Analysis of early morbidity and outcomes in macrosomic newborns.

INTRODUCTION AND AIM: Term macrosomic newborns (≥ 4000 g) born between 37 and 42 weeks of gestation face greater risks of early neonatal morbidity and mortality than term infants with appropriate birth weights. Our study aimed to evaluate these outcomes specifically in term macrosomic newborns. METHODS: From January 2022 to June 2023, we included singleton live-born infants with a birth weight ≥ 2500 g who were delivered at 37-42 weeks of gestation. The study group consisted of newborns with a birth weight ≥ 4000 g, while the control group was comprised of newborns with a birth weight of 2500…

Predicting Morbidity in Pancreaticoduodenectomy: A Focus on Nutritional Status.

Background and aim Pancreaticoduodenectomy (Whipple procedure) remains associated with high postoperative morbidity, including surgical site infections (SSIs) and pancreatic fistulas, despite advances in surgical care. Although the role of nutrition in pancreatic surgery is increasingly recognized, prospective data evaluating multiple nutritional indices in predicting postoperative morbidity remain limited. This study investigates the association between preoperative nutritional status and postoperative complications following pancreaticoduodenectomy. Methods A prospective observational study …

The anion gap as a predictive tool for morbidity and mortality.

Since the early 1970s, the anion gap has been a fundamental component of acid-base disorder diagnosis and workup. In the past decade, a vastly expanded and ever-increasing literature base has explored its utilization as a morbidity and mortality predictive parameter in various clinical conditions. This discussion will review its history/derivation and reference interval associated issues followed by a literature review of current studies focusing on its relatively recent use as a clinical predictive marker. An English language PUBMED search was performed to include all listings up through 12/3…

Postpartum Hemorrhagic Morbidities with Livebirth versus Stillbirth.

OBJECTIVE: ACOG publications on stillbirth or postpartum hemorrhage (PPH) do not consider stillbirth as a risk factor for postpartum hemorrhagic morbidity. This study aimed to ascertain the likelihood of composite maternal hemorrhagic outcome (CMHO) among individuals who delivered vaginally with livebirth versus a stillbirth. STUDY DESIGN: This was a retrospective cohort study of all parturients greater than 20 weeks gestation who delivered vaginally at a single level IV site within 24 months. Demographic differences and baseline PPH risks were analyzed. CMHO included any of the following: es…

Eosinopenia in bronchiectasis: A novel biomarker for morbidity and mortality.

BACKGROUND: The paradigm of bronchiectasis is shifting away from its exclusive characterization as a neutrophilic condition. Patients with bronchiectasis and high eosinophil levels have been found to have a specific phenotype, but the clinical effect of eosinopenia remains unclear. METHOD: A retrospective, single-center, observational study was conducted at a tertiary medical center. Adult patients under follow-up for bronchiectasis from January 2007 to August 2020 were categorized by blood eosinophil count (BEC) as follows: eosinopenia (<100 cells/µL), normal (100-299 cells/µL), and eosinoph

Significance of GNAS mutations for morbid obesity in children.

BACKGROUND: Hereditary forms of obesity are characterized by early severe heterogeneous manifestations of the phenotype along with a rapid progression to morbid obesity, primary due to pathogenic variants of certain genes. Most forms are characterized by moderate to severe neuropsychic developmental delays, dysmorphic features and organ-specific developmental anomalies. RESULT: We searched for hereditary causes of morbid obesity in children by exome sequencing. As a result, we have identified 5 variants in the GNAS locus, two of which were identified for the first time: NM_000516.7(GNAS):c.20

Co-Morbid Hypothyroidism and Liver Dysfunction: A Review.

The liver and thyroid hormones interact at multiple levels to maintain homoeostasis. The liver requires large adequate amounts of thyroid hormones to execute its metabolic functions optimally, and deficiency of thyroid hormones may lead to liver dysfunction. Hypothyroidism has been associated with abnormal lipid metabolism, non-alcoholic fatty liver disease (NAFLD), hypothyroidism-induced myopathy, hypothyroidism-associated gallstones and occasionally, interferon-induced thyroid dysfunction. NAFLD remain an important association with hypothyroidism and further studies are needed that specifica

The impact of gut microbiota on morbidities in preterm infants.

The gut microbiota undergoes substantial development from birth, and its development in the initial years of life has a potentially lifelong effect on the health of the individual. However, various factors can disrupt the development of the gut microbiota, leading to a condition known as dysbiosis, particularly in preterm infants. Current studies involving adults have suggested that the gut microbiota not only influences the gut but also has multidimensional effects on remote organs; these pathways are often referred to as the gut-organ axis. Imbalance of the gut microbiota may lead to the dev

[Treatment of ventral hernias in patients with morbid obesity].

The review is devoted to the treatment of ventral hernias in patients with morbid obesity. This issue is important due to significant number of such patients and no unambiguous clinical recommendations. The advantages of simultaneous surgery (with bariatric intervention) are obvious, i.e. lower risk of postoperative hernia incarceration and no need for re-hospitalization with another intervention. High risk of bariatric population makes it necessary to minimize surgery time and surgical trauma. A staged approach with reducing body weight surgically or conservatively before hernia repair is oft

Effect of Maternal Coronavirus Disease on Preterm Morbidities.

OBJECTIVE: Coronavirus disease (COVID-19) during pregnancy may have an impact on preterm morbidities due to the inflammatory nature of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Exposure to intrauterine inflammation could result in adverse consequences in preterm infants. We aimed to determine the effect of maternal coronavirus disease on preterm morbidities at a tertiary neonatal intensive care unit. STUDY DESIGN: This observational cohort study compared the clinical outcomes of preterm infants < 37 gestational weeks with and without maternal COVID-19. The study

Single anastomosis gastric bypass for morbid obesity.

SUMMARY: Carcinoid heart disease is a rare complication of carcinoid syndrome, resulting in right-sided valvular heart disease and subsequent heart failure due to long-term exposure to vasoactive substances. The management of this condition is complex, often requiring surgical intervention. Current perioperative regimens entail the use of prophylactic somatostatin analogs to prevent carcinoid crisis; however, regimens vary widely among practitioners and evidence supporting their efficacy in this clinical setting is mixed. This case report describes the perioperative management of a 65-year-old
