Patient-specific 3D printed guides for repairing posterior root tears of the medial meniscus: A cadaveric study.

OBJECTIVES: The aim of this study was to evaluate the accuracy of tibial tunnel placement using patient-specific three-dimensional (3D)-printed guides for repairing medial meniscus posterior root tears designed from magnetic resonance imaging (MRI) scans. MATERIALS AND METHODS: Five right fresh-frozen cadaveric knees underwent MRI scanning with 1-mm isotropic slices. For each specimen, custom guides were created to generate two transosseous tibial tunnels, with design based on anatomical landmarks including the medial tibial eminence (MTE) and the posterior cruciate ligament (PCL) insertion. …

High precision measurement of meniscus profile on liquid column in cylindrical capillary based on telecentric imaging technology.

The precise characterization of capillary meniscus has been a measurement bottleneck in interface science for three centuries. The accuracy of traditional measurement methods is not only limited by perspective errors and refractive distortions, but also lacks effective precision closed-loop verification mechanisms. In response to this challenge, this paper applies the telecentric imaging technology to the detection of the meniscus profile, which avoids the perspective error of the traditional imaging system, so that the absolute accurate correction function of the meniscus profile in the cylin…

Impact of time between meniscal injury and isolated meniscus repair on post-operative outcomes: A systematic review.

BACKGROUND: Meniscal tears are one of the most common knee injuries. After the diagnosis of a meniscal tear has been made, there are several factors physicians use to guide clinical decision-making. The influence of time between injury and isolated meniscus repair on patient outcomes is not well described. Assessing this relationship is important as it may influence clinical decision-making and can add to the preoperative patient education process. We hypothesized that increasing the time from injury to meniscus surgery would worsen postoperative outcomes. AIM: To investigate the current lite…

All-Inside Arthroscopic Meniscus Repair for Patients Over 40 Years of Age: Is Forty the New Twenty?

INTRODUCTION: Meniscal injuries are a common challenge in orthopedic surgery. For decades, partial or total meniscectomy has been the primary surgical treatment for meniscal tears. In recent years, the increased recognition of menisci's biomechanical importance has progressively shifted the paradigm towards meniscus repair. However, meniscus-sparing surgery remains the treatment of choice for selected lesions in young and active patients, especially for young and active patients. In this study, we evaluated the effectiveness of all-inside sutures in treating meniscus tears in patients over 40.…

Anterior meniscus extrusion is associated with anterior tibial osteophyte width in knee osteoarthritis - The Bunkyo Health Study.

BACKGROUND: In knee osteoarthritis (OA), medial meniscus extrudes both medially and anteriorly. We reported that full-length width of medial tibial osteophyte, which comprises cartilage and bone parts, is directly associated with medial meniscus extrusion in early-stage knee OA and hypothesized that anterior tibial osteophyte (ATO) is also associated with anterior meniscus extrusion (AME). Thus, we aimed to examine their prevalence and relationship. METHODS: Elderly subjects (638 females and 507 males; average 72.9 years old) in the Bunkyo Health Study cohort were enrolled. MRI-detected OA ch…

Evaluation of knee bone morphology in juvenile patients with complete discoid lateral meniscus using magnetic resonance imaging.

PURPOSE: The characteristic two-dimensional bone morphology in patients with a discoid lateral meniscus (DLM) has been described. However, the associated three-dimensional imaging findings have not been characterized. This study was performed to identify differences in the knee bone morphology between juvenile patients with a DLM and those with a normal meniscus using magnetic resonance (MR) imaging. METHODS: The DLM group comprised 33 consecutive juvenile patients (33 knees) with a complete DLM, and the control group comprised 24 juvenile patients (24 knees) with normal menisci on the basis …

Injectable ECM hydrogel for delivery of BMSCs enabled full-thickness meniscus repair in an orthotopic rat model.

