Electromyography and nerve conduction studies of the lower extremity: uses and limitations.

Electrodiagnostic testing (EDX), the combination of electromyography and nerve conduction studies, is a useful extension of the physical exam. In the proper clinical context, it can help distinguish between the various nerve processes that may contribute to foot pain. This article discusses those processes, their clinical presentation, and the relative usefulness of EDX for each.

Insidious phrenic nerve involvement in postpolio syndrome.

A 49-year-old woman with amyotrophic sequelae of poliomyelitis experienced progressive left upper limb weakness and breathing discomfort while walking that had developed over one year prior to presentation. She had flaccid quadriplegia which was more marked in the left upper limb, with the C4- and C5-innervated muscles being most severely affected. Chest radiographs double exposed at maximal inspiration and expiration revealed poor respiratory movements in the left hemidiaphragm. Phrenic nerve conduction study demonstrated conspicuous diaphragmatic paralysis on the left side. The electrophysio

Carpal tunnel pressure alters median nerve function in a dose-dependent manner: a rabbit model for carpal tunnel syndrome.

BACKGROUND: An in vivo animal model for carpal tunnel syndrome (CTS) is presented which allows for graded application of pressure to the median nerve within the carpal canal. We hypothesized that such pressure would cause electrophysiologic changes in the median nerve in a dose-related manner, with NCS/EMG changes consistent with CTS in humans. METHODS: In 40 New Zealand white rabbits, ranging from 2 to 2.5 kg, angioplasty catheters were placed in the carpal tunnel in the forepaws and pressures ranging from 50 to 80 mmHg applied to one side while the contralateral side served as the control a

Peripheral neuropathy--a novel finding in dyskeratosis congenita.

We report the case of a 3.5-year-old boy who presented with truncal ataxia, microcephaly and delayed global development in infancy. Hypoplasia of cerebellum and corpus callosum and delayed myelination were found on brain MRI. Failure to thrive, sparse hairs and dystrophic nails became evident at the age of 2 years. He subsequently developed bleeding tendency, thrombocytopenia and hypocellularity on bone marrow examination leading to a diagnosis of dyskeratosis congenita. Impaired pain perception with slowing of nerve conduction velocities was demonstrated, suggesting a mild peripheral neuropat

A patient with Marfan's syndrome and neurofibromatosis type 1 with polyneuropathy.

Both Marfan's syndrome and neurofibromatosis type 1 are hereditary, autosomal dominant conditions. Here, we report the rare case of a patient fulfilling the clinical criteria for both diseases. In the absence of a family history of either of the two conditions, two independent de novo mutations are the most likely cause.

Usefulness of additional nerve conduction techniques in mild carpal tunnel syndrome.

This study was done to assess the percentage of abnormality in additional nerve conduction techniques after normal median distal latency (routine) in mild carpal tunnel syndrome (CTS). Bilateral nerve conduction studies were carried out in 116 consecutive symptomatic CTS patients (153 hands). Mild cases were based on normal routine (< 3.7 ms, peak-measured, 14 cm) and at least one technique abnormal of the following: sensory median-radial difference (MR); sensory median-ulnar difference (MU4); mixed palm median-ulnar difference (MUP); median palm latency (PW); and motor median distal latency (

An increased frequency of carpal tunnel syndrome in sarcoidosis. Results of a study based on nerve conduction study.

Sarcoidosis is a systemic disease which affects the musculoskeletal system; however, its association with carpal tunnel syndrome (CTS) has been reported only rarely. In our study, we tried to determine the prevalence of CTS in our consecutive sarcoidosis patients based on their symptoms and nerve conduction study (NCS). Fifty-six patients (mean age: 45.2) who were followed up with the diagnosis of sarcoidosis at our center and who gave informed consent for NCS were included into the study. The patients were questionned for the presence of hand symptoms: paresthesia, pain, nocturnal paresthesia

Electrodiagnostic challenges in the evaluation of lumbar spinal stenosis.

Taken together, the most common electromyographic finding in lumbar spinal stenosis is bilateral multilevel radiculopathy. The sensitivity of electrodiagnostic testing for radiculopathy is difficult to quantify because there is no criterion standard. Differentiating peripheral neuropathy and lumbar spinal stenosis on electromyography and routine nerve-conduction studies can be clinically challenging, especially when the two entities may be present simultaneously in older patients.

