Peroral endoscopic myotomy followed by antireflux mucosectomy for gastroesophageal reflux in a rare case of Allgrove syndrome.

This report relates to two female children (identical twins) aged 13 years, one of whom presented with dysphagia, vomiting, nasal regurgitat … d shed tears since birth (Positive Schirmer’s test). Both successfully underwent peroral endoscopic myotomy and had good improvement in symptoms. At three months, both developed gastroesophageal reflux which needed antireflux mucosectomy in one and the other responded to medical therapy. This is a rare case of Allgrove syndrome with multisystem disease, a …

Clinical and nutritional correlates associated with weight changes in achalasia patients and the impact of laparoscopic Heller myotomy.

BACKGROUND: Achalasia is characterized by symptoms of esophageal obstruction, preventing food consumption. However, weight loss is observed only in a subset of patients, and data from literature is conflicting. AIMS: Our study aimed at evaluating predictors of weight loss in achalasia patients and at verifying the impact of treatment on nutritional status. METHODS: 123 achalasia patients, eligible for laparoscopic Heller myotomy, were studied. Demographic, clinical and nutritional data (calorie intake and macronutrient composition) were recorded at baseline and one-year post-treatment. Signi…

Frantz's tumor as incidental finding during Heller myotomy for achalasia: case report and review of literature.

INTRODUCTION: Solid pseudopapillary tumor is a low-grade malignancy of the pancreas and predominantly affects young women. This neoplasm is a rare pancreatic entity with vague clinical presentation. Diagnosis is often incidental through imaging or even during surgical approach for another condition. CASE PRESENTATION: A 22-year-old Brazilian female with gastrointestinal symptoms was diagnosed with achalasia and underwent Heller myotomy. Intraoperatory findings included an enlarging mass in the distal pancreas. During follow-up for the surgical approach of achalasia, a hypothesis of Frantz's t…

Repeat peroral endoscopic myotomy with simultaneous submucosal and muscle dissection as a salvage option for recurrent achalasia.

BACKGROUND: For recurrent achalasia after initial peroral endoscopic myotomy (POEM) failure, repeat POEM (Re-POEM) has been reported as a treatment option. However, severe esophageal interlayer adhesions caused by previous procedures impede the successful establishment of a submucosal tunnel and lead to aborted Re-POEM procedures. Our team previously described POEM with simultaneous submucosal and muscle dissection (POEM-SSMD) as a feasible solution for achalasia with severe interlayer adhesions. AIM: To investigate the effectiveness and safety of Re-POEM with simultaneous submucosal and musc…

Role of Peroral Endoscopic Myotomy in Advanced Achalasia Cardia With Sigmoid and/or Megaesophagus: A Systematic Review and Metanalysis.

BACKGROUND/AIMS: Sigmoid esophagus and/or megaesophagus are considered as an advanced stage in the natural history of achalasia cardia. The role of peroral endoscopic myotomy (POEM) in these subset of patients is emerging. We performed a systematic review and metanalysis to study the efficacy of POEM in advanced achalasia cardia with sigmoid and megaesophagus. METHODS: A literature search in PubMed and Embase was done from inception till August 3, 2021 to look for studies reporting exclusively on the role of POEM in advanced achalasia cardia with sigmoid and/or megaesophagus. The random effec…

Peroral endoscopic myotomy in a 9-year-old girl with achalasia and annular stenosis in the lower esophagus.

We present a case report of peroral endoscopic myotomy (POEM) in the treatment of pediatric patient with achalasia and annular stenosis in lower esophagus. A 9-year-old girl was newly diagnosed with achalasia. After assessment, the patient underwent POEM procedure and the retrievable stent was implanted in the esophagus. Postoperatively, lung CT suggested esophagopleural fistula, while gastroscopy showed that the metal stent was displaced to the esophageal lumen. A fistula was found in the lower esophageal segment after the stent was removed. Emergency operation of endoscopic perforation repai…

Influence of esophageal morphology on the clinical efficacy of peroral endoscopic myotomy in treating advanced achalasia cardia.

Peroral endoscopic myotomy (POEM) is the first-line treatment of achalasia cardia (AC). However, the efficacy of POEM in treating patients with advanced AC remains to be determined. The aim of the present study was to evaluate the feasibility and clinical outcome of POEM in treating patients with advanced AC involving different esophageal morphologies. The study was a single-center, retrospective analysis of patients suffering from advanced AC. The primary endpoint was the Eckardt score at the follow-up examination. Secondary endpoints were procedural-related details, including the operation t…

Histologic Findings in Mucosa and Muscularis Propria Biopsied During Peroral Endoscopic Myotomy in Patients With Achalasia.

BACKGROUND: Peroral endoscopic myotomy (POEM) has been increasingly used to treat achalasia. Previous studies have reported high frequency of muscular eosinophilic infiltration in achalasia. Esophageal mucosal changes in achalasia have only been studied in esophagectomy specimens. Cardia mucosal changes in achalasia have not been reported previously. We aimed to further characterize the esophageal, gastric cardia, and muscularis propria changes in achalasia. METHODS: This was a pilot study. Patients with clinically and radiographically confirmed achalasia who underwent POEM were enrolled in t…
