Heterogeneity in spasmodic dysphonia. Neurologic and voice findings.

Spasmodic dysphonia is a disturbance of phonation with laryngeal spasms. We report voice and neurologic examination findings in 45 subjects. Neurologic abnormalities were found in 32 subjects (71.1%). Rapid alternating movement abnormalities, weakness, and tremor were common. Incoordination and spasticity were rare. Lower extremity findings were frequent. Abnormalities were bilateral. Spasmodic dysphonia severity was related to age. Type, severity, and duration of vocal symptoms were not different for subjects with or without neurologic abnormalities. Vocal tremor was more frequent in neurolog…

Current clinical use and perceived impact of the sensory collapse test: A survey of peripheral nerve surgeons and hand therapists.

BACKGROUND: The Sensory Collapse Test (SCT) is a clinical maneuver used in the assessment of peripheral nerve compression. Despite being widely described, its actual use and perceived impact in clinical practice remain unclear. OBJECTIVES: To evaluate the current clinical use of the SCT, factors associated with its adoption, and its perceived influence on clinical decision-making among professionals involved in peripheral nerve care. METHODS: An international, web-based survey was conducted among clinicians involved in peripheral nerve surgery and hand therapy. The questionnaire assessed fam

Bedside Neurological Check Frequency Does Not Explain Outcomes for Patients With Coma and Disorders of Consciousness: A Curing Coma Campaign Scoping Review.

The frequency and nature of neurological exams (neuro-checks) in patients with severe acquired brain injury resulting in coma or disorders of consciousness (DoC) remain variable, with limited evidence guiding practice and poor understanding of their role in predicting and preventing neurological deterioration, functional recovery and adverse effects such as delirium. This scoping review aims to explore the frequency of bedside neurological exams within the first 7 days of injury impact on clinical outcomes in adult patients with severe acquired brain injury including mortality, neurological de

Tactile stimulation of the perigenital region during manual bladder expression improved the urine stream in cats affected by upper motor neuron injury.

OBJECTIVE: The aim of the study was to evaluate whether the tactile stimulation of the perigenital region together with manual bladder expression (MBE) facilitated the urine stream in cats with acute or chronic upper motor neuron injury (UMNI). ANIMALS: 34 cats with UMNI having urinary retention. METHODS: All the cats had a complete neurologic examination, which determined the localization of the UMNI between T3 and L3. They were classified as chronic UMNI if the injury had occurred more than 3 days previously. The cats were divided equally into 2 groups: the M group (n = 17) managed with on…

Assessment of neurological sequelae and new-onset symptoms in the long-term follow-up of paediatric Guillain-Barre syndrome: A longitudinal study from India.

BACKGROUND: Guillain-Barre syndrome (GBS) is the commonest cause of acute flaccid paralysis in children. There is a paucity of studies that assess the long-term outcome of paediatric GBS. AIM: To assess the frequency of neurological sequelae and the new-onset symptoms in the long-term follow-up of paediatric GBS and to identify the risk factors associated with them. METHODS: This longitudinal study involved 78 children with GBS treated between January 2015 and 2021. The parents of those children were contacted to visit the hospital for a detailed neurological examination and to look for new-

Nerve Ultrasound Protocol to Detect Dysimmune Neuropathies.

Nerve ultrasound is increasingly used in the differential diagnosis of polyneuropathy as a complementary tool to nerve conduction studies. Morphological alterations of the peripheral nerves, such as increasing the cross-sectional area (CSA), have been described in various immune-mediated polyneuropathies. The most prominent morphological changes in nerve ultrasound have been described for the chronic inflammatory demyelinating polyneuropathy (CIDP)-spectrum disease. CIDP may be distinguished from hereditary and other polyneuropathies by measuring the extent and pattern of nerve swellings (CSA

[Peripheral Nerve Disease].

The Diagnostic process of neurological disorders consists of anatomical and etiological diagnoses, and that of neuropathies usually starts with a suspicion of peripheral nerve disturbance based on neurological findings, which is subsequently confirmed by electrophysiology. However, there are numerous pitfalls in interpreting neurological features and electrophysiological data in the anatomical diagnosis process, possibly leading to misdiagnoses of "neuropathy" in patients with myelopathies, myopathies, neuromuscular junction diseases, or motor neuron diseases. We present the case of a 7-year-o

Mononeuropathies in pregnancy.

