Reinventing the wheel: a modern perspective on the bilateral inverted 'L' osteotomy.

The bird-face deformity of a severe class II, high Frankfort-mandibular plane angle with significant retrogenia, often associated with diminutive condyles and reduced posterior face height, poses many challenges to the orthognathic surgeon. Of greatest concern in these patients is the degree of mandibular advancement required and the potential for relapse. The sagittal split osteotomy is the workhorse of mandibular surgery but does not allow significant lengthening of the ramus, which is desirable in this group of patients. An inverted 'L' osteotomy of the mandible to facilitate ramus lengthen

The Class II Division 2 craniofacial type is associated with numerous congenital tooth anomalies.

The aim of the present study was to examine whether a putative relationship exists between the Class II division 2 craniofacial type and congenital anomalies of the dentition, such as missing teeth, peg-shaped laterals, transpositions, supernumerary teeth and canine impactions. Two hundred and sixty-seven untreated patients with Class II division 2 malocclusion were examined. The results show that 56.6 per cent of the patients exhibited some form of congenital tooth anomaly, 13.9 per cent agenesis of the upper lateral incisors, 7.5 per cent peg-shaped upper laterals, while impacted canines wer

A solitary maxillary central incisor treated orthodontically: a case report.

The presentation of a solitary maxillary central incisor is reported and its association with other congenital abnormalities discussed. It is an important finding for dental health professionals, since it may indicate the presence of other significant midline congenital abnormalities. It may also indicate the presence of associated disorders that profoundly affect growth and development and which, once identified, may be treated.

Changes in functional occlusion during the postorthodontic retention period: a prospective longitudinal clinical study.

INTRODUCTION: Functional occlusion has received little attention in short- and long-term evaluations of orthodontic treatment results. Thus, the purpose of this study was to determine the long-term changes in functional occlusion during the postorthodontic retention period. METHODS: One hundred fifteen consecutive patients treated with multibracket appliances were examined twice: directly after appliance removal (T1) and after an average retention period of 24 months (T2). The clinical examination comprised the assessment of anteroposterior and lateral interferences (slides) between the retru

Nonsurgical treatment of an adult with a skeletal Class II gummy smile using zygomatic temporary anchorage devices and improved superelastic nickel-titanium alloy wires.

Patients with a severe gummy smile and a skeletal Class II profile are difficult to treat. This case report describes an effective treatment alternative for improving a gummy smile in a patient with a severe Class II molar relationship, severe crowding, and lip protrusion using zygomatic anchorage devices and improved superelastic nickel-titanium wires. A 36-year-old woman had an excessive overjet and a deep overbite with a bilateral Angle Class II molar relationship. The cephalometric analysis demonstrated a Class II skeletal relationship (ANB, 9.5°), retroclination of the mandible (FMA, 38.4

Quality of occlusal outcome in adult Class II patients treated with completely customized lingual appliances and Class II elastics compared to adult Class I patients.

OBJECTIVES: The aim of this investigation was to evaluate whether Class II malocclusion in adult patients can be successfully corrected using a completely customized lingual appliance (CCLA) in combination with Class II elastics. METHODS: In order to detect differences in the final treatment outcome, two groups were matched for age and gender. Treatment results of 40 adult orthodontic patients with a Class I malocclusion (Group 1) were compared to 40 adults with a Class II malocclusion (Group 2). All patients had completed treatment with a CCLA (WIN, DW Lingual Systems, Bad Essen, Germany) wi

Effect of dentofacial deformity on maximum isometric tongue strength.

The objective of this study was to determine whether dentofacial deformities (class II and class III) influence maximum isometric tongue strength compared with a group without deformities. A total of 126 adult patients participated in the study. Of these, 45 had a class II diagnosis (14 men and 31 women) and 81 a class III diagnosis (35 men and 46 women), all of them with indication of orthognathic surgery. Fifty adult volunteers (17 men and 33 women) with no changes in dental occlusion (class I) and with no clinical signs of dysfunction of the temporomandibular joint represented the control g

Cranial base angulation in Brazilian patients seeking orthodontic treatment.

The relationship between cranial base flexion and facial prognathism has been of interest to orthodontists, orthognathic maxillofacial surgeons, and plastic surgeons. This study aimed to identify the cranial base flexion of adults with anteroposterior skeletal disharmony. Because the sample was representing a mixed racial population, the authors performed an investigation to confirm an association with other researches performed in different parts of the world. The results revealed an increase in the cranial base flexion in cases diagnosed as class II skeletal pattern and reduction in the grou

Lower molar and incisor displacement associated with mandibular remodeling.

The purpose of this study was to quantify the amount of alveolar modeling at the apices of the mandibular incisor and first molar specifically associated with appositional and resorptive changes on the lower border of the mandible during growth and treatment. Cephalometric data from superimpositions on anterior cranial base, mandibular implants of the Björk type, and anatomical "best fit" of mandibular border structures were integrated using a recently developed strategy, which is described. Data were available at annual intervals between 8.5 and 15.5 years for a previously described sample of
