Third window vibroplasty: an alternative in surgical treatment of tympanosclerotic obliteration of the oval and round window niche.

OBJECTIVE: To describe a new technique in surgical treatment of obliterative tympanosclerosis by applying a floating mass transducer (FMT) to a third window. PATIENT: A 64-year-old woman with a severe combined hearing loss due to tympanosclerosis received a third window vibroplasty. INTERVENTION: A mastoidectomy and a posterior tympanotomy via the large facial recess were performed. The promontory was exposed by a transcanal approach. The third window was performed anterior inferior to the round window. The membranous cochlea was left intact. The FMT was gently pushed into the perichondrium-…

[Fitness to dive in the otorhinolaryngological field].

In line with the rising number of recreational divers, the otorhinolaryngologist has to deal with growing numbers of diving-associated disor … r amplifiers or cochlear implants, after sinus or scull base surgery and even after canal wall down mastoidectomy, provided that certain requirements are fulfilled. Assessing fitness to dive after inner ear barotrauma as well as after inner ear decompression illness requires meticulous consideration of residual damage and possible underlying conditions like vas …

Ossiculoplasty by interposition of a minor columella between the tympanic membrane and stapes head.

This study analyzes short- and long-term results of 436 ossiculoplasties accomplished by a minor columella sculptured in the remnants of the incus and placed between the tympanic membrane and stapes head. Most cases were operated on by a transcanal approach through an ear speculum, under local anesthesia. Cases in which an associated mastoidectomy was performed to remove diseased tissue are excluded. The residual air-bone gap was < 20 dB in 86.1% of cases 2 months after surgery, in 77.0% 1 year later, and then remained stable over time, even in patients seen 5 to 15 years after the operation. …

A case of acquired petrous cholesteatoma associated with insidious middle ear infection treated by staging the surgical procedures.

Surgical treatment of petrous cholesteatoma is difficult, especially in infected cases, since it often involves vital structures. We report … atment was not effective in eradicating the otorrhea. Four weeks after the first operation (radical mastoidectomy), the second operation was conducted following a combined middle cranial fossa and transmastoid approach. The postoperative course was uneventful. Normal facial nerve function was preserved and unsteadiness disappeared, but hearing could not be pres …

Placebo effect in surgery for Meniere's disease: a three-year follow-up study of patients in a double blind placebo controlled study on endolymphatic sac shunt surgery.

In a previously published double blind, placebo controlled study, the efficacy of an endolymphatic sac-mastoid shunt was compared with a purely placebo operation (mastoidectomy) in controlling the symptoms in 30 patients with typical Meniere's disease. Minor differences could be demonstrated after one year between patients with the shunt versus the sham operation, but the greatest difference was between the pre- and postoperative scores, and both groups improved significantly. It was concluded that the impact of the various endolymphatic sac shunts upon the symptoms in patients with Meniere's …

Ménière's disease: endolymphatic sac decompression compared with sham (placebo) decompression.

The placebo effect in surgery for Ménière's disease was investigated in a double-blind, controlled surgery by comparing the effect of a regular endolymphatic shunt with the effect of a purely placebo operation (regular mastoidectomy). Thirty patients with typical Ménière's disease participated in the study. They were selected for surgery because of unsuccessful medical treatment and were chosen randomly for each treatment group. The patients filled in daily dizziness questionnaires for 3 months before and 12 months after surgery, registering nausea, vomiting, vertigo, tinnitus, hearing impairm…

Combined Heermann and Tos (CHAT) technique in cholesteatoma surgery: surgical technique and preliminary results.

The combined Heermann and Tos (CHAT) technique is the combination of Heermann's 'cartilage palisade tympanoplasty' and Tos's 'modified combined approach tympanoplasty = modified intact canal wall mastoidectomy'. The first author (Cem Uzun) performed the CHAT technique as a one-stage operation in 15 ears of 15 patients with cholesteatoma. Two patients (one with a follow up of less than six months and one who did not show up at the final re-evaluation) were excluded from the study. Median age in the remaining 13 patients was 37 years (range: 14-57 years). Cholesteatoma type was attic, sinus (Tos…

Homograft tympanoplasty in perspective. A long-term clinical-histologic study of formalin-fixed tympanic membranes used for the reconstruction of 125 severely damaged middle ears.

