The surgical care practitioner: a feasible alternative. Results of a prospective 4-year audit at St Mary's Hospital Trust, London.

INTRODUCTION: Surgical care practitioners (SCPs) are an expanding group of professionals, drawn from nursing and the allied health professions. Amongst other functions, SCPs can provide a range of surgical procedures including a 'minor surgical' service. The aim of this study was to audit the volume and outcomes related to the SCP service at St Mary's since its inception. PATIENTS AND METHODS: All prospectively collected data regarding SCP-managed patients between 2001 and 2005 were retrospectively audited. Volume, case mix, waiting times, complications and patient satisfaction were recorded

Males, females and hermaphrodites. An inaugural lecture delivered by Professor Ursula Mittwoch at University College London on 24 October 1985.

Ancient myths of an hermaphrodite origin of man and his subsequent bisection into male and female individuals receive unexpected confirmation from the embryological evidence. The genital ridge contains the components to form either a testis or an ovary, while two sets of genital ducts are the forerunners of the male and female reproductive tracts. The presence of potentially hermaphrodite rudiments in different organisms resulted in the failure of attempts to formulate the determination of sex in terms of classical genetics. This, in turn, has led to the hypothesis that the function of the mam

Clinical findings and serological evidence of HTLV-III infection in homosexual contacts of patients with AIDS and persistent generalised lymphadenopathy in London.

Between 1980 and 1984 28 homosexual men who had had ano-genital intercourse with patients with either acquired immunodeficiency syndrome (AIDS) or persistent generalised lymphadenopathy (PGL) were followed up. The pattern of the sexual links indicated that within this group there were two clusters, one consisting of 7 men and the other of 13.17 of the 28 contacts became ill with either AIDS or PGL; among those in the clusters, 4 died of AIDS and 11 had PGL, and of the rest 2 had PGL. 16 of the 19 men in the clusters who were tested for HTLV-III antibodies were seropositive, as were 7 of those

Trends, disparities, and pregnancy outcomes of gestational diabetes mellitus in North West London: A nine-year population-based cohort study.

BACKGROUND: Gestational diabetes mellitus (GDM) affects approximately 1 in 20 pregnancies in England. North West London (NWL) serves a diverse population of 2.4 million with substantial ethnic and socioeconomic variation. However, population-level epidemiological evidence from NWL remains limited. This study aims to characterise GDM incidence, temporal trends, risk determinants, and associated pregnancy outcomes. METHODS: A retrospective population-based cohort study using routinely collected data from the Whole Systems Integrated Care database included all birth episodes from 2017 to 2025, c…

Large-scale multiethnic electronic health record resource for diabetes complications research: the North West London Diabetes Cohort (NWLDC) - cohort profile.

PURPOSE: The North West London Diabetes Cohort is established to provide systematic characterisation of a large diabetes population as a foundation for complications research and prognostic modelling. Many predictive modelling studies neglect the essential descriptive characterisation of underlying cohorts, focusing narrowly on model accuracy. This cohort profile addresses this gap by comprehensively describing the demographic composition, clinical characteristics and complication incidence patterns. The notably diverse, multiethnic population enables examination of ethnic disparities and supp…

Long-Term Trends in Incidence and Risk Factors for Hemorrhagic Stroke Subtypes Over 24 Years: The South London Stroke Register.

BACKGROUND: Hemorrhagic stroke (HS), including intracerebral and subarachnoid hemorrhage (ICH and SAH), has high mortality and morbidity. We assessed the 24-year trends in incidence, prestroke risk factors, medications, and demographic patterns in a multiracial and multiethnic population. METHODS: We used 1995 to 2018 data from the South London Stroke Register. The annual incidence rates of HS, ICH, and SAH were standardized to the 2011 England and Wales population and stratified by age, sex, and race or ethnicity. Incidence trends were assessed using Cochran-Armitage tests and Poisson regres…

Variation in colon cancer survival for patients living and receiving care in London, 2006-2013: does where you live matter?

