All-cause and cause-specific mortality rates after severe extremity injuries among previously deployed active duty service members.

BACKGROUND: Previous research has shown that active duty military personnel who sustain extremity injuries while in service are at elevated risk for serious physical and psychological health issues that could affect their long-term functioning and quality of life yet longer-term mortality has not been studied in this population. OBJECTIVE: To determine whether rates of all-cause and cause-specific mortality are elevated for active duty U.S. service members who sustained traumatic limb injuries in service, compared to the broader population of deploying service members. To assess differences i

Characterization of spinal injuries sustained by American service members killed in Iraq and Afghanistan: a study of 2,089 instances of spine trauma.

BACKGROUND: This study sought to characterize spine injuries among soldiers killed in Iraq or Afghanistan whose autopsy results were stored by the Armed Forces Medical Examiner System. METHODS: The Armed Forces Medical Examiner System data set was queried to identify American military personnel who sustained a spine injury in conjunction with wounds that resulted in death during deployment in Iraq or Afghanistan from 2003 to 2011. Demographic and injury-specific characteristics were abstracted for each individual identified. The raw incidence of spinal injuries was calculated and correlations

Insights From Testicular Cancer Deaths in the U.S. Military: Case Reports.

Testicular cancer is the most common cause of cancer in the U.S. male active duty population. Advancements in surgical and chemotherapy approaches have led to a 95% 5-year survival rate across all stages. The United States Preventive Services Task Force recommends against non-symptomatic clinical screening or patient self-examination citing the high survival rate and overall low civilian incidence. We present 2 case reports of active duty individuals who died from complications of testicular cancer. Case 1 was found deceased. At autopsy, a pelvic mass was found disrupting the aorta along with

GABAA receptor subunit modulation reversed electrophysiological network alterations after blast exposure in rat organotypic hippocampal slice cultures.

Throughout training and deployment, some military service members are frequently exposed to shock waves due to blasts, and some complain of myriad neurological symptoms. In rat organotypic hippocampal slice cultures (OHSCs), blast-induced traumatic brain injury (bTBI) causes deficits in some electrophysiological measures, like long term potentiation, a neuronal correlate for learning and memory. In this study, we further characterized the alterations in the hippocampal network of OHSCs following a single moderate blast exposure. Connectivity and clustering coefficients were reduced across the

Survivable Deployments of Optical Sensor Networks against Multiple Failures and Disasters: A Survey.

Optical sensing that integrates communication and sensing functions is playing a more and more important role in both military and civil applications. Incorporating optical sensing and optical communication, optical sensor networks (OSNs) that undertake the task of high-speed and large-capacity applications and sensing data transmissions have become an important communication infrastructure. However, multiple failures and disasters in OSNs can cause serious sensing provisioning problems. To ensure uninterrupted sensing data transmission, survivability has always been an important research emph

Measuring Resilience to Operational Stress in Canadian Armed Forces Personnel.

Adaptability to stress is governed by innate resilience, comprised of complex neuroendocrine and immune mechanisms alongside inherited or learned behavioral traits. Based on their capacity to adapt, some people thrive in stressful situations, whereas others experience maladaptation. In our study, we used state-of-the-art tools to assess the resilience level in individuals, as well as their susceptibility to developing military stress-induced behavioral and cognitive deficits. To address this complex question, we tested Canadian Armed Forces (CAF) personnel in three distinct stress environments

Resting-state functional connectivity in combat veterans suffering from impulsive aggression.

Impulsive aggression is common among military personnel after deployment and may arise because of impaired top-down regulation of the amygdala by prefrontal regions. This study sought to further explore this hypothesis via resting-state functional connectivity analyses in impulsively aggressive combat veterans. Male combat veterans with (n = 28) and without (n = 30) impulsive aggression problems underwent resting-state functional magnetic resonance imaging. Functional connectivity analyses were conducted with the following seed-regions: basolateral amygdala (BLA), centromedial amygdala, anteri

Different types of combat experiences and associated symptoms in OEF and OIF National Guard and reserve veterans.

OBJECTIVE: It is well established that exposure to combat is a risk factor for posttraumatic stress disorder (PTSD). The experiences of military personnel serving in combat zones vary widely however, leading to increased attention to the impact of different types of combat trauma. The present study examined the relationships among 3 conceptually based categories of combat exposure with 4 PTSD symptom clusters (reexperiencing, avoidance, numbing, and hyperarousal) and symptoms of guilt, depression, and anxiety. METHOD: Participants were 206 National Guard and Reserve members who had recently r

Nutritional armor for the injured warfighter: omega-3 fatty acids in surgery, trauma, and intensive care.

Nutrients have traditionally been viewed as a means to provide basic energy for cellular homeostasis and amino acids for protein synthesis in all humans. Young, healthy men and women in the military today are presumed to be well nourished and mentally and physically fit to perform their duties in austere environments. Exposure to high-intensity projectiles, blast injuries, and other wounds of war, however, is an everyday occurrence during deployment that potentially challenges all homeostatic mechanisms. After sustaining such devastating injuries, critically ill, surgical, and trauma patients

Wearable Neurotechnology for the Treatment of Insomnia: The Study Protocol of a Prospective, Placebo-Controlled, Double-Blind, Crossover Clinical Trial of a Transcranial Electrical Stimulation Device.

Sleep disruption and deprivation are epidemic problems in the United States, even among those without a clinically diagnosed sleep disorder. Military service members demonstrate an increased risk of insomnia, which doubles after deployment. This study will investigate the ability of a translational device, Teledyne PeakSleep™ (Teledyne Scientific & Imaging, Durham, NC, USA), to reduce sleep onset latency and the time spent awake after sleep onset, with improvement in the subjective benefits of sleep for patients with insomnia by enhancing the brain rhythms within the frontal lobe implicated in

Trauma exposure and transdiagnostic distress: Examining shared and posttraumatic stress disorder-specific associations.

We examined transdiagnostic and posttraumatic stress disorder (PTSD)-specific associations with multiple forms of trauma exposure within a nationwide U.S. sample (N = 1,649, 50.0% female) of military veterans overselected for PTSD. A higher-order Distress factor was estimated using PTSD, major depressive disorder (MDD), and generalized anxiety disorder (GAD) symptoms as indicators. A structural equation model spanning three assessment points over an average of 3.85 years was constructed to examine the unique roles of higher-order Distress and PTSD-specific variance in accounting for the associ

A Decade of War: Prospective Trajectories of Posttraumatic Stress Disorder Symptoms Among Deployed US Military Personnel and the Influence of Combat Exposure.

Posttraumatic stress disorder (PTSD) is a common psychiatric disorder among service members and veterans. The clinical course of PTSD varies between individuals, and patterns of symptom development have yet to be clearly delineated. Previous studies have been limited by convenience sampling, short follow-up periods, and the inability to account for combat-related trauma. To determine the trajectories of PTSD symptoms among deployed military personnel with and without combat exposure, we used data from a population-based representative sample of 8,178 US service members who participated in the
