Secondhand Tobacco Smoke and Functional Impairments in Older Adults Living in the Community.

BACKGROUND/AIM: There has been no comprehensive examination of the potential association of SHS with broad functional limitation assessment … ry (PCS) of the Spanish version of the SF-12, frailty with a Deficits Accumulation Index (DAI), and mobility limitations with the Rosow-Breslau scale. Baseline exposure to SHS was assessed by serum cotinine, and past exposure was self-reported. Cross-sectional analyses were performed using linear and logistic regression models, whereas functional performance changes …

Impact of mobility restrictions on active aging; cross-sectional associations and longitudinal changes parallel to COVID-19 restrictions.

BACKGROUND: Meaningful activities can be done in or around home, but opportunities for participation and active aging decrease when moving in smaller areas. Active aging refers to having an active approach to life in line with one's goals, ability and opportunities. In adults over 75 years with different baseline neighborhood mobility levels, we studied active aging scores two years prior to and amid COVID-19, when governments restricted mobility of residents to slow the COVID-19 outbreak. METHODS: AGNES cohort data were collected in 2017-2018 and spring 2020. Individuals were queried about t

Specific slow tests are not mandatory in patients with extremely short standard (3.2 km/hr 10% slope) test durations during exercise oximetry.

AIM: To compare the transcutaneous oxygen pressure results observed in patients with severe walking limitation during standard procedures (3.2 km/hr, 10% slope) versus during a test performed at a low speed (2 km/hr, 10% slope). METHODS: In 31 patients, the decrease from rest of oxygen pressure (DROP) index was measured on both buttocks, both thighs and both calves during two consecutive tests on treadmill. The maximal walking time (MWT) and the minimal DROP values (DROPmin ) observed during the 2 tests were compared with t test. Correlation of DROPmin values during the slow and standard proc

The development and validation of a prototype mobility tracker for assessing the life space mobility and activity participation of older adults.

BACKGROUND: There is increasing interest in examining the life space mobility and activity participation of older adults in the community using sensor technology. Objective data from these technologies may overcome the limitations of self-reported surveys especially in older adults with age-associated cognitive impairment. This paper describes the development and validation of a prototype hybrid mobility tracker for assessing life space mobility and out-of-home activities amongst 33 community-ambulant older adults in Singapore. METHODS: A hybrid mobility tracker was developed by combining a p

How persons with transtibial amputation regulate lateral stepping while walking in laterally destabilizing environments.

BACKGROUND: Persons with lower limb amputation often experience decreased physical capacity, difficulty walking, and increased fall risk. To either prevent or recover from a loss of balance, one must effectively regulate their stepping movements. It is therefore critical to identify how well persons with amputation regulate stepping. Here, we used a multi-objective control framework based on Goal Equivalent Manifolds to identify how persons with transtibial amputation (TTA) regulate lateral stepping while walking without and with lateral perturbations. RESEARCH QUESTION: When walking in desta

The Association between Public Transportation and Social Isolation in Older Adults: A Scoping Review of the Literature.

Inadequate public transportation was recognized as a barrier to social participation, especially for older adults in rural communities and with mobility issues. Older adults will not benefit from opportunities to engage with their community and maintain social networks if they are unable to access them. The purpose of this scoping review was to make recommendations for further research and to summarize areas for improvement identified in the literature that will aid in the development of public transportation initiatives that can better address social isolation for older adults (≥ 55 years of

Walking ability to predict future cognitive decline in old adults: A scoping review.

Early identification of individuals at risk for cognitive decline may facilitate the selection of those who benefit most from interventions. Current models predicting cognitive decline include neuropsychological and/or biological markers. Additional markers based on walking ability might improve accuracy and specificity of these models because motor and cognitive functions share neuroanatomical structures and psychological processes. We reviewed the relationship between walking ability at one point of (mid) life and cognitive decline at follow-up. A systematic literature search identified 20 l

[Prevention of impaired mobility in the elderly in primary care: a brief report].

