[Treatments of staghorn calculi].

The treatment of staghorn calculi is still difficult even though urolithiasis treatment can now be performed by endourology and extracorporeal shock-wave lithotripsy (ESWL) without open surgery. Because of the variety in volume and pattern of the staghorn calculi, especially with or without dilated renal pelvis and calyces, selection of treatment methods is important. The combined method of percutaneous nephrolithotomy (PNL) and ESWL has been highly effective in the complete removal of most staghorn calculi. Developments of the ESWL instrument and tools have increased the success of less invas

Therapeutic possibilities for urolithiasis in childhood.

OBJECTIVES: We present our case studies on paediatric urolithiasis, the techniques employed in its treatment and its results. MATERIAL AND METHODS: A retrospective study of paediatric urolithiasis of the upper urinary tract (UUT) treated at our centre between 2003 and 2014. We recorded demographic, clinical, diagnostic and therapeutic data and the complications. The therapeutic plan was recorded as isolated (extracorporeal lithotripsy, ureterorenoscopy, nephrolithotomy or surgery) or combined therapy. RESULTS: We examined 41 renal/urethral units in 32 patients. The median age was 5 years (ra

Clinical application of percutaneous nephrostomy in some urologic diseases.

Percutaneous nephrostomy was applied in some other urologic diseases and the efficacy was evaluated. Percutaneous nephrostomy for percutaneous nephrolithotomy (PNL) was performed in patients with various renal, perinephric and bladder diseases (n=79). The tract establishment, operation duration and complications were observed and the efficacy was assessed. The results showed that the tracts were successfully established in 79 cases. The operation lasted 4-20 min. 12F-16F single tract was established in nephrohydrop patients and 16F-20F single or multiple tracts were established in patients wit

Navigating the triple threat: Management of post-ESWL urinoma, steinstrasse, and obstructive uropathy: A challenging case report.

Urinoma (collection of urine) formation secondary to extracorporeal shock wave lithotripsy (ESWL) is one of the rare complications encountered postprocedure and co-existence of urinoma with steinstrasse is a rare and challenging scenario to manage in the setting of obstructive uropathy. ESWL is known for being noninvasive for breaking kidney and ureteric stones. Urinoma is a collection of urine outside the collecting system in the retroperitoneum or perirenal space. Early detection and management are necessary to prevent complications related to urinoma and steinstrasse. This case report is ab

From Prostate Enlargement to Bladder Stone: Large Bladder Stone as a Complication of Long-Standing BPH: A Case Study.

Vesicular calculi, or bladder stones, are a common occurrence in males of older age. Although there is a variety of different etiologies reported, one of the less common ones includes a background of benign prostatic hyperplasia (BPH). This case report describes a rare presentation of a large vesicular calculus in an old male patient that was diagnosed along with BPH. The patient, with a history of alcohol abuse and hypertension, previously had a percutaneous nephrolithotomy. Diagnosis was confirmed through transrectal ultrasound, x-ray Kidney-Ureter-Bladder, and cystourethroscopy. He underwen

Management of urinary stones in the patient with spinal cord injury.

The main objective of the urologist treating patients with spinal cord injury is to preserve renal function. In order to minimize morbidity, all attempts must be made to maintain a low bladder pressure, sterile urine, and a non-obstructed drainage system. These patients remain at increased risk of infection and stone formation despite all clinical intervention. Diligence in routine follow-up and careful evaluations will provide the best possible results. In the face of urolithiasis, ESWL, percutaneous nephrolithotomy, and chemolysis are effective means of treatment with acceptable morbidity. W

[Open surgery of calculus of the kidney pelvis. Results and complications in the era of extracorporeal lithotripsy].

From 1989 to 1993, 11,535 extracorporeal shock wave lithotripsy (ESWL) sessions were performed in 6,066 patients in our centre. In contrast, simple or extended pyelolithotomy was the treatment of choice in a small group of patients. The object of this paper is to review the results and complications of this type of operation. A total of 57 pyelolithotomies was performed, including 33 extended pyelolithotomies, in 55 patients (26 men and 29 women). The mean age of the series was 50 years (range: 5-77). The operated kidney was the left kidney in 34 cases and the right kidney in 23 cases. The mea

Two-year-old girl with impacted ureteral stone successfully treated with a single session of combined percutaneous nephrostomy and ureteroscopy.

