A 48-year-old woman with a severely atrophied maxilla was treated with open sinus augmentation surgery along with Le Fort I osteotomy with a pedicled buccal fat pad graft to position the maxilla in a right occlusal plane with respect to the mandible and to construct adequate bone volume allowing proper implant placement. Six dental implants were inserted in the maxilla, and a fixed metal-resin screw-retained prosthesis was fabricated for the maxilla and mandible.

BACKGROUND: A one-stage surgical technique has been adopted that allows simultaneous implant placement in as little as 1 to 2 mm of residual bone. The aim of this retrospective study was to describe results and complications in sinus augmentation of severely atrophic edentulous maxillae in patients with 1 to 3 mm residual bone height and compare them to a group of patients treated following the standard protocol with residual bone height > 4 mm.
METHODS: The study consisted of a study group (N = 25) and a control group (N = 30). All patients were consecutive admissions treated for one-stage s…

During a 12-year period (1984-1996), 118 maxillary inlay autogenous bone grafts and 248 commercially pure titanium threaded root-form endosseous implants were placed in 54 consecutively treated patients with compromised maxillary bone. In this retrospective clinical study, 3 groups of patients were reviewed, group selection being based on anatomic location and surgical access to the recipient site. Group 1 included patients with bone grafts placed in the antrum floor via an intraoral antrostomy exposure, group 2 included patients with bone grafts placed in the nasal floor via an anterior intra…

In the severely resorbed maxilla, a 10-year success rate of only 49-74% of implants in combination with autogenous bone grafts has been reported. We developed a modified technique of antral inlay grafting and lateral and vertical onlay grafting of the severely resorbed maxilla for inserting implants to retain dentures. The clinical and radiologic results are presented. In 21 patients with severely resorbed edentulous maxillae, a total of 20 bilateral and one unilateral antral inlay graftings and lateral and vertical onlay graftings were performed after a prosthodontic setup. We opened the maxi…

Treatment of the atrophic maxilla using an onlay bone-grafting technique in combination with simultaneous insertion of endosteal implants (Branemark System) was performed in 17 patients. They were followed during a 3-year period. At the end of this period, 14 patients had stable bridges in use and one patient had an overdenture. Two patients had had to return to conventional dentures due to implant losses. The survival rate of the implants after 3 years use was 75%. It is concluded that, in spite of the implant losses, onlay bone grafting to the maxilla is a valuable method in cases of severe …

Changes in dental and skeletal relationships were evaluated 5 years postsurgically in 49 patients whose maxilla had been superiorly repositioned by a Le Fort I osteotomy. All of the patients had at least 2 mm of intrusion at the maxillary incisor and molar; none had a mandibular ramus osteotomy or other osteotomy except genioplasty. Only 6.5% had 2 mm or greater of net vertical change in skeletal or dental landmarks at 1 year postsurgery. From 1 to 5 years postsurgery, minimal changes in skeletal and dental landmarks occurred in the majority of the patients, but approximately 25% of the patien…

The accuracy of full-arch implant impressions is critical for prosthetic passivity and long-term clinical success. This study compared the trueness and repeatability of twelve digital impression systems (DIS), including nine intraoral scanners (IOS) and three stereophotogrammetric (SPG) systems, for a fully edentulous maxilla restored with eight multi-unit abutments (MUAs). Accuracy was assessed using a coordinate measuring machine-derived reference model and a clinically driven 'worst-MUA' approach, considering the suprastructure as a single functional unit. Trueness and repeatability were ev…

It remains uncertain whether replacing posterior zygomatic implants with pterygoid implants reduces the stress exerted on anterior zygomatic implants in the canine region. This three-dimensional finite element analysis (FEA) study aims to compare stress and strain patterns between two implant configurations under the influence of masticatory muscle forces. A 3D (FEM) model of an edentulous, atrophic maxilla was developed using data from a cone-beam computed tomography (CBCT) scan. Two configurations were examined: Model A, which included bilateral zygomatic implants, and Model B, featuring one…