The loss autonomy in the aging population is a major public health issue. Mobility impairment, which precedes loss of autonomy, could yet be accessible to multi-modal and personalised programs to enhance balance and physical performances, and avoid loss of autonomy. Moreover, such preventive programs are likely to be more effective when the patients are taken in charge before presenting with difficulties in daily living. The aging demographics and the consequences of the loss of autonomy clearly indicate a need for better addressing these patients with lowering mobility, even though they will

[Usefulness of rehabilitation in inflammatory myopathies].

Rehabilitation, for a long time not recommended, seems today to hold a prominent place within the therapeutic arsenal of inflammatory myopathies. The difficulty of its evaluation, apart from the low prevalence of these diseases, lies in a triple heterogeneity: first that distinguishing the different forms of inflammatory myopathies and the phase where they are active; second, that concerning the endpoint considered to assess the efficiency of the intervention; lastly, the diversity of the rehabilitation programs that can be undertaken. Between 1993 and 2016, about 30 studies estimating the reh

The reliability and minimal detectable change of Timed Up and Go test in individuals with grade 1-3 knee osteoarthritis.

BACKGROUND: The Timed Up and Go (TUG) test is quick and easy tests to assess patients' functional mobility. However, its reliability in individuals with knee osteoarthritis (OA) has not been well established. The aims of this study were to determine the reliability and minimal detectable change of the TUG test in individuals with doubtful to moderate (Grade 1-3) knee OA. METHODS: Sixty-five subjects (25 male, 40 female), aged 45-70 years, with knee OA participated. Inter-rater reliability was assessed using two observers at different times of the same day in an alternating order. Intra-rater

Forensic issues at the extremes of weight: From frailty syndrome through frail obesity to morbid obesity.

It is well recognised that there has been a striking increase in average body size (measured by the body mass index) in a number of countries over recent years, such that it has been referred to as an obesity epidemic. This has been associated with increased numbers of these cases in the forensic mortuary with the associated practical difficulties in body handling and the complexities of identifying multiple comorbidities. At the other extreme of body weight lies the frailty syndrome which manifests with unintentional weight loss, decreased muscle mass (sarcopaenia), reduced physical strength

Quantification of Outdoor Mobility by Use of Accelerometer-Measured Physical Behaviour.

Hip fractures in older persons are associated with both low levels of daily physical activity and loss of outdoor mobility. The aim was to investigate if accelerometer-based measures of physical behaviour can be used to determine if people undertake outdoor walking and to provide reference values for physical behaviour outcomes related to outdoor mobility. Older persons (n = 245), ≥70 years, one year after hip fracture, participated. Six objective measures of physical behaviour collected by an activity monitor were compared with self-reported outdoor mobility assessed with the Nottingham Exten

Scurvy revealed by difficulty walking: three cases in young children.

Scurvy is rare in developed countries but is known to cause lower-extremity pain and refusal to ambulate in children. Since the discovery of the link between scurvy and dietary deficiency of ascorbic acid, there has been a substantial decrease in its prevalence and recognition. Here we describe 3 cases of scurvy in young children presenting with difficulty walking. Only 1 of 3 patients had gingival lesions at the initial presentation. Two cases underwent an extensive evaluation for hematologic and rheumatologic diseases before the diagnosis of scurvy was made. Dietary histories eventually reve

Developing the principles of chair based exercise for older people: a modified Delphi study.

BACKGROUND: Chair based exercise (CBE) is suggested to engage older people with compromised health and mobility in an accessible form of exercise. A systematic review looking at the benefits of CBE for older people identified a lack of clarity regarding a definition, delivery, purpose and benefits. This study aimed to utilise expert consensus to define CBE for older people and develop a core set of principles to guide practice and future research. METHODS: The framework for consensus was constructed through a team workshop identifying 42 statements within 7 domains. A four round electronic De

Mobility parameters in health applications--a literature review.

Independent mobility is linked to self-determined life and quality of life. [1] Since human mobility follows reproducible pattern [2], it may be useful to assess mobility in health applications. This article represents a literature review which considers the question which parameters represent mobility, how they are assessed (are tracking technologies used?) and their relation to health. There are several mobility parameters which were grouped into quantitative, qualitative and combined parameters. Especially combined parameters seem to be useful in health applications, which should motivate f