Impacted stones frequently cause changes in the ureter, including edema of the ureteral wall, stone embedding in the ureteral mucosa or ureteral bending, which often preclude spontaneous passing of the stone and increase the risk of complications during surgery. When stone impaction is suspected preoperatively, management should be adapted accordingly. However, surgical treatment strategies remain controversial in pediatric patients because of the scarcity of cases reported. We describe the case of a 2-year-old girl with a right impacted ureteral stone who presented with gross hematuria and py

Fungaemia caused by obstructive renal candida bezoars leads to bilateral chorioretinitis: a case report.

BACKGROUND: Renal fungal bezoars are remarkably rare and mostly occur in immunodeficient patients. Only a small number of cases with immunocompetent patients have been published so far. The published treatment approaches comprised systemic antimycotic therapy and surgical or minimal invasive removal of the fungal balls. In some cases irrigation of the renal duct system with amphotericin B was performed. By obstruction of the urinary tract bezoars can lead to infected hydronephrosis and severe urosepsis with high lethality. Fungaemia can cause fungal colonization in different distant organs. Fu

Visceral organ-to-percutaneous tract distance is shorter when patients are placed in the prone position on bolsters compared with the supine position.

BACKGROUND AND PURPOSE: Percutaneous nephrolithotomy (PCNL) in the prone position is associated with a 0.1% risk of colon injury, yet there have not been any reported cases of colon injury with supine PCNL. The aim of the present study was to prospectively compare CT scans of patients performed in both supine and prone positions on bolsters. PATIENTS AND METHODS: Sixteen consecutive patients (mean age 55, 12 men) with 19 renal units (3 bilateral) who presented for PCNL underwent preoperative supine (without bolsters) and prone (with bolsters) noninfused CT scans. Two Forgotten Ipsilateral Double J Stents Managed in a Single Session: A Rare Endourologic Challenge.

Forgotten ureteral stents may lead to severe complications such as encrustation, infection, obstruction, and stone formation, especially when retained beyond 12 weeks. Dual forgotten stents in the same renal system managed in a single operative session are rare. A 51-year-old male with no urinary symptoms was incidentally found to have two forgotten left DJ (Double J) stents placed in 2021 and 2024. CT showed encrustation with multiple renal and ureteric stones. Renal function was preserved. A single-session tri-modality approach-cystolithotripsy, ureteroscopic lithotripsy, and lower-pole perc

Early Evaluation of Safety and Efficacy of Robotic-Assisted Combined IntraRenal Surgery with the MONARCHTM Platform.

BACKGROUND AND OBJECTIVE: Percutaneous nephrolithotomy (PCNL) is the standard of care for treating complex renal stones, but only a minority of urologists in the United States independently obtain percutaneous access. The MONARCH™ Platform, Urology, is an FDA-cleared robotic platform that may simplify percutaneous access and efficiently treat complex renal stones. In this study, we evaluate the clinical, procedural, and patient-reported outcomes using this technology. METHODS: In a preliminary prospective study in two institutions (University of Wisconsin and Indiana University), patients wer

Insights of stone-induced carcinogenesis of the urinary tract through biopsies of renal pelvic lesions during lithotomy.

INTRODUCTION: Nephrolithiasis is an established risk factor for upper tract urothelial carcinoma (UTUC), but the carcinogenic mechanisms remain unclear. PATIENTS AND METHODS: This study analyzed renal pelvic mucosal biopsies obtained during percutaneous nephrolithotomy (PCNL) or retrograde intrarenal surgery (RIRS) for stones, aiming to characterize the spectrum of stone-associated urothelial lesions and gain insights into lithogenic carcinogenesis. RESULTS: A total of 72 biopsies were performed on suspicious areas in 68 patients undergoing PCNL or RIRS. Histopathological results revealed 5

Urologic treatment of nephrolithiasis.

PURPOSE OF REVIEW: The purpose of this review is to describe the options and recent developments in the urologic treatment of nephrolithiasis in children. The prevalence of nephrolithiasis in children has increased over time. The urologic treatment of nephrolithiasis ranges from observation to medical expulsive therapy with an alpha blocker for a stone in a ureter to a variety of procedures for stone removal. RECENT FINDINGS: Observation for small, asymptomatic renal stones is a reasonable strategy in children. Medical expulsive therapy with an alpha blocker may have a beneficial effect for p
