Small Femoral Nails and the Development of a Nonunion: A Case-Control Study.

BACKGROUND AND OBJECTIVE: Locked intramedullary (IM) nails are the primary treatment modality for femoral shaft fractures. This study aimed to determine the risk of developing a femur nonunion as a function of nail-to-bone diameter mismatch. MATERIALS AND METHODS: A retrospective case-control study was performed at a Level I trauma center. Adult patients with femoral shaft fractures (AO/OTA 32 classification) treated with IM nail fixation from 2016 to 2022 were identified from the institutional database. Cases required nonunion surgery. Controls were randomly selected in a 3:1 ratio and inclu…

The Relative Effects of Monotherapies for Psoriatic Nails: A Network Meta-Analysis Study.

BACKGROUND: Various treatments exist for nail psoriasis (NP). We determined the relative efficacy of various monotherapies through Bayesian network meta-analyses (NMAs). METHODS: We systematically reviewed the literature to identify eligible studies which-within the patient, intervention, comparator, outcome (PICO) context-determined the impact of biologic monotherapies on NP in terms of two outcome measures, namely, (1) the 16 to 24-week mean change in the Nail Psoriasis Severity Index (NAPSI) (i.e., outcome 1), and (2) the proportion who attained a Physician Global Assessment of fingernails…

Safety of Cement-augmented Femoral Cephalomedullary Nails: A Meta-analysis and Systematic Review.

Trochanteric fractures of the femur pose an increasing burden for elderly people. The standard treatment for these types of fractures includes cephalomedullary nailing, which can be augmented with cement. Although many studies have reported on the stability of this augmented construct, few studies have examined its clinical benefit and safety. Therefore, the objective of this meta-analysis is to examine the perioperative complications and postoperative mortality associated with cement-augmented nails in the management of intertrochanteric and pertrochanteric fractures of the hip. A search of P…

Clinical and Radiographic Outcomes of Small Caliber Intramedullary Nails for Tibial Shaft Fractures.

INTRODUCTION: Tibial shaft fractures, frequently treated with intramedullary nailing (IMN), are high-risk fractures of nonunion. The effect of intramedullary nail diameter on fracture union reduction remains an area of investigation, with many surgeons anecdotally preferring to place at least a 10-mm tibial nail. We hypothesized that small-caliber nails (SCNs) (diameter ≤9 mm) are safe to use and have no difference in complication rates compared with large-caliber nails (LCNs) (≥10 mm). METHODS: A retrospective study was conducted on patients with tibial shaft fractures undergoing reamed IMN …

Terry's Nails Without Systemic Disease: A Case Report of a Unique Clinical Sign.

Terry's nails are a clinical sign of apparent leukonychia, marked by proximal nail whitening and a preserved distal band, commonly linked to systemic diseases such as liver cirrhosis and chronic kidney disease. We report a case of a 26-year-old man with asymptomatic leukonychia of all fingernails and toenails since early childhood. Physical examination revealed features consistent with Terry's nails. A comprehensive systemic evaluation was unremarkable. This case demonstrates that idiopathic Terry's nails, though extremely rare, can occur in otherwise healthy individuals.

Ultrasound Evaluation of Cosmetic Changes in Nails: A Case Series and Literature Review.

BACKGROUND: The use of semi-permanent nail polishes and acrylic nails is rising due to cosmetic appeal, but may lead to underrecognized complications. High-frequency ultrasound (HFUS) offers a non-invasive method for assessing nail structural changes beyond clinical examination. OBJECTIVES: To describe ultrasound findings in patients with complications from semi-permanent polishes and acrylic nails and correlate them with clinical presentations. METHODS: This descriptive study included 13 women over six months who used semi-permanent polishes or acrylic nails and presented with nail alterati…

Comparative analysis of radiological outcomes among cephalomedullary nails: helical, screw and winged screw.

OBJECTIVE: Cephalomedullary nails (CMN) are implants with a high success rate in the surgical treatment of trochanteric fractures. The aim of this study is to compare the radiological outcomes and mechanical complications of femoral trochanteric fractures treated with three different CMNs. METHODS: Intertrochanteric fractures in patients aged 50 years and older treated with CMN between January 2016 and December 2021 were reviewed retrospectively. A total of 158 cases meeting the criteria were included to final analysis. Cases were divided into three groups based on the type of nail used (heli…

A tension system for angular correction of bent intramedullary nails: in vitro analysis.

PURPOSE: Although several techniques have been described for bent intramedullary nail removal, there is no universally accepted strategy. We hypothesized that a device based on the action principle of a three-point bend fixture could facilitate extraction of bent intramedullary nails; this paper describes its design and experimental testing. METHODS: Five large synthetic left femurs and five steel intramedullary nails were used. The nails were bent in a manual hydraulic press and implanted into the model femurs, which had been previously sectioned as necessary. A device to correct the angular…

[Impact of buried versus exposed flexible intramedullary nails osteosynthesis on pediatric forearm fractures].

UNLABELLED: In elastic stable intramedullary nailing (ESIN), there are different opinions among surgeons on whether to leave the nail buried in the same arm or to leave it exposed. OBJECTIVE: To determine the risk of re-fracture in patients with a nail buried directly into the amr or left exposed as a treatment for forearm fractures, and to investigate postoperative complications. PATIENTS AND METHOD: The study included 113 pediatric patients with a forearm fracture of both diaphyses. Two groups were formed according to whether the nail was buried (Group B, n: 53) in the same arm or left exp

Effect of the Intralesional Forms of Methylprednisolone Acetate and Methotrexate on Psoriatic Nails.

Individuals with psoriatic nails often have a lower quality of life relative to their counterparts with healthy nails. Methotrexate (MTX), an anti-neoplastic agent, is a longstanding treatment option for nail psoriasis. In the current study, we compared the effects of MTX to that of a corticosteroid, namely, methylprednisolone acetate (i.e., Depo-Medrol®) across individuals with nail psoriasis. We used a cohort study design, and both agents were administered intralesionally. Outcome variables were based on the Nail Psoriasis Severity Index (NAPSI). We quantified the effect in terms of change i…

The Use of Hindfoot Nails for Elderly Complex Distal Tibial and Ankle Fractures.

Elderly patients who sustain complex ankle or distal tibial fractures are often frail and comorbid and need surgery to contain the talus underneath the tibia in order to protect the soft tissue envelope and allow early unrestricted weightbearing. We performed a retrospective observational review of patients >65 years old who underwent a hindfoot nail fixation of an ankle or distal tibial fracture in our institution. Data collected included: injury sustained, open or closed injury, ASA grade, age at time of surgery, length of stay, postoperative mortality, complications, and further treatment.

[Pédiatrie - Internal lengthening nails: a novel method of lower limb reconstruction in children].

Intramedullary lengthening nails for lower limb reconstruction in children are a rapidly growing field and represent an alternative to external fixators. Today, two types of intramedullary lengthening nails are used: motorized (FITBONE) and magnetically driven (PRECISE) bone lengthening nails. Indications for limb lengthening nails are: 1) Significant lower limb length discrepancy; 2) Short stature and deformities due to musculoskeletal or dysplastic syndromes; 3) Constitutional short stature. Intramedullary lengthening surgery is challenging and represents a safe and reliable mode of treatmen…