BACKGROUND: Marked geographical disparities in survival from colon cancer have been consistently described in England. Similar patterns have been observed within London, almost mimicking a microcosm of the country's survival patterns. This evidence has suggested that the area of residence plays an important role in the survival from cancer. METHODS: We analysed the survival from colon cancer of patients diagnosed in 2006-2013, in a pre-pandemic period, living in London at their diagnosis and received care in a London hospital. We examined the patterns of patient pathways between the area of r…

Prominent amyloid plaque pathology and cerebral amyloid angiopathy in APP V717I (London) carrier - phenotypic variability in autosomal dominant Alzheimer's disease.

The discovery of mutations associated with familial forms of Alzheimer's disease (AD), has brought imperative insights into basic mechanisms of disease pathogenesis and progression and has allowed researchers to create animal models that assist in the elucidation of the molecular pathways and development of therapeutic interventions. Position 717 in the amyloid precursor protein (APP) is a hotspot for mutations associated with autosomal dominant AD (ADAD) and the valine to isoleucine amino acid substitution (V717I) at this position was among the first ADAD mutations identified, spearheading th

Incidence and treatment costs of severe bacterial infections among people who inject heroin: A cohort study in South London, England.

BACKGROUND: People who inject drugs often get bacterial infections. Few longitudinal studies have reported the incidence and treatment costs of these infections. METHODS: For a cohort of 2335 people who inject heroin entering treatment for drug dependence between 2006 and 2017 in London, England, we reported the rates of hospitalisation or death with primary causes of cutaneous abscess, cellulitis, phlebitis, septicaemia, osteomyelitis, septic arthritis, endocarditis, or necrotising fasciitis. We compared these rates to the general population. We also used NHS reference costs to calculate the…

Case Report: Old World Mucosal Leishmaniasis: Report of Five Imported Cases to the Hospital for Tropical Diseases, London, United Kingdom.

Old World species of Leishmania typically cause visceral and cutaneous leishmaniasis. Mucosal involvement is typically seen with infection by Leishmania species found in South America, usually after the healing of cutaneous leishmaniasis. We present five imported cases of mucosal leishmaniasis caused by Old World Mediterranean Leishmania infantum exclusively affecting the nasal mucosa or vocal cord. In only one case was there a recollection of a preceding cutaneous lesion compatible with cutaneous Leishmaniasis. Of significance was that four out of five cases were receiving local corticosteroi

Impact of an 18-month, NHS-based, treatment exposure for heroin dependence: results from the London Area Treat 2000 Study.

We set out to examine the impact of treatment for heroin dependence on drug use, injecting behavior, health problems, criminality, and physical and mental health over 18 months among heroin-dependent Londoners. A total of 100 heroin users were recruited for this longitudinal prospective cohort study with repeated measures (T0 as baseline, T1 after 9 months, and T2 after 18 months). The psychiatric evaluation and assessment of drug abuse levels were determined by the CIDI and the EuropASI. Additional evaluations included the WHO-DAS II for disability assessment and the UCLA-SSI for social suppo…

High-risk sexual behaviour increases among London gay men between 1998 and 2001: what is the role of HIV optimism?

OBJECTIVE: To examine whether HIV optimism (i.e. optimism in the light of new HIV drug therapies) can account for the recent increase in high-risk sexual behaviour among London gay men. METHODS: Gay men (n = 2938) using London gyms were surveyed annually between 1998 and 2001. Information was collected on HIV status, unprotected anal intercourse (UAI) in the previous 3 months, and agreement with two statements concerning the severity of and susceptibility to HIV infection. Those who agreed were classified as 'optimistic'. RESULTS: Between 1998 and 2001, the percentage of men reporting high-r…

Oral and intravenous itraconazole for systemic fungal infections in neutropenic haematological patients: meeting report. London, United Kingdom, 20 June 1998.

Effective prevention, or treatment, of invasive fungal infection in the neutropenic patient has hitherto been unsatisfactory because of either an inadequate anti-fungal spectrum of the agent or important toxicity. Itraconazole is effective against a broad spectrum of the opportunistic pathogens seen in Europe and North America. Prior problems with absorption, e.g. in the marrow transplant recipient, have been overcome with the introduction of an oral solution and an i.v. preparation. The deliberations of an expert meeting held in June, 1998 include recommendations on which patient requires one
