on the mainland close to Gruinard. This occurred in 1942--43, when BW experiments were conducted on the island as part of the war effort by scientists from Porton Down under the auspice

Risk factors for sporadic cryptosporidiosis were determined in 152 patients and 466 unmatched controls who resided in two local government districts in North Cumbria, North West England, from March 1, 1996, to February 29, 2000. Risk was associated with the usual daily volume of cold unboiled tap water drunk (odds ratio [OR] 1.40, 95% confidence intervals [CI] 1.14 to 1.71 per pint consumed per day [p = 0.001]) and short visits to farms (OR 2.02, 95% CI 1.04 to 3.90, p = 0.04). Fifty-six (84%) of 67 fecal specimens from patients obtained from January 1, 1998, and February 29, 2000, were Crypto…

Smallpox has plagued humans for thousands of years. Although smallpox has been eliminated as an active disease in humans, there is a threat that terrorist organizations or nations could use smallpox as a weapon against the population of the United States. The U.S. government has recognized this threat and has asked the CDC to prepare for such a possibility. A plan to identify, treat, and vaccinate the population in any area of the country where an outbreak of smallpox might occur has been developed and is being implemented. The CDC plan calls for rapid response by local, state, and federal res

The epidemiology of ruminant trypanosomosis was investigated during a two-year period in Kano State, Nigeria. Prevalence was 5.3 +/- 1.3% (mean +/- confidence interval), 1.2 +/- 1.6% and 0.7 +/- 1.3% in cattle, sheep and goats, respectively. Prevalence of bovine trypanosomosis was higher during the second year (6.1%) than in the first (4.8%). Infections doubled during the rains (7.6%) in comparison with an average of 3.8% during the dry season. The northern guinea vegetational zone recorded a high infection rate (Tudun-Wada local government area (LGA), 16.7%). It was the only area in which tse…

During the 8 years of a poliomyelitis control programme in Ilorin Local Government Area, Nigeria, a maximum of 85% of children aged 12-23 months were estimated to have received three doses of trivalent oral polio vaccine (TOPV). The estimated incidence of paralytic poliomyelitis decreased by 65%, suggesting a low vaccine efficacy of approximately 76% for the currently used TOPV. A more detailed study of TOPV efficacy (eg case control or cohort study) is required, however, to provide a more accurate estimate. Similar low-cost efforts at documenting the impact of TOPV on the incidence of poliomy…

A cross-sectional survey conducted in 1986 revealed the occurrence of guineaworm disease in a total of 70 villages in 3 out of the 17 local government areas (L.G.A.) of the State with an average prevalence rate of 56.9%. All villages affected, 39 in Ogoja L.G.A., 23 in Obubra L.G.A. and 8 in Ikom L.G.A., were located in the North Western part of the state. In all areas affected, there were no sources of safe drinking water and 95% of the people obtained their drinking water from ponds and wells with only a negligible number treating the water before drinking. Evidence of spill-over was found i…

The outcome of an intervention of a shigellosis epidemic during April-July 1985 at Dimla, northern Bangladesh is reported. People of the epidemic-affected community operated a makeshift hospital to provide early indoor treatment for 1,708 patients of blood dysentery, based on empirically selected antibiotic(s), oral rehydration of the patients and giving them high-protein diets. They raised funds and received technical and logistic assistance from the physicians and officials of the local government health centre and of the International Centre for Diarrhoeal Disease Research, Bangladesh. Gaza's health system has been devastated by a confluence of political determinants that long predated the 2023-25 Israeli military assault and were dramatically intensified during it. Using historical, political economy, ethical, and health systems lenses, this article argues that settler colonialism, military occupation, and a protracted blockade created chronic shortages, workforce erosion, and institutional fragility, leaving services acutely vulnerable to targeted destruction of facilities and personnel. We examine the role and limitations of international medical deployments and field hos

State and local governments are free to enact infectious disease control laws to protect the public health but must conform those laws to right-based limitations imposed by the United States Constitution. Tuberculosis (TB) control laws that empower public health agencies and their representatives to institute restrictions on movement and activities for persons with TB have remained largely unchanged for decades and warrant review to ensure consistency with modern legal principles. During the coronavirus disease 2019 pandemic, there was increased attention to the tension between nonpharmaceutic…

BACKGROUND AND OBJECTIVES: Peste des Petits Ruminants (PPR) remains a major constraint to small ruminant production in Nigeria, threatening rural livelihood and food security, despite the existence of a national control program. Empirical evidence on farmers' management practices and reasons for low vaccination uptake remains limited. This study aims to explore local management practices, perceptions, and barriers to vaccination among smallholder farmers in Bauchi, Kano and Plateau states.
METHOD: A socioeconomic study was conducted in 52 villages across the three states, with a total of 104