This seven and a half year clinical-histologic study evaluates the effectiveness of buffered, formaldehyde-fixed homograft tympanic membranes for reconstructing the severely damaged middle ear in 125 consecutive patients. Indications for use of homograft tympanoplasty were limited to those cases in which standard tympanoplasty had already failed to produce a satisfactory hearing or anatomic result (i.e., recurrent perforations or draining radical mastoidectomy cavity), or to those cases in which there was a high risk of unsatisfactory result with standard tympanoplasty techniques (i.e., total …

Comparison of microscopic and 3D exoscopic canal wall down tympanomastoidectomy for primary middle ear cholesteatoma.

BACKGROUND: The three-dimensional (3D) exoscope is an emerging alternative to the operating microscope (OM) in otologic surgery. AIMS: To assess its clinical utility in cholesteatoma surgery. METHODS: Retrospective review of 36 primary cases (21 OM, 15 exoscope) comparing setup time, surgical duration, audiological outcomes, and recurrence. RESULTS: Groups were comparable in background and stage. Setup time was shorter with exoscope (microscope: 20 min, range 18-22, exoscope: 17 min, range 14-20) (p = .002), while other outcomes showed no significant difference. CONCLUSIONS: The 3D exoscop

Microsurgical Resection of Petroclival Meningioma via the Posterior Petrosal Approach: Three-Dimensional Operative Video.

Petroclival meningiomas are complex, deep-seated lesions related to many critical neurovascular structures. We present the case of a 44-year-old woman who had presented with a history of severe facial pain, hearing loss, and tinnitus on the left side, associated with left facial hypoesthesia (Video 1). Preoperative magnetic resonance imaging demonstrated a mass highly suggestive of a left petroclival meningioma. Considering the worsening symptoms and important mass effect, microsurgical resection using the posterior petrosal approach was performed. Mastoidectomy was performed first, followed b…

Cholesteatoma disease recidivism after canal wall up tympanomastoidectomy with or without obliteration (CLEAR-EAR): A protocol for a randomized controlled trial.

BACKGROUND AND RATIONALE: Cholesteatoma treatment relies on surgery with disease removal as the primary goal. While canal wall down (CWD) and canal wall up (CWU) tympanomastoidectomy remain standard approaches, mastoid obliteration has gained popularity. Canal wall up preserves ear canal anatomy and adding the obliteration technique suggests lower recurrence and residual rates. However, controversy persists and surgical consensus is lacking. Current decision-making relies on personal experience and patient-related factors rather than high-quality evidence. This study therefore investigates whe…

Mastoid Surgery Does Not Normalize Tympanometric Middle Ear Pressure in Children With Cholesteatoma.

OBJECTIVE: To determine whether transcanal surgery without mastoidectomy, canal wall up (CWU), canal wall down (CWD), or CWU mastoid obliteration (MO) surgery for pediatric cholesteatoma has a more favorable effect on middle ear pressure (MEP) homeostasis. METHODS: Data from children having mastoid surgery for acquired cholesteatoma were collected prospectively. Tympanometric values of MEP were compared after transcanal, CWU, CWD, and MO surgery analyzing (i) a single measure per ear from last clinic visit, and (ii) linear mixed-effects modeling (LMEM) to control for multiple measures, surger…

Non-Hodgkin Lymphoma Mimicking Vestibular Schwannoma.

Progressive unilateral hearing loss and an MRI are usually enough to diagnose vestibular schwannoma (VS). We were consulted by a 45-year-old man with otalgia and left-sided hearing loss as well as ipsilateral facial paralysis that had begun two weeks prior. Due to a possible atypical presentation of VS, an MRI was ordered, which revealed an intracanalicular lesion occupying the left cerebellopontine angle cistern. With no signs of systemic disease and considering the total left ear deafness, the patient underwent retrolabyrinthine mastoidectomy. During the procedure, a mass incompatible with V…