While signs and symptoms of peripheral neuropathy may appear to be similar among all patients, further evaluation both at the bedside and beyond demonstrate distinct differences in the pattern of certain neuropathies. A working knowledge of these differences and of the available tools to distinguish them is quite useful to the clinician. This chapter provides an overview of the distinction among various neuropathy profiles. Focal neuropathies may occur from compression or from entrapment. Neurologic examination aids in anatomic localization, which further refines and directs electrodiagnostic …

Exacerbation of neurological symptoms and COVID-19 severity in patients with preexisting neurological disorders and COVID-19: A systematic review.

BACKGROUND: Patients with chronic diseases likely develop severe 2019 coronavirus disease (COVID-19). However, little is known about the effects of COVID-19 on patients with neurological disorders. We conducted a systematic review to evaluate the severity of COVID-19 and its effect on neurological symptoms in patients with preexisting neurological disorder and COVID-19. METHODS: We searched the MEDLINE (PubMed) and medRxiv databases for reports of patients with both preexisting neurological disorders and COVID-19. Studies reporting data on changes in the symptoms of preexisting neurological d

The relationship between neurovascular coupling, vision and sensory performance, and concussion history in Special Operations Forces combat soldiers.

OBJECTIVE: To determine the relationship between neurovascular coupling (NVC), vision and sensory performance in Special Operations Forces (SOF) combat soldiers with and without concussion history. METHODS: We studied 61 SOF combat soldiers (male, age = 33.8 ± 3.7 years, n = 40 with concussion history [Median = 3; range = 1-10+]). We instrumented our participants with transcranial Doppler to quantify NVC response during reading and visual search tasks. All participants completed vision and sensory performance testing (Senaptec Sensory Station). We performed separate multiple regressions to de

Safe prognostication following cardiac arrest: The role of the pharmacokinetics of fentanyl in patients treated with targeted temperature management.

BACKGROUND: Neurological prognostication following cardiac arrest (CA) is complex and sedative agents may significantly impair responses to clinical examination. This study investigates the elimination of fentanyl in patients treated with targeted temperature management (TTM). METHODS: We measured the blood concentration of fentanyl in 23 post-cardiac arrest patients treated with TTM following discontinuation of continuous infusion. Fentanyl was discontinued when the patients were rewarmed to a temperature of 36-36.5 °C and a blood sample taken 12 h later. Measured concentrations were compare

Painful Myoclonus Triggered by Lateral Antebrachial Cutaneous Nerve Entrapment at the Brachioradialis Muscle.

Compression of the lateral antebrachial cutaneous nerve is a rare clinical entrapment syndrome often overlooked as an initial etiology of pain. We present a case of an episodic upper limb painful movement disorder (myoclonus) in a 16-yr-old adolescent girl with a remote history of a surgically stabilized supracondylar humeral fracture who was later found to have entrapment of the lateral antebrachial cutaneous nerve. The incidence of a painful myoclonus triggered by a peripheral nerve entrapment is unknown. Combining a history and physical examination, electromyography, nerve conduction studie

Impact of Sports-Related Subconcussive Injuries in Soccer Players.

Sports-related subconcussive impacts to the head are receiving increased interest. Recent evidence indicates that subconcussive impacts will have greater relevance across time because of the number of repetitive impacts. Soccer players are at risk of receiving at least one impact during a soccer game. The authors review the cognitive-communication functioning following subconcussive head injuries in youth and recommendations for baseline assessments and cognitive-communication dysfunctions after subconcussive impacts in youth. The review is followed by a description and discussion of a study t

Don't neglect the clock drawing test.

INTRODUCTION: A 63-year-old, right-handed woman with a history of hypertension presented to the ED with left arm paresis of 2 days duration. Three weeks before admission, she had flu-like symptoms with intermittent left arm weakness that had recovered briefly but recurred 2 days prior to her presentation. On neurological examination, GCS was 15 and cranial nerves' function was normal. Left upper limb strength was 4/5. There was left arm drift and pronation but the patient denied noticing any difference between the positions of her arms. Hyper-reflexion was presented in the left arm. The rest o
