[Surgically treated epidural hematoma of the spinal cord at the Th1-Th2 levels associated with arachnoid cyst].

In a man aged 28 clinical picture, myelography and computerized myelotomography demonstrated epidural haematoma at the level of Th1-Th2 vertebrae with associated arachnoidal cyst. Owing to initially erroneous diagnosis of transverse myelitis the haematoma was diagnosed only after 7 days and removed surgically with simultaneous evacuation of the cyst. The therapeutic result was minimal.

Cytomegalovirus-associated encephalomyelitis in an immunocompetent adult: a two-stage attack of direct viral and delayed immune-mediated invasions. case report.

BACKGROUND: It is clinically rare to find cytomegalovirus (CMV)-associated encephalomyelitis in immunocompetent adults. Here, we present the case of an adult patient who developed acute transverse myelitis that was followed by immune-mediated disseminated encephalomyelitis. CASE PRESENTATION: A 38-year-old man developed acute paraplegia with paresthesia below the level of the T7-8 dermatome. Both brain and spinal cord MRIs performed at admission appeared normal. Corticosteroid therapy was initiated, with the later addition of high-dose intravenous immunoglobulins. After polymerase chain react

Encephalitis and myelitis in tropical countries: Report from the Task Force on Tropical Diseases by the World Federation of Societies of Intensive and Critical Care Medicine.

Tropical diseases are those that occur primarily or solely in the tropics, and as such include infectious diseases that are particularly prevalent in hot, humid conditions. The incidence of encephalitis in tropical countries is reported to be as high as 6.34/100,000/year. The term encephalitis implies inflammation of the brain and includes the presence of encephalopathy with two and more of the following features: fever, seizures and/or focal neurological findings; a cerebrospinal fluid pleocytosis; electroencephalographic findings or abnormal neuroimaging suggestive of encephalitis. Transvers

Novel advances in the diagnosis and treatment of neuromyelitis optica: is there a need to redefine the gold standard?

Neuromyelitis optica (NMO) is an inflammatory disease of the central nervous system characterized by severe attacks of optic neuritis and myelitis and, unlike multiple sclerosis, was initially thought to spare the brain in the early stages. The term NMO spectrum disorder (NMOSD) was recently used to qualify restricted forms of the disorder, which include recurrent optic neuritis, relapsing transverse myelitis and some encephalitic/brainstem presentations associated with positive aquaporin4 antibodies. It was also recently found that other immunological targets such as myelin oligodendrocyte gl

Myelitis in systemic lupus erythematosus: clinical characteristics and effect in accrual damage. A single-center experience.

Objective The purpose of this study was to describe the clinical characteristics of acute transverse myelitis, including the time of their presentation, and to evaluate their effect on accrual damage in patients with systemic lupus erythematosus (SLE). Methods Patients with SLE who were hospitalized because of incident, noninfectious myelitis at our institute between January 1997 and December 2013 were identified. As a control group, we selected for each of the patients in the study group one SLE patient hospitalized at the closest date to the case due to other severe non-neuropsychiatric (NP)

Cytokine profile and case series of tract-specific myelitis following SARS-CoV-2 infection.

Background: The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is a potential trigger for central nervous system (CNS) autoimmune disorders. The most common type of spinal cord pathology following novel coronavirus infection is immune-mediated/autoimmune transverse myelitis (TM); however, there are also rare forms of spinal cord pathology ‒ tract-specific myelitis ‒ previously considered as non-autoimmune-originated. Methods: The current study includes case series of 5 patients with a rare type of myelitis with predominant involvement of the dorsal and lateral columns following c

[Mycoplasma pneumoniae encephalitis in immunocompetent child].

Mycoplasma pneumoniae is a common cause of community-acquired pneumonia, but this organism can also affect almost every organ system besides the lung. Neurological manifestations (meningitis, encephalitis, transverse myelitis, Guillain-Barrè syndrome) are the most frequent extrapulmonary complications of Mycoplasma pneumoniae. We report a case of a immunocompetent child affected by encephalitis caused by Mycoplasma pneumoniae.

A Rare Case of Eosinophilic Myelitis Due to Gnathostomiasis.

Eosinophilic myelitis is an important cause of transverse myelopathy and has to be considered in an appropriate clinical setting. Eosinophilic myelitis due to parasitic infection should be suspected in cases with cerebrospinal fluid (CSF) eosinophilia along with migratory serpiginous skin lesions and recent travel to endemic areas. We report a case with a 1-month history of fever followed by truncal paresthesias, erythematous creeping skin eruptions, and paraparesis with blood and CSF eosinophilia on a background history of consuming undercooked fish. Predictive value of MRI parameters in severity and recovery of first-episode myelitis in aquaporin-4 antibody disease.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) associated with aquaporin-4 antibodies (AQP4-Ab) typically causes longitudinally-extensive transverse myelitis (LETM). Few data exist about the association of MRI features with LETM attack severity and recovery. METHODS: AQP4-Ab positive NMOSD patients with a first myelitis attack were retrospectively identified and spinal MRI scans reviewed. Association of MRI features with EDSS scores at attack nadir and recovery was evaluated. RESULTS: 22 patients were included. Median nadir EDSS score was 8 (range 1 to 8.5). Nadir EDSS scores cor

When is neuromyelitis optica diagnosed after disease onset?

