Drugging unconventional targets: insights from Huntington's disease.

Classical targeted drug discovery is based on targeting druggable targets, typically kinases and receptors of which the function can be agonized or antagonized. This strategy meets difficulties in cases such as Huntington's disease (HD) and similar neurodegenerative disorders, where the pathological function of the protein causing the disease is not clear. HD is caused by mutant HTT protein (mHTT) containing an expanded polyglutamine (polyQ) stretch, but the function of mHTT and how mHTT causes HD are unknown, thus preventing efforts to screen for mHTT 'inhibitors'. However, HD is appealing fo

The prospect of molecular therapy for Angelman syndrome and other monogenic neurologic disorders.

BACKGROUND: Angelman syndrome is a monogenic neurologic disorder that affects 1 in 15,000 children, and is characterized by ataxia, intellectual disability, speech impairment, sleep disorders, and seizures. The disorder is caused by loss of central nervous system expression of UBE3A, a gene encoding a ubiquitin ligase. Current treatments focus on the management of symptoms, as there have not been therapies to treat the underlying molecular cause of the disease. However, this outlook is evolving with advances in molecular therapies, including artificial transcription factors a class of engineer

Novel therapeutic approaches to autosomal dominant polycystic kidney disease.

Autosomal dominant polycystic kidney disease (ADPKD) is an inherited disorder characterized by the progressive growth of renal cysts that, over time, destroy the architecture of the renal parenchyma and typically lead to kidney failure by the sixth decade of life. ADPKD is common and represents a leading cause of renal failure worldwide. Currently, there are no Food and Drug Administration-approved treatments for the disease, and the existing standard of care is primarily supportive in nature. However, significant advances in the understanding of the molecular biology of the disease have inspi

Usefulness of ancillary methods for diagnosis, prognosis and targeted therapy in thyroid pathology.

The development of molecular analyses for thyroid pathologies is on going. These analyses provide new diagnostic tools with the aim of accurately distinguishing malignant and benign thyroid tumors. They are particularly useful as most of them can be done preoperatively on thyroid fine-needle aspiration biopsy samples. Furthermore, molecular biomarkers may play a promising role since they are able to predict the prognosis of patients with thyroid tumors. Moreover, identification of molecular markers as well as a better understanding of thyroid carcinogenesis will help develop innovative targete

Mitochondria targeted therapeutic approaches in Parkinson's and Huntington's diseases.

Substantial evidence from both genetic and toxin induced animal and cellular models and postmortem human brain tissue indicates that mitochondrial dysfunction plays a central role in pathophysiology of the neurodegenerative disorders including Parkinson's disease (PD), and Huntington's disease (HD). This review discusses the emerging understanding of the role of mitochondrial dysfunction including bioenergetics defects, mitochondrial DNA mutations, familial nuclear DNA mutations, altered mitochondrial fusion/fission and morphology, mitochondrial transport/trafficking, altered transcription and

Metastatic pheochromocytoma and paraganglioma: focus on therapeutics.

Metastatic pheochromocytomas and paragangliomas are rare and challenging tumors. The tumor burden, combined with excessive catecholamine production, predispose to a broad spectrum of complications that range from spinal cord compression to any organ damage, all of which may lead to decreased quality of life and overall survival. Current therapies include surgery, systemic chemotherapy and radiopharmaceutical agents. Surgery is often a preferred therapy because it may cure or allow a long-term remission in patients with locoregional or isolated resectable distant metastases. Additionally, surge

The targets of curcumin.

Curcumin (diferuloylmethane), an orange-yellow component of turmeric or curry powder, is a polyphenol natural product isolated from the rhizome of the plant Curcuma longa. For centuries, curcumin has been used in some medicinal preparation or used as a food-coloring agent. In recent years, extensive in vitro and in vivo studies suggested curcumin has anticancer, antiviral, antiarthritic, anti-amyloid, antioxidant, and anti-inflammatory properties. The underlying mechanisms of these effects are diverse and appear to involve the regulation of various molecular targets, including transcription fa

Protein Tyrosine Phosphatases in Cancer: From Oncogenic Drivers to Therapeutic Targets.

