Recurrent vomiting is common in children with severe mental retardation and leads to significant morbidity with malnutrition, anemia, and aspiration pneumonitis. Spasms of the abdominal muscles and diaphragm, uncoordinated peristalsis, and central nervous system disorders are causes of dysphagia and continuous gastroesophageal reflux. It is desirable that mentally retarded children with vomiting have a barium swallow and esophagoscopy as early as possible. Fundoplication should be performed before complications develop. Spasms with aspiration followed by apnea, in particular, are life-threaten

A comparative experimental-psychological study of characteristics of psychic activity disorders was conducted in patients of 2 nosological groups: patients with schizophrenia and patients with astheno-neurotic disturbances in neurasthenia. The authors used methods directed at the study of characteristics of involuntary and voluntary attention as the direct expression and prerequisite of the activity of psychic processes. On the basis of the experimental data, the authors attempt to develop a psychological classification of characteristics of determinant factors of changes in psychic activity o

The homeless mentally ill represent a pivotal and urgent challenge to the mental health field in the 1980s. Those homeless who have extended histories of psychiatric hospitalization stand as harsh reminders of the failures of deinstitutionalization, while young mentally ill homeless adults who never have been treated as inpatients testify to the gaps and unrealized promises of community-based care under deinstitutionalization. Homelessness and mental illness are social and clinical problems, respectively, distinct in some ways but intertwined in others. Some of the factors that contribute to h

The clinical and laboratory investigation of the etiology of mental retardation is discussed. By clinically determining the stage of onset of the mental retardation, it is possible to dispense with a large number of special investigations. It is concluded that the laboratory is chiefly of value in confirming clinical suspicions, and that only rarely will random or routine testing yield an unexpected diagnosis. The possible exception is dermatoglyphic analysis in nondysmorphic, idiopathically retarded patients. This examination demonstrated probable antenatal causative factors in a high percent

ecent epidemiological studies in Swedish school age children revealed a prevalence of severe mental retardation (SMR = IQ less than 50) of 0.3% and of mild mental retardation (MMR = IQ 50-70) around 0.4%. In SMR prenatal causes were found in 55%, perinatal in 15-20%, no traceable brain pathology in 18%. Corresponding figures for MMR were 23%, 18% and 55%, respectively. Down syndrome was the largest single cause of SMR and polygenic subcapacity considered to be that of MMR. Chromosomal errors were detected among 29% SMR and 4% MMR school children. Fragile X accounted for 4% SMR and 10% MMR in b

Pathogenic factors in a mentally retarded population were evaluated by comparing their frequency among three groups of patients: a control group with predominantly genetic retardation and consanguineous parents; a group with severe idiopathic retardation and unrelated parents; and a group with mild idiopathic retardation and unrelated parents. Seven factors were found to be significantly more common among the patients with idiopathic retardation than in the genetic control group: a history of maternal reproductive inefficiency; bleeding during pregnancy; toxemia during pregnancy; signs of peri

When confronted with a patient with an alteration in mental status following surgery, the physician must consider both organic and psychosocial causes and their synergistic interactions. A careful medical assessment will rule out metabolic disorders, sepsis, cardiopulmonary complications, and adverse drug effects. A mental status examination and psychiatric history from the patient and family will identify psychosocial determinants. Simultaneous treatment of the organic and psychosocial components is essential if the physician hopes to provide the best patient care and maximize the likelihood

In a multicentered mental health system in which the population has a choice of several treatment facilities, common problems such as geographical distance and infrequent contacts between agencies lead to interagency distrust, unrealistic expectations about patient care, and fears of being overburdened by other agencies. These problems are compounded by the passive bureaucratic attitude which pervades many community mental health systems: unwillingness to do more than the minimum; cunning efforts to send difficult patients to other parts of the mental health system (such as "sending patients b