To diagnose neuromyelitis optica (NMO), the 2006 NMO diagnostic criteria is commonly used. However, adequate studies about the time course of NMO according to the criteria have been lacking. The aim of the study was to identify the interval between disease onset and diagnosis of NMO, as well as the clinical characteristics and time course, according to the 2006 NMO diagnostic criteria in Japanese patients with NMO. Clinical progression and time course of 43 Japanese patients with NMO who fulfilled the 2006 NMO diagnostic criteria with mean disease duration of 14.2 years were investigated retro

Seronegative Neuromyelitis Optica Spectrum Disorder in a Six-Year-Old Patient: A Case Report.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare demyelinating disease of the central nervous system (CNS) in children. Its main clinical manifestations are optic neuritis and myelitis. In most cases, it is associated with suggestive magnetic resonance imaging (MRI) and positive serology (AQP4-IgG). We report a six-year-old girl with acute asymmetric quadriparesis, bilateral decreased visual acuity, and sudden loss of sphincter control. Although AQP4-IgG testing was negative, MRI demonstrated findings consistent with NMOSD. Consequently, she received high-dose methylprednisolone, intra

Seronegative Neuromyelitis Optica Spectrum--the challenges on disease definition and pathogenesis.

Neuromyelitis optica spectrum disorders (NMOSD) are characterized by severe optic neuritis and/or longitudinally extensive transverse myelitis, and some brain lesions are also unique to NMOSD. Serum autoantibodies against aquaporin-4 (AQP4) are detected in most cases of NMOSD. However, some patients with NMOSD remain seronegative despite repetitive testing during attacks with highly sensitive cell-based assays. The differential diagnosis of NMOSD is not restricted to multiple sclerosis and it includes many diseases that can produce longitudinally extensive myelitis and/or optic neuritis. We re

Diagnosis, treatment, and functional outcomes for two adolescent female patients with lupus myelitis: a case report.

INTRODUCTION: Transverse myelitis is a rare neurologic complication associated with systemic lupus erythematosus (SLE), also known as lupus myelitis. Little is known about the optimal treatment regimen for the disease or the functional outcomes after diagnosis, especially for pediatric patients. METHODS: A retrospective case series at a large, academic pediatric tertiary care center was performed to describe the clinical presentation, diagnostic approach, early treatment, and functional outcomes in two pediatric patients diagnosed with lupus myelitis as a presenting sign of new-onset SLE. Discriminating long myelitis of neuromyelitis optica from sarcoidosis.

OBJECTIVE: To compare longitudinally extensive myelitis in neuromyelitis optica spectrum disorders (NMOSD) and spinal cord sarcoidosis (SCS). METHODS: We identified adult patients evaluated between 1996 and 2015 with SCS or NMOSD whose first myelitis episode was accompanied by a spinal cord lesion spanning ≥3 vertebral segments. All NMOSD patients were positive for aquaporin-4-immunoglobulin G, and all sarcoidosis cases were pathologically confirmed. Clinical characteristics were evaluated. Spine magnetic resonance imaging was reviewed by 2 neuroradiologists. RESULTS: We studied 71 patients

Paraneoplastic Neuromyelitis Optica Spectrum Disorder Associated With Ascending Colon Cancer Expressing Aquaporin-4 and Glucose-Regulated Protein 78.

Neuromyelitis optica spectrum disorder (NMOSD) is a central nervous system demyelinating disease, typically defined by the presence of optic neuritis and longitudinally extensive transverse myelitis. A 69-year-old woman presented with acute longitudinally extensive transverse myelitis and motor and sensory deficits. Laboratory testing confirmed the presence of aquaporin-4 (AQP4) antibodies, leading to a diagnosis of NMOSD. Her symptoms improved following methylprednisolone pulse therapy. Further evaluation revealed concurrent ascending colon cancer. Surgical resection of the tumor was performe

Neurological side effects of SARS-CoV-2 vaccinations.

SARS-CoV-2 and adverse reactions to SARS-CoV-2 vaccinations show a tropism for neuronal structures and tissues. This narrative review was conducted to collect and discuss published data about neurological side effects of SARS-CoV-2 vaccines in order to discover type, frequency, treatment, and outcome of these side effects. The most frequent neurological side effects of SARS-CoV-2 vaccines are headache, Guillain-Barre syndrome (GBS), venous sinus thrombosis (VST), and transverse myelitis. Other neurological side effects occur in a much lower frequency. Neurological side effects occur with any o

MRI of the first event in pediatric acquired demyelinating syndromes with antibodies to myelin oligodendrocyte glycoprotein.

Antibodies against the myelin oligodendrocyte glycoprotein (MOG-Ab) can be detected in various pediatric acquired demyelinating syndromes (ADS). Here, we analyze the spectrum of neuroradiologic findings in children with MOG-Ab and a first demyelinating event. The cerebral and spinal MRI of 69 children with different ADS was assessed in regard to the distribution and characteristics of lesions. Children with acute disseminated encephalomyelitis (n = 36) or neuromyelitis optica spectrum disorder (n = 5) presented an imaging pattern characterized predominantly by poorly demarcated lesions with a