Protein tyrosyl phosphorylation is a dynamic reversible regulatory process that is essential for virtually all aspects of cellular function. The dysregulation of protein tyrosyl phosphorylation is one of the main causes of human diseases such as cancer, diabetes, autoimmune disorders, and neurological diseases. Protein tyrosine kinases (PTKs) have paved the way towards defining the significance of protein tyrosyl phosphorylation in human health and disease. However, homeostatic control of protein tyrosyl phosphorylation is also regulated by protein tyrosine phosphatases (PTPs). The actions of

NLR inflammasome pathways: key targets for pathogenesis and therapy of metabolic diseases.

The NOD-like receptor (NLR) inflammasome system is an evolutionarily conserved intracellular surveillance network that responds to pathogen-associated molecular patterns (PAMPs), damage-associated molecular patterns (DAMPs), and metabolism-associated molecular patterns (MAMPs). Beyond the well-characterized NLRP3 inflammasome, accumulating evidence suggests that NLRP1, NLRP6, NLRC5, NLRP12, and NLRP2 are also implicated in metabolic disorders, including obesity, type 2 diabetes mellitus (T2DM), atherosclerosis, and metabolic dysfunction-associated steatotic liver disease (MASLD). This review s

Pediatric thyroid cancer: Distinct pathogenesis, emerging therapeutic strategies and long-term survivorship management.

Pediatric thyroid cancer is the most common endocrine malignancy in children and represents a distinct clinical entity compared with adult disease. Although children frequently present with advanced-stage disease, they typically achieve excellent long-term outcomes. This paradox creates a central therapeutic challenge: maximizing oncologic control while minimizing the lifelong consequences of overtreatment. This review aims to synthesize the evolving management paradigm for pediatric thyroid cancer, with a dual focus on implementing precision oncology for advanced disease and safely de-escalat

Adjuvant therapy in biliary tract tumors: how will immunotherapy fit in the future?

INTRODUCTION: Biliary tract tumors comprise a group of poor prognosis malignancies, most of them being diagnosed at an advanced stage. Standard treatments include surgery with curative intent and adjuvant chemotherapy. Even so, the benefit obtained is marginal, and relapses are frequent. AREAS COVERED: Here, we made a review of the literature trying to summarize all trials related to adjuvant therapy, as well as those yet to come. We also focused on detecting the unmet needs in this field, which may represent a change in the current strategy. EXPERT OPINION: A clear definition of resectabili

Insights into the therapeutic targeting of ASIC3 in pain: hurdles for clinical development.

INTRODUCTION: Acid-sensing ion channel 3 (ASIC3) is a promising therapeutic target for pain because of its unique peripheral expression profile in somatosensory neurons and channel properties for sensing mild extracellular acidosis associated with inflammation, ischemia, and metabolic changes. AREAS COVERED: As a highly sensitive acid sensor, ASIC3's activity is also enhanced by endogenous modulators in the inflammatory 'soup,' such as arachidonic acid and hypertonicity, and by cold temperatures. Furthermore, ASIC3 activation can be independent of acid changes by molecules such as lysophospha

Glypican-3-targeted therapy in hepatocellular carcinoma: biological insights and therapeutic strategies.

Glypican-3 (GPC3) is an oncofetal cell surface proteoglycan that is highly expressed in hepatocellular carcinoma (HCC) and largely absent from normal adult tissues, making it an attractive target for gene and cell therapies. A broad range of GPC3-directed strategies has been explored, including antibody-based agents, bispecific engagers, chimeric antigen receptor (CAR) and T cell receptor (TCR)-engineered immune cells, vaccine platforms, and emerging radiopharmaceutical approaches. However, durable clinical responses have remained limited. Here, we synthesize biological and translational evide

Targeting the Epigenetic-Immune Axis in Acute Myeloid Leukemia: Current Advances and Future Directions.

PURPOSE OF REVIEW: Acute myeloid leukemia (AML) exemplifies how malignant transformation can be driven by both epigenetic deregulation and genetic mutation. Aberrant DNA methylation, histone modifications, and chromatin remodelling cooperate to block myeloid differentiation, sustain leukemic self-renewal, and promote immune evasion. This review examines the molecular mechanisms underlying epigenetic dysregulation in AML and highlights recent advances in epigenetic and immune-based therapeutic strategies. RECENT FINDINGS: Recent studies have identified multiple druggable epigenetic regulators,