Quantitative response characteristics of thermoreceptive and nociceptive lamina I spinothalamic neurons in the cat.

The physiological characteristics of antidromically identified lamina I spinothalamic (STT) neurons in the lumbosacral spinal cord were examined using quantitative thermal and mechanical stimuli in barbiturate-anesthetized cats. Cells belonging to the three main recognized classes were included based on categorization with natural cutaneous stimulation of the hindpaw: nociceptive-specific (NS), polymodal nociceptive (HPC), or thermoreceptive-specific (COOL) cells. The mean central conduction latencies of these classes differed significantly; NS = 130.8 +/- 55.5 (SD) ms (n = 100), HPC = 72.1 +/

Selective measurement of digital nerve conduction velocity.

We developed a new method to measure the nerve conduction velocity of a single digital nerve. In 27 volunteers (27 hands), we separately stimulated each digital nerve on the radial and ulnar sides of the middle and ring fingers. A double-peaked potential was recorded above the median nerve at the wrist joint when either the radial-side nerve or the ulnar-side nerve of the middle finger was stimulated. The first peak of this potential had disappeared after the digital nerve was blocked under the stimulating electrodes, and the peak appeared again coinciding with the decrease of anesthesia. Shif

Common peroneal neuropathy due to surfing.

Common peroneal neuropathy is uncommon in children and adolescents. In this population, it is usually caused by direct nerve injury at the fibular head level. Most commonly, the nerve is damaged during sports-related blunt trauma. Other etiologies such as hereditary neuropathies and bone tumors are much less frequent. In some cases, repetitive microtrauma to the peroneal nerve is felt to cause neuropathy. We describe the case of a teenager who developed common peroneal neuropathy in association with prolonged wave-surfing in the presence of weight loss.

Electrodiagnosis in entrapment neuropathy by the arcade of Struthers.

Two cases of high ulnar nerve neuropathy are reported. Lesions were localized at the midarm level by electrophysiologic studies. In the first case, the lesion was found mainly to be a prolonged neurapraxia, and neurolysis was effective. The ulnar nerve was swollen 1 cm in length under the arcade of Struthers. After neurolysis, the palsy recovered rapidly. In the other case, the lesion seemed to be a mild injury to the myelin sheath. Delayed segmental conduction velocity and partial conduction block were found at the midarm level. The paresis improved slightly during the 11-month followup witho

Clinical, electrophysiological, and molecular genetic studies in a new family with paramyotonia congenita.

OBJECTIVES: To characterise the clinical and electrophysiological features and to determine the molecular genetic basis of pure paramyotonia congenita in a previously unreported large Irish kindred. METHODS: Clinical and neurophysiological examination was performed on three of the five affected family members. Five unaffected and three affected members of the family were available for genetic testing. Direct sequence analysis of the SCN4A gene on chromosome 17q, was performed on the proband's DNA. Restriction fragment length polymorphism (RFLP) analysis was used to screen other family members

Nerve conduction changes during lower limb lengthening. Somatosensory evoked potentials (SEPs) and F-wave results.

The effect of lower limb lengthening on nerve conduction was investigated in 5 achondroplastic subjects who underwent callotasis on a "cross-over" basis. Somatosensory evoked potentials (SEPs) and F waves from the posterior tibial nerve (PTN) were studied preoperatively and then after removal of the axial fixators. SEPs at the end of lengthening showed that both the latency of the plexus potential (P9) and, albeit to a lesser extent, the interpeak time between the plexus and the spinal cord (N15) potentials were significantly increased. The central conduction time (N15-P33) and the amplitude o

Muscle involvement in leprosy. Study of the anterior tibial muscle in 40 patients.

The involvement of skeletal striated muscle in leprosy is considered secondary due to peripheral neuropathy, but some studies point it to a primary muscle lesion. In order to investigate the muscle involvement in leprosy, we studied 40 patients (lepromatous 23, tuberculoid 13, borderline 2 and indeterminate 2). The motor nerve conduction of the peroneal nerves had a reduction of the velocity, decreased compound muscle action potential and sometimes absence of potentials. The electromyographic study of the anterior tibial muscle showed signs of recent and chronic denervation in 77.5% of the cas
