Purtscher's retinopathy is a traumatic angiopathy, most commonly caused by head and chest trauma. Retinal signs include white ischemic infarcts (cotton-wool spots or Purtscher flecken) and hemorrhages (dot and blot, preretinal, or flame). The prognosis for patients with decreased vision is unpredictable. We report one case of this disease developing 3 days after a motorcycle accident.

We present the case of a 39-year-old sportswoman who was involved in a motorcycle accident in 2012. She sustained a complex proximal femoral fracture that involved the neck and trochanteric region. We decided to preserve the natural bone stock and repair the fractures, adopting skeletal traction in the first stage and then implanting an intramedullary nail. Twenty months later the fracture was healed clinically and radiologically and the patient was able to walk and run without pain.

Positional dislocation of the pisiform repeatedly occurred in the right wrist of a 19-year-old man following a motorcycle accident. The pisiform repeatedly dislocated distal to the triquetrum when the wrist was extended to 45 degrees or more, and the pisiform was reduced to proper joint congruity after wrist flexion of 55 degrees or more. The patient had a background of generalized joint laxity. Resection of the pisiform resulted in a pain-free, fully functional wrist.

A 10-year study of the helicopter transfer of motorcyclists injured during the International Motorcycle Tourist Trophy races (TT) and the amateur Manx Grand Prix (MGP) is reported. A total of 266 riders was transferred from the scene of the accident to hospital by air. The overall mortality rate for those riders airlifted was 3.38 per cent. The figures are compared with similar emergency helicopter services.

Cases of head injury are reviewed in which riders wearing jockey skull caps have suffered impacts to the front, back or side of their helmets. The design and constructional materials of such helmets are assessed. Impact tests that simulate the accidents confirm the low energy absorption potential of some helmets for lateral impacts. Most pedal or motorcycle helmet designs afford better lateral impact protection.

METHOD: Using a case-control study design with New Zealand data, fatal injury risk curves for motorcyclists and car drivers were modeled. A total of 142 fatally injured drivers/riders (cases) and 58,000 control drivers/riders were studied. For motorcyclists, there were 13 cases and 194 controls.

We report herein an 11-year-old pedestrian struck by a motorcycle, who suffered subdural hematoma and aortic rupture and developed traumatic pseudoaneurysm. He was treated conservatively for the first 24 hours and submitted to stent placement occluding the aneurysm neck. There were no recurrences in the post-procedure period and the child was discharged after 21 days without sequelae. This report also reviews trauma mechanisms and management of such threatening lesions in the pediatric population.

Two asthmatic patients developed unilateral diaphragmatic paralysis from phrenic nerve injury, in one case following cervical chiropractic manipulation and in the other after a motorcycle accident. Both presented with increased dyspnea and orthopnea. Diagnosis, severity, and level of the lesion were established by neurophysiological methods, which are preferred to chest radiography and diaphragmatic ultrasonography. In spite of only partial electrophysiological recovery of the nerve, both patients were asymptomatic 1 year later.

Testicular dislocation is an uncommon injury after blunt scrotal trauma (with only 150 cases reported) that must be properly diagnosed and treat in order to avoid a testicular atrophy. We presented a case report of traumatic dislocation of right testicle after a motorcycle accident. This type of testicular injury should be included in the whole evaluation of every polytrauma patient and usually requires a surgical treatment. The aim of this report is to clarify some aspects of the management of such rare entity.