BACKGROUND AND PURPOSE: Despite their vulnerability to radiation-induced damage, the maxilla and dentition are not routinely contoured in head and neck (HN) radiotherapy (RT), preventing accurate dose assessment and establishment of dose-effect relationships. This literature review aimed to: (1) identify published dentofacial contouring approaches, (2) synthesise current methods, and (3) where feasible, compare practices in paediatric and adult RT.
MATERIALS AND METHODS: MEDLINE and Embase were searched for studies that contoured the maxilla, mandible, or teeth in HN RT. Reference lists were …

This meta-analysis compares the accuracy of mandible-first and maxilla-first approaches in bimaxillary orthognathic surgery to improve clinical decision-making. A systematic search was performed in PubMed, Web of Science, Embase, and Cochrane databases up to August 2024. The analysis included randomized controlled trials and cohort studies with a minimum of 10 patients. Data extraction followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, and pooled effect estimates for continuous outcomes were calculated using weighted mean difference (WMD) with 95% CIs. Hete…

PURPOSE: This study examined the effect of assistive device use on the precision of digital impressions for multiple implants placed in a fully edentulous maxilla in vivo.
METHODS: A total of eight participants with fully edentulous maxillae and four implants at position #15, #12, #22, and #25 were included in the study. The assistive device was made using CAD/CAM technology. Digital impressions were obtained using an intraoral scanner with (AD+) and without (AD-) the assistive device that attached to the scan bodies. Each participant underwent five digital and conventional impressions (verif…

This dental technique demonstrates an approach to integrating an intra-oral scan and a poly vinyl siloxane impression of the functional sulcus for a completely edentulous maxilla. The intra-oral scan and polyvinyl siloxane impression are aligned in the oral cavity via the design and subsequent manufacture of a custom impression tray with a large window. This technique allows precise alignment of the two scans, whereas other proposed methods rely on alignment ex vivo using third-party software which may introduce errors. The technique explained here can be applied in a variety of clinical scena…

BACKGROUND: This clinical study presents a novel technique for basal bone anchorage, a surgical approach designed for immediate loading in the atrophic maxilla, specifically in Cawood Class VI cases. By leveraging the structural integrity of basal bone, this method eliminates the need for invasive bone grafting procedures, reducing patient morbidity, shortening treatment timelines, and enhancing overall patient acceptance.
METHODS: A single-arm clinical investigation, involving a carefully selected cohort of seven patients, assessed the feasibility, functional outcomes, and implant stability …

This study constitutes a retrospective analysis, with the objective of evaluating the long-term clinical outcomes of custom-made DMLS maxillary subperiosteal implants in patients diagnosed with severe bone atrophy over a follow-up period of up to 10 years. A total of ten patients with severe maxillary bone loss, classified as Cawood and Howell Class IV to VI, who were unsuitable for conventional dental implant treatment, were treated with subperiosteal implants between 2015 and 2025. The implants were designed on the basis of computed tomography (CT) imaging data using CAD/CAM software and the…

Osteoglophonic dysplasia (OD) is a rare skeletal disorder marked by craniosynostosis, craniofacial anomalies, and abnormal bone development, primarily caused by FGFR gene mutations. We present the case of a 26-year-old Chinese female, who has been followed since age 11 for recurrent central giant cell granulomas (CGCG) and non-ossifying fibroma affecting her jaws and lower limbs. While her craniofacial and limb features have been described in two earlier publications, this report provides a detailed account of the aggressive and recurrent maxillary CGCG that resulted in significant clinical mo…

BACKGROUND: The standard screening protocol for radiographic examination in dentistry as per the American Dental Association recommendations is a panoramic radiograph (PAN) and four horizontal bitewings. PAN inherently suffers from several shortcomings like the superimposition of anatomic structures, especially of the cervical spine that obscures a significant portion of the anterior maxilla and mandible. This region has a significant amount of pathology that is not adequately imaged. Three-dimensional (3D) imaging provides circumferential information on the area of interest and adds value to …