Meniscal injuries have poor intrinsic healing capability and are associated with the development of osteoarthritis. Decellularized meniscus extracellular matrix (mECM) has been suggested to be efficacious for the repair of meniscus defect. However, main efforts to date have been focused on the concentration, crosslinking density and anatomical region dependence of the mECM hydrogels on regulation of proliferation and differentiation of adult mesenchymal stem cells (MSCs) in vitro 2D or 3D culture. A systematic investigation and understanding of the effect of mECM on encapsulated MSCs response …

FOXO1 and FOXO3 transcription factors have unique functions in meniscus development and homeostasis during aging and osteoarthritis.

The objective of this study was to examine FoxO expression and FoxO function in meniscus. In menisci from human knee joints with osteoarthritis (OA), FoxO1 and 3 expression were significantly reduced compared with normal menisci from young and old normal donors. The expression of FoxO1 and 3 was also significantly reduced in mouse menisci during aging and OA induced by surgical meniscus destabilization or mechanical overuse. Deletion of FoxO1 and combined FoxO1, 3, and 4 deletions induced abnormal postnatal meniscus development in mice and these mutant mice spontaneously displayed meniscus pat…

A novel technique for modified all-inside repair of bucket-handle meniscus tears using standard arthroscopic portals.

BACKGROUND: Bucket-handle meniscus tears (BHMT) are often displaced and unstable. The inside-out technique of repairing such tears is currently the gold standard. All-inside repair with meniscal fixators is getting increasingly popular. Shortcomings of the inside-out technique include neurovascular complications, especially saphenous nerve palsy, and retention of a non-resorbable suture which can result in discomfort to patient, granuloma formation, and a foci of infection. Hence, the purpose of this project was to innovate a novel all-inside technique to precisely reduce and fix BHMT while av…

Transient Common Peroneal and Tibial Nerve Palsy Following Knee Arthroscopy for the Treatment of Discoid Lateral Meniscus.

The number of knee arthroscopies has increased rapidly since the 1980s and are among the most common orthopedic procedures today. It is generally considered a minimally invasive surgery with relatively low morbidity, but it is not without risk of complications. We report the case of a 14-year-old boy who developed a lesion of common peroneal nerve and tibial nerve after knee arthroscopy for a torn discoid lateral meniscus, which was confirmed by electromyogram study.

Abnormal Mechanical Loading Induces Cartilage Degeneration by Accelerating Meniscus Hypertrophy and Mineralization After ACL Injuries In Vivo.

BACKGROUND: Although patients with an anterior cruciate ligament (ACL) injury have a high risk of developing posttraumatic osteoarthritis (PTOA), the role of meniscus hypertrophy and mineralization in PTOA after an ACL injury remains unknown. PURPOSE/HYPOTHESIS: The purpose of this study was to determine if menisci respond to abnormal loading and if an ACL injury results in meniscus hypertrophy and calcification. The hypotheses were that (1) abnormal mechanical loading after an ACL injury induces meniscus hypertrophy and mineralization, which correlates to articular cartilage damage in vivo, …

Osteoarthritic changes in vervet monkey knees correlate with meniscus degradation and increased matrix metalloproteinase and cytokine secretion.

OBJECTIVE: Meniscus injury increases osteoarthritis risk but its pathobiology in osteoarthritis is unclear. We hypothesized that older adult vervet monkeys would exhibit knee osteoarthritic changes and the degenerative menisci from these animals would secrete matrix metalloproteinases (MMPs) and pro-inflammatory cytokines that contribute to the development of osteoarthritis. DESIGN: In a cross sectional analysis of healthy young adult (9-12 years) and old (19-26 years) adult female vervet monkeys, knees were evaluated in vivo with computed tomography (CT) imaging, and joint tissues were morph…

Do cartilage repair procedures prevent degenerative meniscus changes?: longitudinal t1ρ and morphological evaluation with 3.0-T MRI.

BACKGROUND: Cartilage repair (CR) procedures are widely accepted for treatment of isolated cartilage defects in the knee joint. However, it is not well known whether these procedures prevent degenerative joint disease. HYPOTHESIS: Cartilage repair procedures prevent accelerated qualitative and quantitative progression of meniscus degeneration in individuals with focal cartilage defects. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: Ninety-four subjects were studied. Cartilage repair procedures were performed on 34 patients (osteochondral transplantation, n = 16; microfracture, …

Long-term results of meniscus repair and meniscectomy: a 13-year functional and radiographic follow-up study.

This study investigated the long-term outcome of common meniscus treatment (meniscectomy, repair). A consecutive series of 30 patients with open meniscus repair were compared retrospectively to 30 patients who had an arthroscopic partial or subtotal meniscectomy. The groups were matched according to sex, age, meniscus lesion, and follow-up time. The patients were aged 13-43 years at the time of operation; all had intact cruciate ligaments, and none had had previous surgery on the knee. Patients were reexamined at a mean of 13 years after the operation. In addition, for a subgroup of 22 matched…

Preparation of a novel acellular vascular matrix scaffold and its repair effect on medial meniscus defects in rabbits.

Research on the repair and regeneration of injured menisci has remained a focal point of investigation. This study explored the preparation method of a novel acellular vascular matrix (AVM) scaffold and analyzed and evaluated its physicochemical properties as well as its repair effect on medial meniscus defects in rabbits. Histological staining results of the AVM scaffold prepared by a self-designed triple (physical, chemical, and enzymatic) decellularization protocol were similar to those of normal meniscus. Collagen quantification confirmed no statistically significant difference in collagen…

Discoid Meniscus in Children: Why Meniscal Preservation and Repair Should Be the Goal in Modern Pediatric Knee Surgery.

Discoid meniscus is the most common congenital meniscal abnormality encountered in pediatric patients and frequently involves the lateral meniscus. While some discoid menisci remain asymptomatic, many children and adolescents develop pain, mechanical symptoms, or instability requiring surgical treatment. Historically, subtotal or total meniscectomy was commonly performed to address pain, mechanical symptoms, and functional limitation associated with symptomatic discoid meniscus. However, growing evidence regarding the biomechanical and long-term consequences of meniscal loss has led to a signi…

Simultaneous reconstruction of a medial meniscus posterior root tear and medial patellofemoral ligament reconstruction with a synthetic graft.

Patellar dislocations are common injuries, accounting for approximately 3% of all knee injuries. We present the case of an early 20s man who had sustained a traumatic patellar dislocation after a direct blow to the right knee. The dislocation was reduced, and MRI was conducted, which demonstrated concurrent posterior medial meniscal root (PMR) and medial patellofemoral ligament (MPFL) tear in the right knee. Following a repeat presentation of recurrent dislocation, the patient was surgically managed with arthroscopic simultaneous reconstruction of the PMR and the MPFL. This case demonstrated t

Volume Measurements of the Knee Articular Cartilage and Epiphyseal Bone for Evaluating the Structural Characteristics of the Discoid Lateral Meniscus.

Introduction Previous studies have identified morphological features in the bone and cartilage of knees with discoid lateral meniscus (DLM), but these investigations often depended on simple geometric variables like angles and linear dimensions. Three-dimensional (3D) bone and articular cartilage models can be used to obtain the volumes of knee structures. This study aimed to evaluate the volume of articular cartilage and epiphyseal bone in non-pathological knees and those with DLM and to assess their structural differences. Materials and methods This study included 19 magnetic resonance imagi…

Repair of radial tears of the lateral meniscus on a stable knee: Results at a minimum follow-up of 2 years.

INTRODUCTION: In younger patients, meniscal repair is recommended for isolated lateral meniscus tears that are most often due to acute trauma. But there is little published data on the outcomes of repairing this specific type of lesion. The goal of this study was to evaluate the clinical outcomes, report the failure rate of repairing radial tears of the lateral meniscus in stable knees and determine the risk factors for failure. MATERIALS AND METHODS: All patients who had a stable knee and underwent arthroscopic repair of a radial lateral meniscus tear between April 2013 and December 2019 wer…
